Since its introduction to clinical practice nearly 20 years ago, wireless capsule endoscopy has revolutionized the landscape in the diagnosis and management of small bowel diseases. Over the past 10 years, capsule endoscopy has evolved beyond the small intestine and a range of capsules are now available to examine the esophagus, stomach and colon. Because of its ease of use, tolerability, paucity of complications and ability to visualize the entire gastrointestinal tract, capsule endoscopy has entered the mainstream of clinical practice. This review of the literature summarizes the current state of capsule training and highlights the limited data available to assess reader competence and standards expected of an independent practitioner. There are neither standardized teaching strategies nor national or international metrics for accreditation of physicians and nonphysicians interested in mastering this examination. Summating the few publications, there appears to be consensus that diagnostic expertise improves with experience, and that trainees should be fully supervised for at least 20 full case studies. Formative and summative assessment is advisable and the number of taught cases should not be the sole determinant of competence. The review also highlights differences in recommendations from major national gastroenterology societies. Finally, the authors discuss areas of unmet needs in teaching and learning for capsule endoscopy. 展开更多

吞咽辅助凝胶用于脑卒中后患者口服胶囊可行性的吞咽造影研究 预览

目的:通过吞咽造影检查评估吞咽辅助凝胶应用于辅助脑卒中后患者口服胶囊过程中的安全性及有效性,明确其在临床患者服药管理方面是否具有应用价值。方法:本研究共纳入31例脑卒中后患者,其吞咽功能经功能性经口摄食量表(FOIS)评估为4~7级... 目的:通过吞咽造影检查评估吞咽辅助凝胶应用于辅助脑卒中后患者口服胶囊过程中的安全性及有效性,明确其在临床患者服药管理方面是否具有应用价值。方法:本研究共纳入31例脑卒中后患者,其吞咽功能经功能性经口摄食量表(FOIS)评估为4~7级,洼田饮水试验无呛咳。所有受试者均在吞咽造影下口服胶囊,分别用水和吞咽辅助凝胶送服。根据吞咽造影的结果评定患者口服胶囊的成功率及吞咽的量化参数。分析的量化参数包括胶囊在口腔期的运送时间、咽期吞咽的启动时间、胶囊在咽期的运送时间、环咽肌开放时间、胶囊与水通过环咽肌的时间间隔,运动学参数有舌骨的向上位移和向前位移以及环咽肌开放幅度。结果:辅助凝胶送服时胶囊吞服成功率为80.65%,显著高于水送服时的成功率54.84%,统计分析两者差异有统计学意义(χ^2=4.724,P=0.030),提示凝胶送服的成功率显著优于水送服。量化参数分析结果表明,辅助凝胶送服时,胶囊吞服过程所需的口腔运送时间(3 688.2±3 370.6)ms,显著长于水送服时的口腔运送时间(1 886.2±1 467.4)ms(P<0.05),而辅助凝胶送服胶囊舌骨向上位移(11.74±4.94)mm,显著短于用水送服时(15.61±5.59)mm(P<0.05)。结论:辅助凝胶具有包裹性、顺滑性的优势,能够易化脑卒中后患者胶囊吞咽并增加安全性,可进一步在临床推广应用。 展开更多

An inchworm-like capsule robot(ILCR)is a promising device for a minimally invasive diagnosis and treatment of colon diseases.It consists of two expanders and one extensor,the former provides a traction force by expanding the colon and the latter can elongate and retract to enable active locomotion.However,the locomotion efficiency of the ILCR can be seriously lowered by the complex colon environment featuring slippery,viscoelastic,and suspend properties,which has been a main obstacle to its clinical application.This paper aims at improving the locomotion efficiency of the ILCR by optimizing its extensor design.To do this,the locomotion resistance of the ILCR in the colon is analyzed,and complying with a requirement that the traction force must be larger than the locomotion resistance to avoid slipping,a restriction on the extensor design is obtained.Then under the restriction and with reference to the Hyperelastic model which correlates stress and strain of colon tissue,a model for analyzing the influence of the design parameters of the extensor on the locomotion efficiency of the ILCR is built.With this model,the extensor has been optimized and the optimized results have been used to guide the development of a novel extensor,which employs two pairs of lead-screws and nuts and is actuated by one moto匚Ex-vivo experiment has shown that the novel extensor can improve the locomotion efficiency of an ILCR prototype by 57%,without changing its total length. 展开更多

痹祺胶囊联合针刺治疗神经根型颈椎病疗效研究 预览

目的:探讨痹祺胶囊联合针刺治疗神经根型颈椎病的临床分析。方法:选择神经根型颈椎病患者,按照随机数字表法分为两组,每组50例。对照组给予针刺治疗,1次/d,每次留针30min,观察组在此基础上,加用口服痹祺胶囊治疗,4粒/次,3次/d,对比两组... 目的:探讨痹祺胶囊联合针刺治疗神经根型颈椎病的临床分析。方法:选择神经根型颈椎病患者,按照随机数字表法分为两组,每组50例。对照组给予针刺治疗,1次/d,每次留针30min,观察组在此基础上,加用口服痹祺胶囊治疗,4粒/次,3次/d,对比两组疗效、疼痛积分、症状和体征评分以及安全性评价。结果:观察组总有效率94%显著高于对照组总有效率80%(P<0.05)。治疗前观察组症状和体征积分(23.84±5.72)分,对照组(23.28±5.64)分,两组比较无统计学差异(P>0.05)。治疗后观察组症状和体征积分(14.28±2.15)分,对照组(18.93±3.42)分,两组均显著降低(P<0.05),且观察组下降差值(9.48±2.34)分,显著优于对照组(5.23±1.63)分(P<0.05)。两组疼痛积分治疗前无统计学差异(P>0.05);治疗后两组疼痛积分均显著下降(P<0.05),且观察组显著低于对照组(P<0.05)。结论:痹祺胶囊联合针刺对神经根型颈椎病患者临床疗效较好,可有效缓解疼痛,改善临床症状和体征,且安全性较好。 展开更多

Effects of Peiyuan Tongnao Capsule on Working Memory and the Expression of Glutamic Acid and Receptor in Hippocampal Area in Rats with Cerebral Ischemia

Objective: To explore the effects of Peiyuan Tongnao(PYTN) Capsule on working memory, the content of glutamic acid, and the expression of NMDA receptor 2 B(NR2 B) in rats with cerebral ischemia. Materials and Methods: 52 Sprague-Dawley rats were randomly divided into sham group, model group, nimodipine group, and PYTN group. The bilateral common carotid artery occlusion(BCAO) was performed to establish rat model with cerebral ischemia in the model, nimodipine, and PYTN groups. Gastric lavage with some drug based on body weight conversion was performed daily for 4 weeks in the nimodipine and PYTN groups. The working memory of the rats was tested by Morris water maze. The expression of Nissl body in hippocampus tissue was observed by Nissl staining. The determination of Glu content in hippocampus was detected by high-performance liquid chromatography. The expression level of NR2 B in hippocampus area was determined using Western blot. Results: Morris water maze test on working memory escape latency. In the model group, day 1 versus day 4, there was a statistical difference(P < 0.05). In the nimodipine group, day 1 vs. day 4 displayed a statistical difference(P < 0.05).In the PYTN group, day 1 versus day 3 and day 1 versus day 4 were in significant differences(P < 0.05). The Glu content in hippocampus of the sham group was significantly different from that of model group(P < 0.05). The Glu content in the PYTN group was significantly different from that of the model group(P < 0.05). With regard to the expression of NMDA 2 B in hippocampus between the sham group and the model, nimodipine, and PYTN groups, all were displayed statistical significance(P < 0.01). As the same, the expression of NMDA2 B in the model group was significant from that of the nimodipine group(P < 0.05) and PYTN group(P < 0.01). Conclusion: PYTN capsule was beneficial for improving working memory and protect neural cells in rats of cerebral ischemia, which may be associated with upregulation of the expression of Glu and NMDAR2 B in hippocampus. 展开更多

Since AE are considered to be ... Small bowel vascular lesions, including angioectasia (AE), Dieulafoy’s lesion (DL) and arteriovenous malformation (AVM), are the most common causes of obscure gastrointestinal bleeding. Since AE are considered to be venous lesions, they usually manifest as a chronic, well-compensated condition. Subsequent to video capsule endoscopy, deep enteroscopy can be applied to control active bleeding or to improve anemia necessitating blood transfusion. Despite the initial treatment efficacy of argon plasma coagulation (APC), many patients experience re-bleeding, probably because of recurrent or missed AEs. Pharmacological treatments can be considered for patients who have not responded well to other types of treatment or in whom endoscopy is contraindicated. Meanwhile, a conservative approach with iron supplementation remains an option for patients with mild anemia. DL and AVM are considered to be arterial lesions;therefore, these lesions frequently cause acute life-threatening hemorrhage. Mechanical hemostasis using endoclips is recommended to treat DLs, considering the high re-bleeding rate after primary APC cauterization. Meanwhile, most small bowel AVMs are large and susceptible to re-bleeding therefore, they usually require surgical resection. To achieve optimal diagnostic and therapeutic approaches for each type of small bowel lesion, the differences in their epidemiology, pathology and clinical presentation must be understood. 展开更多

This study evaluated the safety of a modified method to implant an intraocular lens (IOL) into the capsular bag immediately after capsulorhexis with a whole or partial nucleus to protect the posterior capsule (PC) during phacoemulsification in a series of 12 Morgagnian cataracts. For 3 cases of hypermature cataracts with smaller and rigid nuclei, after a complete capsulorhexis, an IOL was directly inserted into the capsular bag, which protected the PC during the subsequent phacoemulsification process in the iris plate. For the other 9 cases with larger and softer nuclei, after the nucleus was partially emulsified, the IOL was inserted into the bag. Even with an obvious surge for some cases, the surgeries were uneventful in all 12 cases, with no PC rent or vitreous loss. IOL implantation into the capsular bag with a whole or partial nucleus can provide effective protection for the PC for hypermature cataract during phacoemulsification. 展开更多

为确定2018年7—11月西藏那曲牦牛发病的死亡原因,以安多县、班戈县、聂荣县和色尼区的26头病死牦牛内脏组织为研究对象,利用常规细菌培养法和特异性基因PCR方法,对分离菌进行鉴定,并对其荚膜血清类型、毒力基因以及致病性进行研究。结... 为确定2018年7—11月西藏那曲牦牛发病的死亡原因,以安多县、班戈县、聂荣县和色尼区的26头病死牦牛内脏组织为研究对象,利用常规细菌培养法和特异性基因PCR方法,对分离菌进行鉴定,并对其荚膜血清类型、毒力基因以及致病性进行研究。结果表明:纯化得到13株分离菌,分离菌的菌落特征、菌体形态、生化鉴定结果均与多杀巴氏杆菌相符,并扩增出多杀巴氏杆菌特异性基因Kmt-1和16S rDNA,菌株Pm-1和Pm-2的Kmt-1基因序列与印度、伊朗、巴基斯坦、俄罗斯的牛源多杀巴氏杆菌的同源性均达98%以上;13株菌的荚膜分型鉴定结果均为B型;对分离菌进行已知的23种毒力基因检测结果显示,此次分离到的13株菌毒力基因的数量在16~19种,ptfA、fimA等16种毒力基因检出率为100%,tadD、toxA等4种毒力基因检出率为0,且每个地区分离株毒力基因的分布相同;致病性试验中对照组小鼠全部存活,处理组小鼠死亡时间为24~72 h,死亡率为100%,病变主要表现为脾脏、肺脏、肝脏、心脏出血坏死。导致此次那曲地区牦牛发病死亡的原因为荚膜B型多杀巴氏杆菌病,本研究可为该病的防治提供依据,也为下一步预防疫苗的研究提供了优势菌株。 展开更多

[Objectives] To establish a high performance liquid chromatography (HPLC) wavelength switching method for the simultaneous determination of content of six constituents (phellodendrine chloride, gentiopicrin, paeoniflorin, tetrandrine, berberine hydrochloride and paeonol) in Cangbaiqutong capsules, and provide a scientific basis for the comprehensive evaluation of the quality of Cangbaiqutong capsule.[Methods] The chromatographic column of Waters XSELECT CSH-C 18 (4.6 mm × 150 mm, 5 μm), the mobile phase of acetonitrile-0.1% phosphate solution, gradient elution (0-15 min,10%-18% A;15-30 min,18%-50% A;30-35 min, 50%-10% A);the flow rate of 1.0 mL/min, wavelength switching of 284 (0-7 min, phellodendrine), 274 (7-10 min, gentiopicrin), 230 (10-14 min, paeoniflorin) and 274 nm (14-35 min, tetrandrine, berberine hydrochloride, paeonol), the injection volume of 10 μL.[Results] There was a good linear relationship between the area of chromatographic peak and the injection volume of phellodendrine chloride, gentiopicrin, paeoniflorin, tetrandrine, berberine hydrochloride and paeonol in the range of 0.150-1.504, 0.768-7.680, 1.096-10.960, 0.220-2.200, 0.296-2.956, 0.0345-0.345 μg, respectively;the average recovery rates ( n =6) were 98.3%, 99.2%, 98.8%, 98.8%, 99.1% and 98.2%, respectively;the RSD value was 1.32%, 1.46%, 1.08%, 1.31%, 1.26% and 1.21%, respectively.[Conclusions] The method can be used to determine many kinds of constituents at the same time, and the operation is simple, accurate and reproducible, and can be used for the quality control of compound Cangbaiqutong capsules. 展开更多

BACKGROUND Crohn’s disease(CD)can affect the entire gastrointestinal tract.Proximal small bowel(SB)lesions are associated with a significant risk of stricturing disease and multiple abdominal surgeries.The assessment of SB in patients with CD is therefore necessary because it may have a significant impact on prognosis with potential therapeutic implications.Because of the weak correlation that exists between symptoms and endoscopic disease activity,the“treat-to-target”paradigm has been developed,and the associated treatment goal is to achieve and maintain deep remission,encompassing both clinical and endoscopic remission.Small bowel capsule endoscopy(SBCE)allows to visualize the mucosal surface of the entire SB.At that time,there is no recommendation regarding the use of SBCE during follow-up.AIM To investigate the impact of SBCE in a treat-to-target strategy in patients with CD.METHODS An electronic literature search was conducted in PubMed and Cochrane library using the following search terms:“capsule endoscopy”,in combination with“Crohn’s disease”and“treat-to-target”or synonyms.Two authors independently reviewed titles and abstracts identified by the search strategy after duplicates were removed.Following the initial screening of abstracts,all articles containing information about SBCE in the context of treat-to-target strategy in patients with CD were included.Full-text articles were retrieved,reference lists were screened manually to identify additional studies.RESULTS Forty-seven articles were included in this review.Two indexes are currently used to quantify disease activity using SBCE,and there is good correlation between them.SBCE was shown to be useful for disease reclassification in patients who are suspected of having or who are diagnosed with CD,with a significant incremental diagnostic yield compared to other diagnostic modalities.Nine studies also demonstrated that the mucosal healing can be evaluated by SBCE to monitor the effect of medical treatment in patients with CD.This re 展开更多

目的观察益气和胃胶囊联合莫沙比利治疗慢性萎缩性胃炎的临床疗效。方法选取2014年9月至2017年9月甘肃省武威肿瘤医院消化内科收治的100例慢性萎缩性胃炎患者作为研究对象,将其随机分为观察组和对照组,每组50例。对照组给予莫沙比利治疗... 目的观察益气和胃胶囊联合莫沙比利治疗慢性萎缩性胃炎的临床疗效。方法选取2014年9月至2017年9月甘肃省武威肿瘤医院消化内科收治的100例慢性萎缩性胃炎患者作为研究对象,将其随机分为观察组和对照组,每组50例。对照组给予莫沙比利治疗,观察组在对照组治疗的基础上给予益气和胃胶囊治疗,治疗3个月后检测血清可溶性白细胞介素-2受体(sIL-2R)和胃泌素,对比分析两组临床疗效。结果观察组患者的总有效率为90.0%(45/50),明显高于对照组患者的78.0%(39/50),差异有统计学意义(P<0.05)。与治疗前比较,两组患者治疗后血清sIL-2R水平显著性降低,血清胃泌素水平显著升高。与对照组治疗后比较,观察组治疗后血清sIL2-R水平均显著降低,血清胃泌素水平均显著增加,差异均有统计学意义(P<0.05)。结论益气和胃胶囊联合莫沙比利治疗慢性萎缩性胃炎临床疗效显著。 展开更多
