多学科会诊模式对1869例单胎胎儿畸形妊娠结局的影响分析

摘要 目的探讨多学科会诊模式对单胎胎儿畸形妊娠结局的影响分析。方法收集2016年3月至2017年2月所有诊断为胎儿畸形并参加多学科会诊的1869例为研究组(MDT组);收集2014年1月至2016年2月门诊传统模式就诊1772例胎儿畸形作为对照组,回顾性分析比较妊娠结局。结果非致死性胎儿畸形MDT组引产率低于对照组,有统计学差异(P<0.05);各个系统可继续妊娠的非致死性畸形比较中,中枢神经系统、心脏、腹腔、泌尿、多发畸形的MDT组引产率低于对照组,有统计学意义(P<0.05);继续妊娠的胎儿中665例出生后一般情况良好,64例已接受手术治疗并成功。结论成立胎儿出生缺陷产前多学科会诊模式不但可以提高预后良好畸形胎儿的存活机会,更有利于促进胎儿医学的发展,值得临床推广。 Objective:Explore the effect of the prenatal multidisciplinary consultation model(MDT)on pregnancy outcome of singleton fetal malformation.Methods:Selected the fetal malformation of multidisciplinary consultation in our hospital from march,2016 to February 2017 as study group(MDT group);Collect 1772 patients(control group)diagnosed with fetal malformation and conducted traditional outpatient consultation From January 2014 to February 2016 as control group.Analyzed two groups retrospectively.Results:For nonfatal abnormalities,MDT group termination rate was lower than the control group,with statistical difference(P<0.05);In the comparison of each system with well prognostic abnormalities,the termination rate of MDT group in central nervous system,heart,abdominal cavity,urinary tract and multiple deformity was lower than the control group,which was statistically significant(P<0.05);665 of the fetuses with continued pregnancy had a good prognosis and 64 had undergone successful surgical treatment.Conclusion:Prenatal multidisciplinary consultation mode of fetal birth defects not only can improve the prognosis of fetus′chance of survival with well prognostic abnormalities,but also promote the development of the fetal medicine.
