Th9、Th17、CD4^+CD25^+FoxP3^+调节性T细胞在慢性淋巴细胞白血病患者外周血中的变化

The role of Th9、Th17 、CD4^+CD25^+FoxP3^+ regulatory T cells in the peripheral blood of patients with chronic lymphocytic leukemia

摘要 目的探讨慢性淋巴细胞白血病患者外周血中Th9、Th17和CD4^+CD25^+FoxP3^+调节性T细胞及相关细胞因子的表达水平及意义。方法采用流式细胞仪检测36例CLL患者和45例健康对照组外周血Th9、Th17和Treg细胞的表达率,采用酶联免疫吸附方法(ELISA)检测外周血中细胞因子IL-9、IL-17、TGF-β的表达水平。结果 1)CLL组患者外周血Th9、Th17细胞比例及细胞因子IL-9、IL-17浓度均明显高于健康对照组[(1.29±0.35)%vs(0.74±0.23)%,P<0.05;(2.15±0.46)%vs(1.08±0.37)%,P<0.05;(24.25±7.05)%vs(16.35±5.42)%,P<0.05;(11.47±3.99)%vs(7.36±2.85%),P<0.05]。CD4^+CD25^+FoxP3^+细胞比例及细胞因子TGF-β浓度明显低于对照组[(4.76±0.89)%vs(7.26±1.95)%,P<0.05;(3.82±1.39)%vs(6.82±2.01)%,P<0.05]。2)相关性分析:患者Th9、Th17细胞比例及IL-9、IL-17浓度与淋巴细胞计数负相关(γ_s=-0.374,P=0.034;γ_s=-0.382,P=0.032;γ_s=-0.445,P=0.011;γ_s=-0.413,P=0.024);患者Treg细胞比例及TGF-β浓度与淋巴细胞计数正相关(γ_s=0.416,P=0.023;γ_s=0.403,P=0.028)。结论慢性淋巴细胞白血病患者Th9、Th17与CD4^+CD25^+FoxP3^+调节性T细胞比例失衡,血清IL-9、IL-17和TGF-β水平变化,这些原因可能参与慢性淋巴细胞白血病的免疫发病过程。 Objective To investigate the presence and significance of Th9,Th17,T regulation cells and the related cytokines ( IL-9、IL-17 、TGF-β) in the peripheral blood of patients with adult Chronic Lymphocytic Leukemia. Methods The peripheral blood samples were obtained from 36 CLL patients in the Second Hospital of Dalian Medical University ( Dalian, China),including 10 females and 26 males;mean of age,61±9 years. The control samples were collected from 45 healthy volunteers ( 13 females, 32 males;mean age,57±11 years),none of which suffered from any immune diseases. The percentages of Th9,Th17 and Treg cells in the samples were detected by Flow cytometry.The serum levels of IL-9, IL-17 and TGF-β were examined by ELISA . Data analysis was carried out using SPSS 18. 0 statistical software. Descriptive data was summarized and analyzed.Results Compared with the control,the percentages of Th9,Th17 and concentrations of IL-9, IL-17 were all significantly increased in patients with CLL[( 1. 29±0. 35)% vs.( 0. 74±0. 23)%,P<0. 05;( 2. 15±0. 46)% vs.( 1. 08 ±0. 37)%,P<0. 05;( 24. 25± 7. 05)% vs.( 16. 35± 5. 42)%,P< 0. 05;( 11. 47± 3. 99)% vs.( 7. 36 ± 2. 85%),P< 0. 05];the percentages of CD4^+CD25^+FoxP3^+ cells and concentrations of TGF-β were lower in the patient group [( 4. 76± 0. 89)% vs.( 7. 26±1. 95)%,P< 0. 05;( 3. 82± 1. 39)% vs.( 6. 82± 2. 01)%,P< 0. 05]. Moreover,the percentages of Th9,Th17 and concentrations of IL-9, IL-17 were all negatively correlated with lymphocytic accounts (γs =- 0. 374,P = 0. 034;γs =-0. 382,P= 0. 032;γs =-0. 445,P = 0. 011;γs =-0. 413,P = 0. 024) of the patients with CLL;the percentages of CD4^+CD25^+FoxP3^+ cells and concentrations of TGF-β were positively correlated with the lymphocytic accounts (γs = 0. 416,P= 0. 023;γs = 0. 403,P= 0. 028) of the patients with CLL. Conclusion The abnormality Th9,Th17 and Treg cells and their related cytokines ( IL-9,IL-17 and TGF-β) changes in patients with CLL ,which may contribute in the immunological pathogenesis of CLL.
