目的了解五年制在校高职护生伦理决策能力现状,探讨其影响因素,为制订针对性的护理教育措施提供依据。方法采用按年级分层按班级随机整群抽样法,抽取两所高等职业技术学校1~4年级在校护生1563名,采用伦理决策能力问卷(Judgements About Nursing Decisions,JAND)进行调查。结果五年制高职在校护生的伦理决策能力总分(302.04±25.10)分,处于高水平者仅占0.77%,其中伦理理论维度得分(155.76±14.85)分,伦理行动维度得分(146.28±15.92)分,两个维度得分比较,t=21.006,P<0.001,差异有统计学意义。人文课程和知识、专业方向、职业情感及资助、宗教信仰是影响五年制高职在校护生伦理决策能力的主要因素(P<0.001,P<0.05),其共解释伦理决策能力总分的35.8%变异量。结论五年制高职在校护生护理伦理决策能力总体处于中等水平,其中伦理行动较弱;人文课程和知识、专业方向、职业情感及受资助、宗教信仰是影响五年制高职护生伦理决策能力的主要因素。加强对五年制高职在校护生人文类课程及知识的教育,加大护理职业情感的教育力度,以全面提高护生伦理决策能力。
Objective To investigate the status and influencing factors of ethical decision-making competence of five-year-system higher vocational nursing students in order to provide the evidence for carrying out targeted education. Methods 1,563 nursing students who came from grade 1 to grade 4 of two higher health vocational schools were selected by randomly cluster sampling according to different grades and classes. The survey tool was the questionnaire of judgments about nursing decision-making (JAND). Results The total score of JAND of the five-year higher vocational nursing students was(302.04±25.10), the high level was only 0.77%, among them, the score on the dimension of ethical theory was(155.76±14.85), and the score on the dimension of ethical action was (146.28±15.92), and the difference between the two dimensions was significant (t=21.006, P<0.001). Humanities courses, professional emphasis, professional emotion, financial support and religious belief were the main influencing factors of the ethical decision-making ability(P<0.001, P<0.05), and a total of 35.8% variance was explained. Conclusions The ethical decision-making competence of five-year-system higher vocational nursing students is at the middle level, the dimension of ethical action is weaker. Humanities courses,professional emphasis, professional emotion, financial support and religious belief are the main influencing factors. Measures should be taken to stimulate professional feelings, strengthen the education of humanities courses and knowledge, and intensify the education so as to comprehensively improve the ethical their decision-making ability.
