中国南部和西南地区HCV感染的基因型分布特征 预览 被引量:9

摘要 目的了解中国南部和西南地区HCV感染基因型分布特点。方法用丙型肝炎病毒(HCV)5’非编码区ABC复合酶切分型的方法,对49例广州和重庆HCV感染者进行基因分型,并对检出的6a型产物进行测序。将所得序列与GenBank参考序列比对,并作同源性分析。结果49例样品中共检出4例6a型,序列分析表明这4例样品都具有6a型特征性,即第-145位的CA插入,并与6a型参考序列同源性最高。分型结果表明,34例广州样品中,1b型25例(72.53%)、2a型5例(14.71%)、3b型1例(2.94%)、6a型2例(5.88%)、1b和2a混合型1例(2.94%);15例重庆样品中1b型8例(53.33%)、2a型1例(6.67%)、3a型1例(6.67%)、3b型3例(20.00%)、6a型2例(13.33%)。结论广州和重庆两个城市的基因型分布都以1b型为主,并首次在广州检出6a型感染,但感染率低于2a型;而重庆则为2a、3a、3b和6a型等多种基因型分布的特点,说明我国南部和西南地区HCV基因型分布与中国大陆其他地区的流行模式存在差异,这可能与其地理位置及静脉药瘾者的迅速增多有关。 [Objectives] To investigate the hepatitis C virus (HCV) distribution in south and southwest of China. [Methods] The genotypes of 49 patients with chronic hepatitis C virus from Guangzhou and Chongqing were determined by Scheme ABC of RFLP based on 5' noncoding region (5'NCR). And the samples of type 6a were subjected to direct DNA sequencing and similarity analysis with the reference sequences from GenBank. [Results] 4 samples of genotype 6a were detected and their sequences of 5'NCR had CA insertion at -145 site, which was the characterization of HCV type 6a. Compared with the references from GenBank, the 4 sequences also showed the largest similarity with genotype 6a. Among 34 patients from Guangzhou, there were 25 (72.53%) type 1b, 5 (14.71%) type 2a, 1 (2.94%) type 3b, 2 (5.88%) type 6a and 1 (2,94%) mixed infection (lb+2a), Among 15 patients from Chongqing, 8 were type 1b (53.33%), 1 was type 2a (6.67%), 1 was type 3a (6.67%), 3 were type 3b (20.00%), and 2 were type 6a (13.33%). [Conclusions] Genotype lb was the most prevalent type in Guangzhou and Chongqing. In Guangzhou, type 2a was still the second predominant genotype and type 6a also existed. And in Chongqing (southwest), multiple HCV genotypes were present. Our results indicated that the HCV genotype distribution in south and southwest of China was different from other regions of mainland. The reason might be that these regions are close to Southeast A- sia and the rapid transmission of IDUs.
