振幅整合脑电图与新生儿神经行为评分在足月窒息新生儿的应用分析 预览

Application Analysis of Amplitude Integrated Electroencephalogram and Neonatal Behavioral Neurological Assessment in Neonates with Full-term Asphyxia

摘要 目的:通过收集并分析足月窒息新生儿的临床资料、振幅整合脑电图(aEEG)及新生儿神经行为评分(NBNA)结果,分析aEEG监测与NBNA检查之间的相关性,为早期诊断新生儿脑损伤提供依据。方法:对2014年6月-2017年6月深圳市第二人民医院新生儿科收治的足月窒息新生儿61例,按照窒息程度分为轻度窒息组36例及重度窒息组25例,同时选取40例无脑损伤高危因素的新生儿作为正常对照组。对三组新生儿进行aEEG监测及NBNA检查,对相关检查指标进行统计学分析。结果:三组新生儿在aEEG监测及NBNA检查的异常率比较差异均有统计学意义(P<0.05),其中重度窒息组两项检查的异常阳性率明显高于正常对照组及轻度窒息组,而轻度窒息组阳性率与正常对照组差异亦有统计学意义(P<0.05)。aEEG监测与NBNA检查之间存在相关性(r=0.332,P=0.001)。结论:aEEG联合NBNA评分对于足月窒息新生儿的早期脑损伤存在重要的价值,能够早期提示足月新生儿窒息所致的脑损伤。 Objective:To analyze the correlation between amplitude integrated electroencephalogram(aEEG)monitoring and neonatal behavioral neurological assessment(NBNA)examination by collecting clinical data,aEEG and NBNA of full-term asphyxiated neonates,and to provide evidence for early diagnosis of neonatal brain injury.Method:From June 2014 to June 2017,61 cases of full-term asphyxia neonates were divided into mild asphyxia group(36 cases)and severe asphyxia group(25 cases)according to the degree of asphyxia.Forty neonates without high risk factors of brain injury were selected as normal control group.The aEEG monitoring and NBNA were performed on the three groups of neonates,and the related indexes were statistically analyzed.Result:There were significant differences in abnormal rates of aEEG monitoring and NBNA test among the three groups(P<0.05).The abnormal positive rates of two tests in severe asphyxia group were significantly higher than those in normal control group and mild asphyxia group,there were also significant differences between mild asphyxia group and normal control group(P<0.05).There was a significant correlation between aEEG monitoring and NBNA examination(r=0.332,P=0.001).Conclusion:AEEG combined with NBNA is of great value in early brain injury in full-term asphyxiated neonates,that can early indicate brain injury caused by asphyxia of full-term neonates.
