目的:探讨抑郁症患者的述情障碍.方法:研究对象为2004-06/10来自中南大学湘雅二医院精神卫生研究所及湖南省脑科医院的门诊及住院患者.用自编的一般情况调查表、多伦多述情障碍量表、汉密顿抑郁量表对抑郁症患者进行评估.结果:抑郁症男性患者的述情障碍量表分(63.65±9.15,t=2.43,P=0.01)高于一般人群(常模),而女性患者(51.29±9.03,t=5.95,P=0.00)低于一般人群,在作因子分析时发现,男性除外向性思维(男性=17.12±4.55,女性=17.89±4 29,F=1.33,P=0.13)这一因子分外,描述情感的能力因子分(男性=12.93±3.61,女性=10.33±3.39,F=3.93,P=0.04)、认识和区分情感与躯体感受的能力因子分(男性=21.59±5.86,女性=14.07±3 90,F=6.01,P=0.01)、幻想因子分(男性=12.01±3.12,女性=9.01±3.01,F=4.05,P=0.04)均高于女性;抑郁症患者述情障碍受到本人(F=-4.287,P＜0.05)和母亲的受教育程度(F=-3.798,P＜0.05)的影响,而受年龄、父亲受教育年限、家庭经济状况、抑郁严重程度的影响不大.在男性患者中,患者本人(F=-3.931,P＜0.05)和母亲的受教育程度(F=-3.907,P＜0.05)影响描述情感的能力;在女性患者中,患者本人的受教育程度不仅影响描述情感的能力(F=-4.554,P＜0.05),还影响认识和区分情感与躯体感受的能力(F=-4.601,P＜0.05)和幻想(F=-4.238,P＜0.05),而母亲的受教育年限则影响患者认识和区分情感与躯体感受的能力(F=-3.935,P＜0.05).结论:男性抑郁症患者可能存在述情障碍,述情障碍在抑郁症患者中可能存在性别差异,抑郁症患者述情障碍可能受到患者本人和母亲的受教育程度的影响.
AIM:To investigate the alexthymia of patients with major depressive disorder. METHODS:The subjects were the inpatients and outpatients from Institute of Mental Health,Xiangya Second Hospital of Central South University and Hunan Provincial Brain Hospital between June and October 2004.The patients with major depressive disorder were evaluated by self made General situation inventory,Toronto alexthymia scale(TAS)and Hamilton depression rating scale for depression(HRSD). RESULTS:The TAS scores of male patients(63.65±9.15) were higher than that of general population(norm)(t=2.43,P=0.01),while female(51.29±9.03) were lower than general population(t=5.95,P=0.00). In the analysis of the factors of TAS,it was found that mean score of all male factors except extroverted thoughts factor(male=17.12±4.55,female=17.89±4.29,F=1.33,P=0.13),such as ability of describing emotion(male=12.93±3.61,female=10.33±3.39,P=3.93,P=0.04),ability of being cognizant of and distinguishing emotion and body sensation(male=21.59±5.86,female=14.07±3.90,F=6.01,P=0.01),fantasy(male=12.01±3.12,female=9.01±3.01,F=4.05,P=0.04) were higher than those of female's.The patients' alexthymia was affected by their own(F=-4.287,P< 0.05) and their mothers'(F=-3.798,P< 0.05) school educational years,and did not being significantly affected by patients' age,fathers' school educational years,family economic status and severe degree of depression.In male patients,the ability of describing emotion was affected by their own(F=-3.931,P< 0.05) and their mothers'(F=-3.907,P< 0.05) school educational years.In female patients,their school educational years affected not only such as the ability of describing emotion(F=-4.554,P< 0.05),but also the ability of being cognizant of and distinguishing emotion and body sensation(F=-4.601,P< 0.05) and fantasy(F=-4.238,P< 0.05),while mother's school educational years affected the ability of being cognizant of and distinguishing emotion and body sensation(F=-3.935,P< 0.05).
