嗜麦芽窄食单胞菌感染危险因素和耐药性分析 被引量:1

Analysis on risk factors and antibiotic resistance of nosocomial infection caused by Stenotrophomonas maltophilia

摘要 目的了解某医院重症医学科嗜麦芽窄食单胞菌感染相关危险因素和耐药现状。方法以2014年1月-2017年10月90例嗜麦芽窄食单胞菌感染患者为观察组,同期30例未感染患者作为对照组,对一般资料、抗菌药物应用和抗菌药物耐药性进行回顾分析。多因素logistic回归分析筛选出导致该菌感染的危险因素。结果住院时间、使用3种以上抗菌药物与嗜麦芽窄食单胞菌感染呈正相关,OR值分别为1.061,7.085,95%CI均〉1.000。药敏试验结果显示,该菌对米诺环素耐药率最低,其次为头孢哌酮/舒巴坦和复方磺胺甲噁唑,对左氧氟沙星耐药率为33.6%,对亚胺培南西司他丁耐药率达100%。结论住院时间、使用3种以上抗菌药物为该菌感染的独立危险因素,嗜麦芽窄食单胞菌对多种抗菌药物耐药,根据药敏调整抗菌药物的使用有利于控制该菌引起的感染。 Objective To investigate the associated risk factors and antibiotic resistance of nosocomial infection caused byStenotrophomonas maltophilia. Methods We retrospectively analyzed the general situation,antibiotic use and antibiotic resistance in 90 patients with Stenotrophomonas maltophilia infection from January 2014 to October 2017 in our hospital. 30 patients with no Stenotrophomonas maltophilia infection in the same period were selected as the control group. Multivariate logistic regression an alysis screened out the risk factors of Stenotrophomonas maltophilia infection. Results Multivariate logistic regression results showed that hospitalization days( OR = 1. 061,P〈0. 05) and the use of more than three kinds of antibiotics( OR = 7. 085,P〈0. 05) were positively correlated with Stenotrophomonas maltophilia infection. Antimicrobial susceptibility showed the lowest resistance rate to minocycline,followed by cefoperazone/sulbactam and sulfamethoxazole,and the drug resistance rate for levofloxacin was 33. 6%. The drug resistance rate for imipenem cilastatin could reach up to100%. Conclusion Hospitalization days and the use of more than three kinds of antibiotics may be independent risk factors of Stenotrophomonas maltophilia infection. Stenotrophomonas maltophilia is resistant to many antibiotics. Adjusting antibiotic use based on antimicrobial susceptibility may be useful in controlling Stenotrophomonas maltophilia infection.
