阿尔茨海默病相关神经丝蛋白早期诊断阿尔茨海默病的应用价值

Clinical significance of urine Alzheimer-associated neuronal thread protein in the early diagnosis of Alzheimer's disease

摘要 目的探讨阿尔茨海默病相关神经丝蛋白(AD7c-NTP)作为早期诊断阿尔茨海默病(AD)的生物标志物的可行性。方法纳入2016年6月至12月在上海市精神卫生中心住院的46例AD患者为AD组,16例血管性痴呆(VD)患者为VD组,同时入选49例正常老年人群为正常组,39例遗忘型轻度认知功能障碍(aMCI)患者为aMCI组。应用蒙特利尔认知评估(MoCA)量表评估认知功能,并采用酶联免疫吸附测定(ELISA)检测患者尿液中的AD7c-NTP含量。结果 MoCA分数分析显示,与正常组比较,其余各组MoCA分数降低,差异均具有统计学意义(P<0.05);与aMCI组比较,AD组和VD组的MoCA分数降低,差异具有统计学意义(P<0.05),但AD组和VD组间差异无统计学意义(P>0.05);尿液AD7c-NTP浓度结果显示,与正常组比较,其余各组AD7c-NTP浓度均较高,差异具有统计学意义(P<0.05);与aMCI组比较,AD组和VD组的AD7c-NTP浓度升高,差异有统计学意义(P<0.05),但AD组和VD组间差异无统计学意义(P>0.05)。尿液AD7c-NTP浓度为1.151 ng/ml时其对应的敏感性和特异性最显著,各组AD7c-NTP浓度与MoCA分数之间无显著相关性(r=-0.149,P=0.130)。结论尿液AD7c-NTP作为AD早期诊断的潜在标志物具有一定的敏感性。 Objective To evaluate the urine Alzheimer-associated neuronal thread protein(AD7c-NTP) used as a biomarker for the early diagnosis of Alzheimer's disease(AD). Methods A total of 46 cases of AD patients in Shanghai mental health center from June 2016 to December 2016 were selected as AD group, 16 cases of vascular dementia(VD) patients were selected as VD group. Meanwhile, 49 normal elderly population(control group) and 39 patients with amnestic mild cognitive impairment(aMCI)(aMCI group)were enrolled in this study. The cognitive function was assessed by Montreal cognitive assessment(MoCA)scale, and the enzyme linked immunosorbent assay(ELISA) used to detect the concentration of AD7c-NTP in urine. Results The MoCA scores showed that, compared with control group, the MoCA scores in three groups were significantly lower(P<0.05), compared with aMCI group, the MoCA scores in AD group and VD group decreased significantly(P<0.05), but no difference between AD group and VD group(P>0.05). Concentration of AD7c-NTP in urine showed that, compared with control group, the concentration of AD7c-NTP in urine in three groups were significantly higher(P<0.05), compared with aMCI group, concentration of AD7c-NTP in urine in AD group and VD group increased significantly(P<0.05), and no difference between AD group and VD group(P>0.05). When the concentration of AD7c-NTP in urine was 1.151 ng/ml, the sensitivity and specificity were the most significant, There was no significant correlation between concentration of AD7c-NTP in urine and MoCA score(r=-0.149,P=0.130). Conclusion The concentration of AD7c-NTP in urine is sensitive to AD patients, and it could be used for AD early diagnosis.
