目的探讨磁共振三维成像技术：容积重建（VR）及最大密度投影（MIP）在胰腺分裂（PD）诊断中的应用。方法回顾性分析近4年MRCP检查诊断PD的8例患者的临床及影像资料。对8例PD的3D-MRCP薄层源数据进行VR及MIP重建,三维重建技术包括直接三维成像法（3D直）及组织结构选择加减法（3D加）。对2DMRCP、3D直-MIP、3D加-VR及3D加-MIP对胰管显示的数量及图像的清晰度、背景抑制及运动伪影进行分析对比。结果 3D加-VR诊断了8例PD,3D加-MIP诊断了7例,2D-MRCP与3D直-MIP均诊断了4例。3D加-VR显示了12处胰管,3D加-MIP显示了10处,2D-MRCP与3D直-MIP仅显示了5处。结论 3D加-VR显示胰管的形态及清晰度优于3D-MIP。3D加-VR不仅增加了PD的诊断率,还提高了显示率及图像质量。
Objective To explore the application of three dimensional magnetic resonance imaging（ 3D-MRI） ： Volume Rendering（ VR） and maximum intensity projection（ MIP） in the diagnosis of pancreas divisum（ PD）. Methods Clinical and imaging of 8 of PD patients were collected and analyzed from 2013 to 2016 by magnetic resonance cholangiopancreatography（ MRCP）. VR and MIP reconstruction were performed on 8 cases. 3D techniques include 3D-straight（ 3DS） and 3Dplus（ 3DP）. The 2 D-MRCP、3DS-MIP、3DP-VR and 3DP-MIP images were analyzed and compared. Results 8 cases were diagnosed by 3DP-VR,7 cases were diagnosed by 3DP-MIP,4 cases were diagnosed by 2D-MRCP and 3DS-MIP. 3DP-VR showed 12 pancreatic ducts,3DP-MIP showed 10 pancreatic ducts,2D-MRCP and 3DS-MIP showed 5 pancreatic ducts.Conclusion 3DP-VR shows the shape and definition of pancreatic duct better than 3D-MIP. 3DP-VR not only increases the diagnostic rate of PD,but also improves the displaying rate and the image quality.
