不同前跷对步态时空参数和下肢关节运动的影响

Effects of different toe spring on gait spatial-temporal parameters and lower limb joint movement

摘要 选取16名成年男性受试者,穿着前跷分别为0°(0#)、5°(1#)、7.7°(2#)、10.3°(3#)和15.5°(4#)的5双测试鞋按日常习惯速度行走,使用红外高速运动捕捉系统(200 Hz)采集下肢运动学数据进行分析.结果表明:穿前跷鞋与地面接触的时间均长于平底鞋(左足P=0.030,右足P=0.002).穿前跷鞋时,左腿膝关节在右足足跟离地(RHO)时刻(P=0.003)、左腿踝关节在左足足跟离地(LHO)时刻(P<0.001)、右足足跟触地(RHS)时刻(P<0.001)、左足足尖离地(LTO)时刻(P=0.023)的屈伸角均大于平底鞋.穿前跷鞋时踝关节屈伸角的最大值(P=0.135)和最小值(P<0.001)均大于平底鞋,但极值的差值(P=0.476)没有差异.因此得出,前跷增大可以延长足与地面的接触时间;前跷增大会增加摆动腿在摆动阶段的膝关节屈角,有利于摆动腿的向前摆动;前跷的改变主要改变了踝关节在矢状面内的运动范围,没有改变踝关节的运动幅度. 16 male subjects were recruited to wear five pairs of testing shoes with toe spring of 0°(0#),5°(1#),7.7°(2#),10.3°(3#) and 15.5°(4#) walking at daily habitual speed. Kinematics data of lower limbs were acquired by eight-camera high-speed video capture system (Motion Analysis Raptor-4, USA, 200 Hz). Results are as follows 丗 The average ground contacting time of foot when wearing toe spring shoes are all longer than wearing 0#( left foot, P = 0. 03. right foot, P = 0. 002). The average left knee flexion angles of subjects when wearing toe spring shoes are all larger than wearing 0# at the time of RH0( right heel off)( P = 0. 003). The average angles of left ankle joint of subjects when wearing toe spring shoes are all larger than wearing 0# at the time of LHO ( left heel off)( P<0. 001), RHS( right heel strike)( P<0. 001) and LTO ( left toe off ground)( P = 0. 023). The average minimum and maximum values of joint angles of ankle of subjects when wearing toe-only rocker sole shoes are all larger than wearing 0#( P = 0. 000, P = 0. 135). The increasing of toe spring angle could extent the ground contacting time of foot and can increase the flexion angle of knee joint of swing leg during swing phase, it is conducive to leg乫s swinging forward. The change of the toe spring angle affects movement range of ankle joint but not affects motion amplitude of ankle joint in sagittal plane.
