目的 探究约束理论优化军人高危产妇门诊管理的应用研究。方法 选取2016年3月至2018年3月于某院产科门诊就诊的军人高危产妇164例为研究对象,随机分为研究组和对照组各82例,其中研究组患者采取约束理论优化军人高危产妇门诊管理,对照组采取常规管理办法。统计两组孕妇对以往产科门诊状况的调查问卷,产科门诊管理满意度调查,在孕期结束后的妊娠结局。结果 调查问卷结果显示在门诊流程管理、检查环节及人为因素等方面存在不足。研究组产妇对军人高危产科门诊管理满意度为98. 8%,对照组产妇为87. 8%,差异具有统计学意义（P〈0. 05）;研究组的产妇妊娠各类不良事件发生率为1. 22%,显著低于对照组的8. 54%,差异具有统计学意义（P〈0. 05）。结论 约束理论可发现并改善军人高危产妇门诊管理过程中存在的瓶颈,通过不断优化和再造军人高危产妇门诊管理体系,显著提升军人高危产妇满意度并降低妊娠不良事件的发生率,不断提升医院为军服务的竞争力和内涵。
Objective To explore the application of military outpatient management of high risk maternity based on theory of constraints. Methods Select 164 military maternity women who had undergone high-risk management in our hospital from March 2016 to March 2018, and randomly divided them into 82 cases in study group and control group, in which the study group adopted TOC management measures. The control group adopted routine management measures. We counted two sets of pregnant women＇s questionnaires on the status of past obstetric clinics, obstetric clinic management satisfaction surveys, and pregnancy outcomes after the completion of pregnancy. Results The survey results showed that there were deficiencies in the management of outpatient procedures, inspection procedures, and human factors. Patients in the study group had 98. 8% satisfaction on outpatient service management and 87. 8% in the control group（P〈0. 05）. The prevalence of adverse events in the study group was 1. 22%, which was significantly lower than 8. 54% in the control group. （P〈0. 05）. Conclusion The TOC can find and improve the bottleneck in the management of high-risk maternal outpatients in the military. Through continuous improving and reengineering of military high-risk maternal outpatient management systems, it significantly improves the satisfaction of military high-risk maternals and reduces the incidence of adverse pregnancy events. It significantly improves the competitiveness and content of the hospital＇s military service.
