糖皮质激素受体基因多态性与三氯乙烯药疹样皮炎的关系探讨 预览

摘要 目的探讨糖皮质激素受体(GR)基因多态性与三氯乙烯药疹样皮炎的关系。方法对2012年1月-2018年10月深圳市职业病防治院收治的350例有三氯乙烯接触史的受检者的临床资料进行回顾性分析,其中60例发生三氯乙烯药疹样皮炎为发生组,余290例为未发生组。检测受检者外周血细胞GR基因R477H、G679S位点的单核苷酸多态性。对比发生组和未发生组基因型分布;采用Logistic回归分析GR基因R477H、G679S位点等位基因与三氯乙烯药疹样皮炎的关系。结果发生组R477H位点、G679S位点CC基因型构成比与未发生组对比差异均无统计学意义(P>0.05),发生组上述位点CG基因型构成比均低于未发生组(P<0.05),GG基因型构成比均高于未发生组(P<0.05);发生组年龄>50岁、三氯乙烯接触时长>3年、直接接触构成比均高于未发生组(P<0.05),且发生组GR基因R477H、G679S位点等位基因频率均高于未发生组(P<0.05);Logistic回归分析结果表明,年龄>50岁、三氯乙烯接触时长>3年、直接接触、GR基因R477H、G679S位点G等位基因均为三氯乙烯药疹样皮炎的危险因素(OR=5.966、5.129、7.221、4.272、7.121,P<0.05)。结论 GR基因R477H、G679S位点等位基因分布、等位基因频率均与三氯乙烯药疹样皮炎有紧密关系,且年龄>50岁、三氯乙烯接触时长>3年等均是其危险因素。 Objective To explore the relationship between gene polymorphism of glucocorticoid receptor and trichloroethylene drug eruption-like dermatitis. Methods The clinical data of 350 patients with a history of exposure to trichloroethylene were collected and analyzed from January 2012 to October 2018 in Shenzhen Occupational Disease Prevention and Control Hospital. 60 cases of trichloroethylene drug rash were recorded as the occurrence group and the remaining 290 cases as the non-occurrence group. The single nucleotide polymorphisms of GR gene R477 H and G679 S loci in peripheral blood cells were detected. The genotype distribution were compared between the occurrence group and the non-occurrence group. Logistic regression analysis was used to investigate the relationship between GR gene R477 H and G679 S loci genotypes and trichloroethylene drug eruption-like dermatitis. Results There was no significant difference in the proportion of CC alleles at R477 H and G679 S loci between the occurrence group and the non-occurrence group(P>0.05). The proportion of CG genotype in the occurrence group was lower than that in the non-occurrence group(P<0.05), and the proportion of GG genotype in the occurrence group was higher than that in the non-occurrence group(P<0.05). Age>50 years old, exposure time > 3 years and direct contact ratio in the occurrence group were higher than those in the non-occurrence group(P<0.05). The alleles frequencies of GR gene R477 H and G679 S loci in the occurrence group were higher than those in the non-occurrence group(P<0.05), and the results of Logistic regression analysis showed that age >50 years old, exposure time >3 years, direct contact, alleles frequencies of GR gene R477 H and G679 S loci were risk factors for trichloroethylene drug eruption-like dermatitis(OR=5.966, 5.129, 7.221, 4.272, 7.121, P<0.05). Conclusion Allele distribution and frequencies of GR gene R477 H and G679 S loci were closely related to trichloroethylene drug eruption-like dermatitis. Age >50 years old and exposure time
