Objective:To explore the construction and practical effect of integrated medical and nurse in chronic disease management model for coronary heart disease.Methods:A total of 200 patients with coronary artery disease diagnosed by coronary angiography from October 2017 to March 2018 were randomly divided into the routine management group and the integrated medical and nurse group with 100 cases each.The routine management group received the routine management model,and the integrated medical and nurse group constructed and implemented the integrated medical and nurse in chronic disease management model of coronary heart disease on the basis of routine nursing.The fatality rate and incidence of rehospitalization,medical expense,heart function grading,cardiac function related parameters,plasma sodium amino terminal B diuresis peptide precursor(NT-pro BNP)level and quality of life [by health survey profiles(SF-36)] score,exercise capacity [by Fugl-Meyer evaluation method(FMA)] score,treatment adherence and patient satisfaction of the two groups were compared.Results:The fatality rate in integrated medical and nurse group case,incidence of rehospitalization,medical expenses were lower than that of conventional management group( P < 0.05 ),the cardiac function indexes is better than the conventional management group( P<0.01,P <0.05),SF-36 and the score of FMA were higher than routine management group( P <0.01),treatment adherence and patient satisfaction were higher than the conventional management group( P<0.01,P <0.05).Conclusion:The integrated medical and nurse management model of chronic disease for patients with coronary heart disease can effectively reduce the mortality rate,rehospitalization rate and medical costs,improve patients′ cardiac function,improve patients′ quality of life,exercise ability,treatment compliance and satisfaction,which is worthy of promotion and application.
