慢性阻塞性肺疾病患者与吸烟者的运动心肺功能对比研究 预览 被引量:17

A comparative study on cardiopulmonary exercise testing in patients with chronic obstructive pulmonary disease and smokers

摘要 目的比较慢性阻塞性肺疾病(COPD)患者、无症状吸烟者和健康对照者的运动心肺功能,探讨吸烟对心肺功能的影响.方法采用负荷递增运动方案,对20例COPD患者、20例无症状吸烟者及20例健康对照者进行心肺运动试验.结果极量运动时,最大运动负荷在健康组、吸烟组和COPD组分别为(140.8±34.8)W、(118.7±36.0)W和(96.0±46.3)W(P<0.05);最大摄氧量(VO2max)在健康组、吸烟组和COPD组分别为(2.005±0.386)L/min、(1.414±0.356)L/min和(1.001±0.514)L/min(P<0.01);最大氧脉搏(VO2/HRmax)在健康组、吸烟组和COPD组分别为(12.6±4.0)mL/min、(9.8±2.7)mL/min和(7.2±3.3)mL/min(P<0.05);最大分钟通气量(VEmax)在健康组、吸烟组和COPD组分别为(72.5±13.8)L/min、(63.7±13.1)L/min和(43.7±19.1)L/min(P<0.01);呼吸储备(BR)在健康组、吸烟组和COPD组分别为(40.1±10.3)%、(24.5±15.1)%和(11.9±8.4)%(P<0.01).且吸烟者VO2max/kg与其吸烟指数呈负相关(r=-0.66,P<0.01).结论吸烟者在尚无临床症状时,其运动心肺功能已明显降低,且吸烟量越大,其心肺功能损害越严重.COPD患者运动心肺功能降低更显著,其中以肺功能较心功能受损严重,说明肺功能损害是COPD患者运动能力降低的主要原因. ObjectiveThe cardiopulmonary exercise testing was performed in patients with chronic obstructive pulmonary disease (COPD),smokers and healthy volunteers and the results were comparaed in order to urge smokers to quit smoking.Methods20 patients with COPD and 20 asymptomatic smokers conducted the cardiopulmonary exercise testing with 20 healthy non-smokers as controls.The incremental test was used.ResultsThe maximal work in patients with COPD [(140.8±34.8)W] was lower than that in smokers[(118.7±36.0)W] and significantly lower than that in healthy volunteers [(96.0±46.3)W],respectively (P<0.05).The maximal volume of oxygen uptake (?O 2max) reduced from (2.005±0.386)L/min to (1.414±0.356)L/min(P<0.01) and (1.001±0.514)L/min(P<0.01).The maximal oxygen pulse (?O 2/HRmax) in patients with COPD were lower than those in smokers and healthy volunteers from (12.6±4.0)mL/min to (9.8±2.7)mL/min (P<0.05) and (7.2±3.3)mL/min (P<0.05)).The maximal minute ventilation volume ( Emax) decreased from (72.5±13.8)L/min to (63.7±13.1)L/min (P<0.05) and (43.7±19.1)L/min (P<0.01).The breathe reserve(BR) in patients with COPD were lower than that in smokers [(24.5±15.1)% vs (11.9±8.4)%,P<0.01],significantly lower than that in healthy volunteers (40.1±10.3)% (P<0.01). ?O 2max/kg in smokers were significantly negatively correlated with their smoke index (r=-0.66,P<0.01).ConclusionsThe cardiopulmonary exercise capacity of smokers has been declined before clinical symptom.The more heavily they smoke,the more markedly cardiopulmonary function is damaged.The cardiopulmonary exercise capacity of patients with COPD is significantly decreased and the lung function damage overweights heart function,so lung function damage is the main reason of exercise capacity limitation.
