138株金黄色葡萄球菌药物敏感试验结果分析

摘要 目的 了解首都医科大学附属北京胸科医院的结核与肿瘤患者中MRSA及甲氧西林敏感的金黄色葡萄球菌(MSSA)对抗生素的敏感和耐药情况,为临床治疗金黄色葡萄球菌提供依据.方法 按照《全国临床检验操作规程》有关要求进行实验,将138株金黄色葡萄球菌对14种抗生素进行药物敏感试验,并收集相关资料进行分析.结果 138株金黄色葡萄球菌分别来自于痰液(89株),伤口分泌物(19株),脓液(14株)和其他样本(16株).共检出MRSA108株,MSSA30株,未检测到对万古霉素耐药的金黄色葡萄球菌.MRSA对万古霉素、呋喃妥因、复方磺胺甲.(恶)唑等药物敏感率为100.0%、98.3%和70.4%,其余11种抗生素的药物敏感率均在0~30.2%之间,为多重耐药;14种抗生素中MSSA对12种药物敏感,敏感率在70.0%~100.O%之间.结论 MRSA对各类抗生素呈多重耐药性;MSSA有很好的药物敏感性.结核和肿瘤患者的免疫力较低,对金黄色葡萄球菌耐药比例较高,以后应加强耐药监测,临床要合理使用抗生素. Objective To study the sensitivity and resistance of methicillin-resistance Staphylococcus aureus (MRSA) and methicillin-susceptible Staphylococcus aureus (MSSA) isolated from patients with tuberculosis and tumor in Beijing Chest Hospital,and provide a basis for treatment of Staphylococcus aureus in clinic.Methods Drug sensitivity tests to 138 strains to 14 antibiotics were done according to the requirement of National Clinical Laboratory Procedures.The data was collected and analyzed.Results Among the 138 strains of Staphylococcus aureus (89 strains from sputum,19 strains from wound exudates,14 strains from pus and 16 strains from other specimen),108 strains were identified as MRSA and 30 strains as MSSA.Staphylococcus aureus resistant to vancomycin was not detected.The susceptibility rates of MRSA to vancomycin,nitrofurantoin,compound sulfanethoxazole were 100.0%,98.3% and 70.4%.The susceptibility rates of MRSA to other 11 kinds of antibiotics were between 0 and 30.2%,which were multiple drug resistant.The susceptibility rates of MSSA to 12 kinds of antibiotics were between 73.3% and 100.0% in 14 kinds of antibiotics.Conclusions MRSA is multiple drug resistance to many kinds of antibiotics.MSSA has good drug sensitivity.The immunity of patients with tuberculosis and tumor is low and the drug resistance rate of Staphylococcus aureus is high.The monitor of drug resistance should been strengthened and the antibiotics should been rational used in clinics.
