四妙散加减对比秋水仙碱治疗痛风疗效和安全性的Meta分析 预览

Meta-analysis of the Efficacy and Safety of Simiao Decoction on Gout Compared with Colchicine

摘要 目的系统评价四妙散加减对比秋水仙碱治疗痛风的疗效和安全性,为临床治疗痛风提供循证依据。方法计算机检索CNKI、万方数据库、VIP数据库、PubMed、Medline、Embase等数据库,收集单用四妙散加减治疗痛风(实验组)对比单用秋水仙碱(对照组)治疗痛风疗效[总有效率、显效率、血尿酸(UA)]和安全性的临床随机对照试验(RCT)。用改良Jadad量表评价纳入研究的方法学质量,并提取有效数据,采用RevMan5.3软件进行Meta分析。结果共纳入16项RCTs,合计1355例患者。结果四妙散加减治疗痛风在总有效率、显效率及降低UA水平方面,疗效较单用秋水仙碱有一定优势,差异具有统计学意义(P<0.05);且不良反应发生率较秋水仙碱低,差异具有统计学意义(P<0.05)。结论四妙散加减治疗痛风性关节炎较秋水仙碱有一定优势,且不良反应发生率较低。 Objective: To systematically evaluate the efficacy and safety of Simiao Decoction on gout compared with colchicine,and to provide evidence-based basis for clinical treatment of gout. Methods: CNKI,Wanfang database,VIP database,PubMed,Medline,Embase database were searched by computer. Randomized controlled clinical trials(RCT) were collected to compare the efficacy(total effective rate,significant efficiency,UA of serum uric acid) and safety of colchicine(the control group) and Simiao Decoction(the experimental group) on gout. The methodological quality of the study was evaluated with the modified Jadad scale,and the valid data were extracted. Meta-analysis was carried out with RevMan 5.3 software. Results: 16 RCTs were included,involving 1 355 patients. Compared with colchicine,Simiao Decoction had certain advantages in total effective rate,marked efficiency and UA level reduction in gout treatment,with statistical significance(P < 0.05). The incidence of adverse reactions was lower than that of colchicine,with statistical significance(P < 0.05). Conclusion: Simiao Decoction has certain advantages over colchicine on gouty arthritis,and the incidence of adverse reactions is low.
