目的:探讨翼状胬肉切除联合自体角膜缘结膜移植术后应用角膜绷带镜的临床效果。方法:随机对照临床试验。选取2017-11/2018-10在我院眼科接受手术治疗的原发性翼状胬肉患者92例92眼,按随机数字表法分为角膜绷带镜组和弹性绷带组,每组各4... 目的:探讨翼状胬肉切除联合自体角膜缘结膜移植术后应用角膜绷带镜的临床效果。方法:随机对照临床试验。选取2017-11/2018-10在我院眼科接受手术治疗的原发性翼状胬肉患者92例92眼,按随机数字表法分为角膜绷带镜组和弹性绷带组,每组各46眼。两组患者均接受翼状胬肉切除联合自体角膜缘结膜移植术,术毕角膜绷带镜组患者配戴角膜接触镜,弹性绷带组患者在术毕结膜囊内涂红霉素眼膏,弹性绷带包眼。裂隙灯显微镜观察术后第1、4、7d角膜上皮创面,对角膜上皮愈合情况进行评分;运用评价视觉模拟评分法评价术后7d疼痛情况;运用问卷评分法评价术后7d舒适情况(包括异物感、流泪)。结果:术后第1d和第7d两组患者角膜上皮愈合情况无明显差异,而术后第4d角膜绷带镜组患者角膜上皮愈合优于弹性绷带组(P<0.05)。此外,角膜绷带镜组患者术后7d疼痛感评分低于弹性绷带组,眼部舒适度评分高于弹性绷带组(P<0.05)。结论:角膜绷带镜不仅可以有效促进角膜上皮愈合,还可以有效减轻疼痛,提高眼部舒适感。 展开更多

静息心率对代谢综合征的影响 预览

目的探讨静息心率(RHR)对不同性别健康人群进展为代谢综合征(MS)的影响。方法选取2011年1月-2012年12月聊城市第二人民医院的健康体检者作为研究对象,并进行5年的随访。采用Logistic回归分析不同RHR水平与MS发病率的关系。结果随访5年后... 目的探讨静息心率(RHR)对不同性别健康人群进展为代谢综合征(MS)的影响。方法选取2011年1月-2012年12月聊城市第二人民医院的健康体检者作为研究对象,并进行5年的随访。采用Logistic回归分析不同RHR水平与MS发病率的关系。结果随访5年后,男性组和女性组MS发病率随着RHR的增快而升高(P<0.05)。对于总人群,随着RHR每增加1个单位,MS的发病风险显著增加,经过年龄、BMI、健康相关行为及预先存在的MS组分校正后差异仍有统计学意义[OR=1.320,(95%CI:1.201,1.451),P=0.000];经性别分层后差异仍有统计学意义[男性组:OR=1.192,(95%CI:1.040,1.367),P=0.012;女性组:OR=1.409,(95%CI:1.235,1.608),P=0.000]。对于总人群,以RHR<70/min为参照组,其余3组的MS发病风险均显著增加,且OR呈递增趋势;经过年龄、BMI、健康相关行为及预先存在的MS组分校正后差异仍有统计学意义[RHR≥90/min组:OR=2.327,(95%CI:1.784,3.037),P=0.000]。经性别分层后差异仍有统计学意义[RHR≥90/min组中男性组:OR=1.644,(95%CI:1.148,2.355),P=0.007];女性组:OR=3.375,(95%CI:2.214,5.146),P=0.000]。结论RHR与MS有着密切的关联性,随着RHR的加快,MS风险逐渐升高,这种影响是独立的,且在女性人群中更为显著。 展开更多

AIM To compare the efficacy of a session of extracorporeal shock wave lithotripsy(ESWL) before endoscopic retrograde cholangiopancreatography(ERCP) vs ERCP only for problematic and large common bile duct(CBD) stones.METHODS Adult patients with CBD stones for whom initial ERCP was unsuccessful because of the large size of CBD stones were identified. The patients were randomized into two groups,an 'ESWL + ERCP group' and an 'ERCP-only' group. For ESWL + ERCP cases,ESWL was performed prior to ERCP. Clearance of the CBD,complications related to the ESWL/ERCP procedure,frequency of mechanical lithotripsy use and duration of the ERCP procedure were evaluated in both groups.RESULTS There was no significant difference in baseline characteristics between the two groups. A session of ESWL before ERCP compared with ERCP only resulted in similar outcomes in terms of successful stone removal within the first treatment session(74.2% vs 71.0%,P = 0.135),but a higher clearance rate within the second treatment session(84.4% vs 51.6%,P = 0.018) and total stone clearance(96.0% vs 86.0%,P = 0.029). Moreover,ESWL prior to ERCP not only reduced ERCP procedure time(43 ± 21 min vs 59 ± 28 min,P = 0.034) and the rate of mechanical lithotripsy use(20% vs 30%,P = 0.025),but also raised the clearance rate of extremely large stones(80.0% vs 40.0%,P = 0.016). Post-ERCP complications were similar for the two groups.CONCLUSION Based on the higher rate of successful stone removal and minimal complications,ESWL prior to ERCP appears to be a safe and effective treatment for the endoscopic removal of problematic and large CBD stones. 展开更多

基于表面等离子体共振的金属纳米结构传感器具有测量精度高、易集成的特点,已广泛应用于生物、化学、材料、光子学和生命科学等诸多领域。然而由于表面等离子体共振在可见光波段具有较大的辐射损耗,导致等离子体共振传感器谐振峰较宽,... 基于表面等离子体共振的金属纳米结构传感器具有测量精度高、易集成的特点,已广泛应用于生物、化学、材料、光子学和生命科学等诸多领域。然而由于表面等离子体共振在可见光波段具有较大的辐射损耗,导致等离子体共振传感器谐振峰较宽,极大地限制了传感器的性能。本文采用不对称双椭圆柱结构在等离子体共振传感器中引入不对称性,产生Fano共振以实现窄带光谱特性。通过优化结构参数,在681 nm波长处,得到了半高全宽(FWHM)仅为10.8nm的谐振谷。该传感器在环境折射率1.0~1.1的变化范围内,折射率灵敏度可以达到299 nm/RIU,品质因数可以达到27.8。该传感器在高折射率灵敏度传感应用方面有着巨大的潜力。 展开更多

The locking plate and percutaneous crossing metallic screws and crossing absorbable screws have been used clinically to treat intra-articular calcaneal fractures, but little is known about the biomechanical differences between them. This study compared the biomechanical stability of calcaneal fractures fixed using a locking plate and crossing screws. Three-dimensional finite-element models of intact and fractured calcanei were developed based on the CT images of a cadaveric sample. Surgeries were simulated on models of Sanders type III calcaneal fractures to produce accurate postoperative models fixed by the three implants. A vertical force was applied to the superior surface of the subtalar joint to simulate the stance phase of a walking gait. This model was validated by an in vitro experiment using the same calcaneal sample. The intact calcaneus showed greater stiffness than the fixation models. Of the three fixations, the locking plate produced the greatest stiffness and the highest von Mises stress peak. The micromotion of the fracture fixated with the locking plate was similar to that of the fracture fixated with the metallic screws but smaller than that fixated with the absorbable screws. Fixation with both plate and crossing screws can be used to treat intra-articular calcaneal fractures. In general, fixation with crossing metallic screws is preferable because it provides sufficient stability with less stress shielding. 展开更多

Hepatocellular carcinoma(HCC) is one of the most common malignant tumors in China. The Barcelona Clinic Liver Cancer(BCLC) staging system is regarded as the gold standard staging system for HCC, classifying HCC as early, intermediate, or advanced. For intermediate HCC, trans-catheter arterial chemoembolization(TACE) is recommended as the optimal strategy by the BCLC guideline. This review investigates whether liver resection is better than TACE for intermediate HCC. Based on published studies, we compare the survival benefits and complications of liver resection and TACE for intermediate HCC. We also compare the survival benefits of liver resection in early and intermediate HCC. We find that liver resection can achieve better or at least comparable survival outcomes compared with TACE for intermediate HCC; however, we do not observe a significant difference between liver resection and TACE in terms of safety and morbidity. We conclude that liver resection may improve the short- and long-term survival of carefully selected intermediate HCC patients, and the procedure may be safely performed in the management of intermediate HCC. 展开更多

Recent investigations have revealed that mi RNAs are closely linked to liver regeneration.DATA SOURCES: All included studies were obtained from Pub Med, Embase, the Science Direct databases and Web of Science, with no limitation on publication year. Only studies published in English were considered.RESULTS: We grouped studies that involved mi RNA and liver regeneration into two groups: mi RNAs as promoters and as inhibitors of liver regeneration. We summarized the relevant mi RNAs separately from the related pathways.CONCLUSIONS: Blocking or stimulating the pathways of mi RNAs in liver regeneration may be novel therapeutic strategies in future regeneration-related liver managements. Digital pulse processing has developed rapidly during recent years.Moreover,it has been widely applied in many fields.In this study,we introduce a digital pulse processing method for 2πa and 2πb emitter measurement.Our digital pulse processing method for 2πa and 2πb emitter measurement is comprised of a field-programmable gate-array-based acquisition card and a pulse-height analysis routine.We established two channels(one for the a emitter and one for the b emitter) on an acquisition board using an analog-to-digital converter with a 16-bit resolution at a speed of 100 million samples per second.In this study,we used captured and stored data to analyze emission rate counts and spectrums.The method we established takes into account noise cancelation,dead-time correction,background subtraction,and zero-energy extrapolation.We carefully designed control procedures in order to simplify pulse-width fitting and threshold-level setting.We transmitted data and commands through a universal serial bus between the acquisition board and the computer.The results of our tests prove that our method performs well in pulse reconstruction fidelity and amplitude measurement accuracy.Compared with the current standard method for measuring 2πa and 2πb emission rates,our system demonstrates excellent precision in emission rate counting. 展开更多

The emission manners of organic light-emitting diodes(OLEDs) have experienced almost three-decade evolution.In this review,we briefly summarized the emission manners of OLEDs including:(ⅰ) emission from singlet exciton;(ⅱ) emission from triplet exciton;(ⅲ) emission from singlet exciton converted from triplet exciton.Then we introduced a new type of OLEDs with the emission from doublet exciton,wherein organic neutral radicals are used as emitters.Due to the spin-allowed transition of doublet excitons,using neutral radicals as emitters is believed to be a new way to break the 25%upper limit of internal quantum efficiency of OLEDs.The progress of emissive stable neutral radicals is also shortly reviewed. 展开更多
