糖尿病肾病并发泌尿系感染的病原菌分布分析及耐药性检测 预览

摘要 目的探析糖尿病肾病并发泌尿系感染的病原菌分布及耐药性情况,为临床医师选择合理治疗方案提供参考依据。方法选择2016年1月至2017年12月时间段内在本院接受治疗的糖尿病肾病并发泌尿系感染79例作为研究对象,收集清晨中段尿尿液样本,染色、鉴定后,镜检复核,分析检验结果。结果79例糖尿病肾病并发泌尿系统感染患者尿液样本经培养分离出菌株121株,革兰阳性菌30株,革兰阴性菌85株,真菌菌株6株;革兰阳性菌以金黄色葡萄球菌56.67%构成比最高,革兰阴性菌以大肠埃希菌45.88%构成比最高;金黄色葡萄球菌耐药最高抗菌药物为青霉素G82.35%,大肠埃希菌耐药最高抗菌药物为氨苄西林94.87%。结论糖尿病肾病并发泌尿系感染的病原菌以革兰阴性菌为主,建议根据药敏试验结果,选择合适的抗菌药物,在满足临床需求的前提下,降低耐药菌发生风险。 Objective To explore the distribution and drug resistance of pathogenic bacteria in diabetic nephropathy complicated with urinary tract infection,and to provide reference for clinicians to choose reasonable treatment.Methods 79 cases with diabetic nephropathy complicated with urinary tract infection treated in Shenyang Weikang Hospital from January 2016 to December 2017 were selected as the study subjects.Urine samples collected from the morning urine were stained and identified.Microscopic examination was performed to check the results.Results 121 strains of bacteria,30 strains of Gram-positive bacteria,85 strains of Gram-negative bacteria and 6 strains of fungi were isolated from urine samples of 79 patients with diabetic nephropathy complicated with urinary tract infection.Gram-positive bacteria were the highest proportion of Staphylococcus aureus(56.67%).Gram-negative bacteria were the highest proportion of Escherichia coli(45.88%).Staphylococcus aureus has the highest resistance to penicillin G(82.35%).Escherichia coli is the most resistant drug to ampicillin(94.87%).Conclusion Gram negative bacteria are the main pathogens of diabetic nephropathy complicated with urinary tract infection.We suggest that appropriate antibiotics should be selected according to the results of drug susceptibility test to reduce the risk of drug-resistant bacteria on the premise of meeting the clinical needs.
