肌电刺激联合盆底肌功能锻炼在治疗产后盆底功能障碍患者中的应用 预览 被引量:10

摘要 目的分析肌电刺激联合盆底肌功能锻炼在减少产后盆底功能障碍患者尿失禁发生中的应用。方法选取2016年1至12月汉川市人民医院妇产科收治的90例产后盆底功能障碍患者,分为A组和B组,两组各45例,A组行盆底肌功能锻炼,B组在A组基础上行肌电刺激治疗,并对比两组治疗后其临床疗效及对生活质量改善的影响。结果 B组盆底Ⅰ类肌纤维肌电压、手测盆底肌力、盆底Ⅰ类肌纤维疲劳度及盆底Ⅱ类肌纤维肌电压均较A组高,差异均有统计学意义(t值分别为3.02、3.95、2.86、2.86,均P〈0.05);B组治疗后其盆腔器官脱垂定量分类法(POP-Q)0度比例较A组高,差异有统计学意义(χ^2=6.94,P〈0.05),而Ⅰ度、Ⅱ度比例较A组低,差异均有统计学意义(χ^2值分别为5.25、5.89,均P〈0.05),B组尿失禁发生率较A组低,差异有统计学意义(χ^2=6.83,P〈0.05);治疗3个月、9个月的盆底功能影响问卷(PFIQ-7)、盆底功能障碍评分法(PFDI-20)评分均较A组低,差异有统计学意义(t=-3.85~-2.96,均P〈0.05)。结论肌电刺激联合盆底肌功能锻炼能够降低产后盆底功能障碍患者尿失禁发生率,改善患者术后盆底功能,进而有助于提高其生活水平,临床上值得进一步推广和应用。 Objective To analyze the application of myoelectricity stimulation combined with pelvic floor muscle training in reducing incidence of urinary incontinence(UI) in patients with postpartum pelvic floor dysfunction. Methods Ninety patients with postpartum pelvic floor dysfunction treated in obstetrics and gynecology department in Hanchuan People's Hospital were selected and divided into group A and group B with 45 cases in each group. Patients in group A received pelvic floor muscle training and those in group B underwent myoelectricity stimulation on basis of treatment received in group A. Clinical efficacy and improvement of quality of life were compared between two groups after treatment. Results Pelvic floor type I muscle fiber voltage,hand measured pelvic floor muscle force,pelvic floor type Ⅰmuscle fiber fatigue and pelvic floor type II muscle fiber voltage in group B were higher than those in group A,and the differences had statistical significance(t value was 3. 02,3. 95,2. 86 and 2. 86 respectively,all P〈0. 05). Ratio of 0 degree classified by pelvic organ prolapse quantification(POP-Q) in group B was higher than that in group A,and difference was statistically significant(χ~2= 6. 94,P〈0. 05),but ratios of Ⅰ and Ⅱ degree in group B were lower than those in group A with statistically significant differences(χ~2value was 5. 25 and 5. 89,respectively,both P〈0. 05). Incidence of UI in group B was lower than that in group A and difference was statistically significant(χ~2= 6. 83,P〈0. 05). Scores of pelvic floor distress inventory(PFDI-20) and of pelvic floor impact questionnaire(PFIQ-7) in group B after three and nine months of treatment were lower than those in group A,and differences had statistical significance(t value ranged from-3. 85 to-2. 96,both P〈0. 05).
