The huge prognostic difference between early and late stage hepatocellular carcinoma(HCC)is a challenging diagnostic problem.Alpha-fetoprotein is the mostly widely used biomarker for HCC used in the clinic,however it’s sensitivity and specificity of is not optimal.The development and application of multiple biotechnologies,including next generation sequencing,multiple“omics”data,that include genomics,epigenomics,transcriptomics,proteomics,metabolomics,metagenomics has been used for HCC diagnostic biomarker screening.Effective biomarkers/panels/models have been identified and validated at different clinical levels.A large proportion of these have a good diagnostic performance for HCC,especially for early HCC.In this article,we reviewed the various HCC biomarkers derived from“omics”data and discussed the advantages and disadvantages for diagnosis HCC. 展开更多

A circulating fluidized bed evaporator(including down-flow,horizontal,and up-flow beds) was constructed to study the effect of flow directions on multiphase flow boiling heat transfer.A range of experimental investigations were carried out by varying amount of added particles(0-2%),circulation flow rate(2.15-5.16 m^3/h) and heat flux(8-16 kW/m^2).The comparison of heat transfer performance in different vertical heights of the horizontal bed was also discussed.Results reveal that the glass bead particle can enhance heat transfer compared with vapor-liquid two-phase flow for all beds.At a low heat flux(q = 8 kW/m),the heat-transfer-enhancing factor of the horizontal bed is obviously greater than those of the up-flow and down-flow beds.With the increase in the amount of added particles,the heat-transfer-enhancing factors of the up-flow and down-flow beds increase,whereas that of the horizontal bed initially increases and then decreases.However,at a high heat flux(q=16 kW/m),the heat-transfer-enhancing factors of the three beds show an increasing tendency with the increase in the amount of added particles and become closer than those at a low heat flux.For all beds,the heat-transfer-enhancing factor generally increases with the circulation flow rate but decreases with the increase in heat flux. 展开更多

目的探讨术前放疗对原发性肝癌(肝癌)伴门静脉癌栓的疗效及其应用价值。方法回顾性分析2014 年1 月至2017 年12 月东方肝胆外科医院收治的248 例肝癌合并门静脉癌栓患者临床资料。其中男189 例,女59 例;年龄32~82 岁,中位年龄53 岁。患... 目的探讨术前放疗对原发性肝癌(肝癌)伴门静脉癌栓的疗效及其应用价值。方法回顾性分析2014 年1 月至2017 年12 月东方肝胆外科医院收治的248 例肝癌合并门静脉癌栓患者临床资料。其中男189 例,女59 例;年龄32~82 岁,中位年龄53 岁。患者均签署知情同意书,符合医学伦理学规定。根据治疗方法不同患者分为术前放疗+手术组(术前放疗组,102 例)和单纯放疗组(146 例)。观察放疗效果和不良反应。生存分析采用非参数法Wilcoxon 检验。结果多数患者放疗期间有不同程度的消化道反应,表现为恶心、呕吐、纳差、腹胀等,未见上消化道出血及严重呕吐,均经对症治疗后缓解。患者均未发生严重的肝损害。通过术前18 Gy 剂量放疗,最终41%(102/248)患者获得手术机会。放疗后肿瘤大面积坏死,门静脉癌栓局限、硬化、纤维化。术前放疗组1 年生存率为49%,明显高于单纯放疗组的32%(Z=44.804,P<0.05)。结论术前放疗可使部分肝癌合并门静脉癌栓患者降期,获得手术切除机会,明显改善患者预后,延长总体生存时间。 展开更多

In localized prostate cancer,a clear correlation between CTC counts and clinicopathological risk parameters and outcome has not been observed.Currently,the focus of research is shifting from CTC enumeration towards molecular characterization of CTC leading to the discovery of markers predicting treatment response.The role of androgen receptor splice variants expressed by CTC as markers of resistance to abiraterone and enzalutamide has been assessed by various studies.The identification of CTC markers predicting treatment response represents a key step to guide the selection of treatment (e.g.,abiraterone/enzalutamide vs taxanes), particularly in patients with mCRPC.As an alternative to CTC,the analysis of circulating tumor DNA has been shown to enable a noninvasive disease characterization having high potential to promote precision oncology. 展开更多

As one of the most important auxiliary systems of a circulating fluidized bed boiler,the limestone pneumatic conveying system is of great significance to its operation.Maldistribution of powder in the different injecting ports seriously limits inner-furnace desulfurization performance owing to inefficient mixing of limestone powder and SO2.The gas-solid flow characteristics of an industrial-scale multiple-branching limestone-conveying pipeline system of a 200 MW circulating fluidized bed boiler were studied using a computational particle fluid dynamics method.The maldistribution intensity was studied under different operating conditions of air velocity and particle mass flow rate.Simulation results indicated that when the air velocity increased,the maldistribution was mitigated,but when the particle mass flow rate increased,the maldistribution strengthened.To solve this problem,two improved schemes were proposed:adding a deflector at different angles and changing the height of pipeline distributor.According to the simulation,the maldistribution could be efficiently mitigated using a distributor height in the range of 100-120 mm and a deflector angle of 10°-30°. 展开更多

肝细胞癌合并胆管癌栓的CT表现及临床应用价值 预览

目的分析肝细胞癌(HCC)合并胆管癌栓的CT表现,以期提高对HCC合并胆管癌栓的影像诊断水平。方法收集2012年1月至2018年9月中山大学附属第一医院和江门市新会中医院26例HCC合并胆管癌栓患者资料,病例均经手术后病理证实,并行64排螺旋CT平... 目的分析肝细胞癌(HCC)合并胆管癌栓的CT表现,以期提高对HCC合并胆管癌栓的影像诊断水平。方法收集2012年1月至2018年9月中山大学附属第一医院和江门市新会中医院26例HCC合并胆管癌栓患者资料,病例均经手术后病理证实,并行64排螺旋CT平扫及双期(动脉期和门静脉期)增强扫描。回顾性分析其CT表现,包括受累胆管的位置、胆管内癌栓的密度、强化方式、肝内肿块与胆管癌栓的关系、胆管壁的情况。以手术及病理结果为"金标准",分析CT对HCC合并胆管癌栓定位及定性的准确性。结果26例均明确诊断为肝内肿块及胆管内肿块,HCC和胆管癌栓在CT上均清楚显示。原发HCC的CT增强扫描表现为三种强化方式:"快进快退"18例(69.2%),"快进慢退"4例(15.4%),"三低"4例(15.4%);胆管癌栓CT表现为扩张的胆管内填充有软组织块影,胆管远端扩张,胆管壁无增厚。24例(92.3%)HCC与癌栓相连,2例(7.7%)HCC与胆管癌栓不相连。增强扫描25例(96.2%)癌栓强化方式与肝内肿瘤的强化方式一致,1例(3.8%)强化方式不一致。CT对HCC合并胆管癌栓的检出例数、成像位置与手术及病理结果基本一致,准确性为100.0%(26/26)。结论HCC合并胆管癌栓的CT表现具有一定的特征性,仔细分析肝内肿块及胆管内癌栓的CT表现,有助于疾病诊断。 展开更多

Long non-coding RNAs(lncRNAs)are members of the non-protein coding RNA family longer than 200 nucleotides.They participate in the regulation of gene and protein expression influencing apoptosis,cell proliferation and immune responses,thereby playing a critical role in the development and progression of various cancers,including colorectal cancer(CRC).As CRC is one of the most frequently diagnosed malignancies worldwide with high mortality,its screening and early detection are crucial,so the identification of disease-specific biomarkers is necessary.LncRNAs are promising candidates as they are involved in carcinogenesis,and certain lncRNAs(e.g.,CCAT1,CRNDE,CRCAL1-4)show altered expression in adenomas,making them potential early diagnostic markers.In addition to being useful as tissue-specific markers,analysis of circulating lncRNAs(e.g.,CCAT1,CCAT2,BLACAT1,CRNDE,NEAT1,UCA1)in peripheral blood offers the possibility to establish minimally invasive,liquid biopsy-based diagnostic tests.This review article aims to describe the origin,structure,and functions of lncRNAs and to discuss their contribution to CRC development.Moreover,our purpose is to summarise lncRNAs showing altered expression levels during tumor formation in both colon tissue and plasma/serum samples and to demonstrate their clinical implications as diagnostic or prognostic biomarkers for CRC. 展开更多

The present work investigated the synergetic effect of pyrolysis-derived char,tar and gas(py-gas)on NO reduction,which may occur in circulating fluidized-bed decoupling combustion(CFBDC)system treating N-rich fuel.Experiments were carried out in a lab-scale drop-tube reactor for NO reduction by some binary mixtures of reagents including char/py-gas,tar/py-gas and tar/char.At a specified total mass rate of0.15 g·min^-1 for NO-reduction reagent,the char/py-gas(binary reagent)enabled the best synergetic NO reduction in comparison with the others.There existed effective interactions between char and some species in py-gas(i.e.,H2,CxHy)during NO reduction by pyrolysis products,meanwhile the tar/py-gas or tar/char mixture only caused a positive effect when tar proportion was necessarily lowered to about 26%.On the other hand,the synergetic effects were not improved for all tested binary reagents by increasing the reaction temperature and residence time. 展开更多

目的探讨外周血液中循环肿瘤细胞(CTCs)和循环游离DNA(cfDNA)检测在乳腺癌患者中的临床价值.方法收集2017年7月至2018年4月就诊于南通大学附属医院的47例乳腺癌患者(Ⅱ期7例、Ⅲ期19例、Ⅳ期21例),24例乳腺良性疾病患者,以及同期28名健... 目的探讨外周血液中循环肿瘤细胞(CTCs)和循环游离DNA(cfDNA)检测在乳腺癌患者中的临床价值.方法收集2017年7月至2018年4月就诊于南通大学附属医院的47例乳腺癌患者(Ⅱ期7例、Ⅲ期19例、Ⅳ期21例),24例乳腺良性疾病患者,以及同期28名健康体检者外周血液标本.采用基于尺寸的高通量微流控芯片捕获CTCs、基于Alu序列的实时荧光定量PCR检测游离DNA长、短片段(247bp、115bp),并以长、短片段扩增产物比值作为DNA完整性指标.采用Mann-WhitneyU检验或Kruskal-WallisH检验比较各组差异,分析CTCs和cfDNA与乳腺癌临床参数的关系.绘制ROC曲线并计算曲线下面积(AUC)用于评价血细胞CTCs及血浆cfDNA检测作为诊断标准的可行性.结果47例乳腺癌患者CTCs(13.98±12.36)个/ml、cfDNA的完整性(Alu247/115)(0.7687±0.3868)与正常对照组的(1.14±1.35)个/ml、0.5094±0.2456相比,差异有统计学意义(U值分别为126.5、359.0,P均<0.001);ROC曲线下面积为:CTCs:0.885(95%CI:0.805~0.965),cut-off值为7.68个/ml,灵敏度为80.4%,特异度为96.4%;Alu247/115:0.727(95%CI:0.608~0.847),cut-off值为0.431,灵敏度为71.7%,特异度为71.4%.CTCs和Alu247/115联合检测时曲线下面积为0.919(95%CI0.854~0.984),高于各指标单项检测.结论CTCs与cfDNA可能是乳腺癌辅助诊断的潜在生物学指标,CTCs与cfDNA联合检测可能提高乳腺癌患者的诊断率. 展开更多

A combustion model of a large-scale supercritical circulati ng fluidized bed (CFB) boiler was developed for comprehensive computational-fluid-dynamics analysis. The model incorporates gas-solid hydrodynamics, coal combustion, heat transfer on heat exchange surfaces in the furnace, and heat transfer between fumace and working medium in the heat transfer tubes. In simulating the dense and dilute phases in the fumace, the gas-solid hydrodynamics is based on the Euler-Euler model and energy-minimization multiscale drag model. Coal combustion entails evaporation, devolatilization, char combustion, gas homoge neous reaction, and pollutant emission. The coefficient ofheat transfer between gas-solid and the waterwall is estimated using the cluster renewal model, and for radiation, the discrete ordinate model is used. Moreover, thermohydraulic processes in the membrane wall are also in eluded in the heat transfer process. The model was successfully applied in simulations of a 350-MW supercritical CFB boiler. Detailed distributions of solids concentration, oxygen, heat flux, and working medium temperature in the boiler furnace are presented. 展开更多

Gastric cancer (GC) remains an important cause of cancer death worldwide with a high mortality rate due to the fact that the majority of GC cases are diagnosed at an advanced stage when the prognosis is poor and the treatment options are limited. Unfortunately, the existing circulating biomarkers for GC diagnosis and prognosis display low sensitivity and specificity and the GC diagnosis is based only on the invasive procedures such as upper digestive endoscopy. There is a huge need for less invasive or non-invasive tests but also highly specific biomarkers in case of GC. Body fluids such as peripheral blood, urine or saliva, stomach wash/gastric juice could be a source of specific biomarkers, providing important data for screening and diagnosis in GC. This review summarized the recently discovered circulating molecules such as microRNAs, long non-coding RNAs, circular RNAs, which hold the promise to develop new strategies for early diagnosis of GC. 展开更多

目的:探讨肝癌合并门静脉癌栓(PVTT)患者术后辅助肝动脉化疗栓塞术(TACE)联合门静脉化疗(PVC)的有效性及安全性。方法:回顾性分析2010年1月-2016年1月收治的经手术治疗的119例肝癌合并PVTT患者临床资料,其中64例术中放置门静脉化疗泵,... 目的:探讨肝癌合并门静脉癌栓(PVTT)患者术后辅助肝动脉化疗栓塞术(TACE)联合门静脉化疗(PVC)的有效性及安全性。方法:回顾性分析2010年1月-2016年1月收治的经手术治疗的119例肝癌合并PVTT患者临床资料,其中64例术中放置门静脉化疗泵,术后行TACE治疗及通过门静脉化疗泵行PVC(TACE+PVC组),5 5例术中未放置门静脉化疗泵,术后行单纯TACE(TACE组)。观察两组术后生存及并发症情况。结果:TACE+PVC组与TACE组中位无瘤生存期及中位生存期分别为13.3个月vs.6.8个月、19.5个月vs.12.5个月;术后0.5、1、2、3年的无瘤生存率及71.9%vs.52.9%、57.5%vs.26.7%、16.4%vs.8.2%、6.9%vs.2.1%,总生存率分别为90.5%vs.89.1%、69.5%vs.50.4%、37.9%vs.12.1%、22.4%vs.8.1%,TACE+PVC组的无瘤生存率与总生存率均明显优于TACE组(P=0.004、P=0.001)。统计分析显示,术后治疗方式是患者术后生存时间的独立影响因素之一(P<0.05)。两组术后各并发症发生率差异无统计科学意义(均P>0.05)。结论:术后辅助TACE联合PVC治疗合并PVTT的肝癌患者安全有效,且效果优于单纯TACE治疗。 展开更多

A cold-model vertical multi-tube circulating fluidized bed evaporator was designed and built to conduct a visualization study on the pressure drop of a liquid–solid two-phase flow and the corresponding particle distribution.Water and polyformaldehyde particle(POM)were used as the liquid and solid phases,respectively.The effects of operating parameters such as the amount of added particles,circulating flow rate,and particle size were systematically investigated.The results showed that the addition of the particles increased the pressure drop in the vertical tube bundle.The maximum pressure drop ratios were 18.65%,21.15%,18.00%,and 21.15%within the experimental range of the amount of added particles for POM1,POM2,POM3,and POM4,respectively.The pressure drop ratio basically decreased with the increase in the circulating flow rate but fluctuated with the increase in the amount of added particles and particle size.The difference in pressure drop ratio decreased with the increase in the circulating flow rate.As the amount of added particles increased,the difference in pressure drop ratio fluctuated at low circulating flow rate but basically decreased at high circulating flow rate.The pressure drop in the vertical tube bundle accounted for about 70%of the overall pressure drop in the up-flow heating chamber and was the main component of the overall pressure within the experimental range.Three-dimensional phase diagrams were established to display the variation ranges of the pressure drop and pressure drop ratio in the vertical tube bundle corresponding to the operating parameters.The research results can provide some reference for the application of the fluidized bed heat transfer technology in the industry. 展开更多

目的:通过CellSearch技术检测非小细胞肺癌(non-small cell lung cancer,NSCLC)患者外周血中循环肿瘤细胞(circulating tumor cells,CTCs)的数目,并探讨其与NSCLC患者预后的相关性。方法:回顾分析2013年6月—2017年6月在吉林省肿瘤医院... 目的:通过CellSearch技术检测非小细胞肺癌(non-small cell lung cancer,NSCLC)患者外周血中循环肿瘤细胞(circulating tumor cells,CTCs)的数目,并探讨其与NSCLC患者预后的相关性。方法:回顾分析2013年6月—2017年6月在吉林省肿瘤医院接受治疗的68例NSCLC患者的资料。所有患者采集外周静脉血10 mL,经CellTracks Autoprep System进行磁珠法富集CTCs,采用CellTracks AnalyzerⅡ系统扫描分析CTCs的个数,将EpCAM+CK+DAPI+CD45-且细胞膜及细胞核染色完整的细胞定义为CTCs,CTCs数目≥1定义为CTCs阳性。采用χ^2检验分析CTC数目与NSCLC患者病理参数的相关性;采用Kaplan-Meier法进行生存分析,COX风险回归模型计算危险比(hazard ratio,HR)和95%可信区间(condence interval,CI)分析CTCs对NSCLC患者总生存期的影响。结果:68例NSCLC患者中18例有CTCs,CTCs阳性率为26.5%(18/68)。Ⅰ+Ⅱ期、Ⅲ期及Ⅳ期患者的CTCs阳性率分别为23.1%(3/13)、25.0%(4/16)和28.2%(11/39);肺腺癌与肺鳞癌患者CTCs阳性率分别为30.4%(14/46)和18.2%(4/22)。多因素回归分析结果显示,病理类型、病理分期和治疗方式均不是CTCs影响预后生存的因素(P值均>0.05)。CTCs数目≥1患者总生存时间明显短于CTCs数目<1患者(HR=0.381,95%CI:0.151~0.963,P=0.041),但CTCs的存在与病理类型、年龄及吸烟无关(P=0.383,P=1.000和P=1.000)。结论:在Ⅰ~Ⅳ期NSCLC患者外周血中均可检测到CTCs,以Ⅳ期患者中检出率最高;CTCs数目≥1患者的生存时间明显短于CTCs数目<1者。CTCs可能是一种有一定临床应用价值的检测指标。 展开更多
