BACKGROUND:Point-of-care ultrasound(POCUS)has become increasingly integrated into the practice of emergency medicine.A common application is the extended focused assessment with sonography in trauma(eFAST)exam.The American College of Emergency Physicians has guidelines regarding the scope of ultrasound in the emergency department and the appropriate documentation.The objective of this study was to conduct a review of performed,documented and billed eFAST ultrasounds on trauma activation patients.METHODS:This was a retrospective review of all trauma activation patients during a 10-month period at an academic level-one trauma center.A list comparing all trauma activations was crossreferenced with a list of all billed eFAST scans.Medical records were reviewed to determine whether an eFAST was indicated,performed,and appropriately documented.RESULTS:We found that 1,507 of 1,597 trauma patients had indications for eFAST,but 396(27%)of these patients did not have a billed eFAST.Of these 396 patients,87(22%)had documentation in the provider note that an eFAST was performed but there was no separate procedure note.The remaining 309(78%)did not have any documentation of the eFAST in the patient’s chart although an eFAST was recorded and reviewed during ultrasound quality assurance.CONCLUSION:A significant proportion of trauma patients had eFAST exams performed but were not documented or billed.Lack of documentation was multifactorial.Emergency ultrasound programs require appropriate reimbursement to support training,credentialing,equipment,quality assurance,and device maintenance.Our study demonstrates a significant absence of adequate documentation leading to potential revenue loss for an emergency ultrasound program. 展开更多

We propose a simple and efficient method that uses a single focused hybrid vector beam to confine metallic Rayleigh particles at multiple positions.We study the force mechanisms of multiple trapping by analyzing the gradient and scattering forces.It is observed that the wavelength and topological charges of the hybrid vector beam regulate the trapping positions and number of optical trap sites.The proposed method can be implemented easily in three-dimensional space, and it facilitates both trapping and organization of particles.Thus, it can provide an effective and controllable means for nanoparticle manipulation. 展开更多

BACKGROUND To date,there are no guidelines on the treatment of solid neoplasms in the transplanted kidney.Historically,allograft nephrectomy has been considered the only reasonable option.More recently,nephron-sparing surgery (NSS) and ablative therapy (AT) have been proposed as alternative procedures in selected cases.AIM To review outcomes of AT for the treatment of renal allograft tumours.METHODS We conducted a systematic review according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses 2009 Checklist.PubMed was searched in March 2019 without time restrictions for all papers reporting on radiofrequency ablation (RFA),cryoablation (CA),microwave ablation (MWA),high-intensity focused ultrasound (HIFU),and irreversible electroporation (IRE) of solid tumours of the kidney allograft.Only original manuscripts describing actual cases and edited in English were considered.All relevant articles were accessed in full text.Additional searches included all pertinent references.Selected studies were also assessed for methodological quality using a tool based on a modification of the Newcastle Ottawa scale.Data on recipient characteristics,transplant characteristics,disease characteristics,treatment protocols,and treatment outcomes were extracted and analysed.Given the nature and the quality of the studies available (mostly retrospective case reports and small retrospective uncontrolled case series),a descriptive summary was provided.RESULTS Twenty-eight relevant studies were selected describing a total of 100 AT procedures in 92 patients.Recipient age at diagnosis ranged from 21 to 71 years whereas time from transplant to diagnosis ranged from 0.1 to 312 mo.Most of the neoplasms were asymptomatic and diagnosed incidentally during imaging carried out for screening purposes or for other clinical reasons.Preferred diagnostic modality was Doppler-ultrasound scan followed by computed tomography scan,and magnetic resonance imaging.Main tumour types were: papillary renal cell carcinoma (RCC) and clear cel 展开更多

A novel technique for the measurement of the coating layer thickness of fine particles was developed in this work based on cross-sectioning of micrometre-sized single coated particles using focused ion beam (FIB) milling. This technique was tested on two batches of aerogel particles coated with thin coatings in a spouted bed. The FIB milling procedure consisted of two steps. First, the desired part of the coated particle was removed using a high ion beam current. The resulting cross-sectioned area was then polished using a lower ion beam current to make the cross-section clearly visible. The FIB milling process was controlled with simultaneous scanning electron microscopy (SEM). Afterwards, the coating layer thickness was evaluated using the SEM images. The coating layer was successfully applied on the porous aerogel microparticles in the spouted bed. The coating uniformity of the highly porous particles increased with increasing sprayed coating solution amount, with up to 91% of the particle pores being covered. The FIB-cross-sectioning technique using an ion beam of 2.50 nA for the first milling and 0.43 nA for polishing of the surface resulted in successful generation of cross-sections of representative particles with a visible particle core and coating layer. A coating layer thickness of approximately 700 nm was achieved on particles with sizes of below 45 μm. 展开更多

Systemic toxicity and insufficient drug accumulation at the tumour s让e are main barriers in chemotherapy. Thermosens让ive liposomes (TSL) combined with high intensity focused ultrasound (HIFU) has emerged as a potential solution to overcome these barriers through targeted drug delivery and localised release. Owing to the multiple physical and biochemical processes involved in this combination therapy, mathematical modelling becomes an indispensable tool for detailed analysis of the transport processes and prediction of tumour drug uptake. To this end, a multiphysics model has been developed to simulate the transport of chemotherapy drugs delivered through a combined HIFU-TSL system. All key delivery processes are considered in the model;these in elude interstitial fluid flow, HIFU acoustics, bioheat transfer;drug release and transport, as well as tumour drug uptake. The capability of the model is demonstrated through its application to a 2-D prostate tumour model reconstructed from magnetic res on ance images. Our results not only dem on strate the feasibility of the model to simulate this combination therapy, but also confirm the advantage of HIFU-TSL drug delivery system with enhancement of drug accumulati on in tumour regi ons and reduction ofdrug availability in n ormal tissue. This multiphysics modelling framework can serve as a useful tool to assist in the design of HIFU-TSL drug delivery systems and treatment regimen for improved anticancer efficacy. 展开更多

The selective recognition of nanoparticles (NPs) can be achieved by nanoparticle-imprinted matrices (NAIMs),where NPs are imprinted in a matrix followed by their removal to form voids that can reuptake the original NPs.The recognition depends on supramolecular interactions between the matrix and the shell of the NPs,as well as on the geometrical suitability of the imprinted voids to accommodate the NPs.Here,gold NPs stabilized with citrate (AuNPs-cit) were preadsorbed onto a conductive surface followed by electrografting of p-aryldiazonium salts (ADS) with different functional groups.The thickness of the matrix was carefully controlled by altering the scan number.The AuNPs-cit were removed by electrochemical dissolution.The recognition of the NAIMs was determined by the reuptake of the original AuNPs-cit by the imprinted voids.We found that the recognition efficiency is a function of the thickness of the NAIM layer and is sensitive to the chemical structure of the matrix.Specifically,a subtle change of the functional group of the p-aryldiazonium building block,which was varied from an ether to an ester,significantly affected the recognition of the NPs. 展开更多

Objective: To study the effect of dexmedetomidine in high intensity focused ultrasound treatment of uterine fibroids on pain degree, pain mediator and stress mediator secretion as well as stress signal molecule expression. Methods: 78 patients with uterine fibroids who received high intensity focused ultrasound treatment in Shaanxi Second Provincial People's Hospital between February 2015 and February 2018 were selected as the research subjects and randomly divided into the Dex group who accepted dexmedetomidine + fentanyl + midazolam anesthesia and the control group who accepted fentanyl + midazolam anesthesia. The pain degree was assessed 1 day and 3 d after surgery;the secretion of pain mediators and stress mediators in serum as well as the expression levels of stress signaling molecules in peripheral blood were measured before surgery and 3 d after surgery. Results: 1 d and 3 d after surgery, the NRS pain scores of Dex group were significantly lower than those of control group;3 d after surgery, serum SP, BK, 5-HT, IL-6, IL-8, NE, COR, ACTH and MDA contents as well as peripheral blood NF-κB, p38MAPK, NOX4 and Nrf2 expression of both groups of patients were higher than those before surgery whereas serum SOD and HO-1 contents as well as peripheral blood Keap1 expression were lower than those before surgery, and serum SP, BK, 5-HT, IL-6, IL-8, NE, COR, ACTH and MDA contents as well as peripheral blood NF-κB, p38MAPK, NOX4 and Nrf2 expression of Dex group 3 d after surgery were lower than those of control group whereas serum SOD and HO-1 contents as well as peripheral blood Keap1 expression were higher than those of control group. In this study, we numerically investigated the nonlinear focused wave group action on a truncated surface-piercing structure, and developed a two-dimensional fully nonlinear numerical tank using the higher-order boundary element method. We determined the amplitude of the wave components of the focused wave group based on the JONSWAP wave spectrum. We discuss the effects of the presence of a surface-piercing structure on the characteristics of the focused wave group and find that the location of the structure does not evidently change the focal location or focal time of the focused wave group. The largest amplitudes of the run-up and horizontal force on the structure occur when the front surface of the structure is at the focal location. The critical draught and breadth of the structure occur when the wave run-up reaches its maximum along the structure. 展开更多

局部晚期胰腺癌聚焦大分割适形放疗的临床分析 预览

目的探讨大分割放疗在局部晚期胰腺癌的可行性,疗效和毒副作用。方法收集我院2010年11月~2015年12月39例Ⅲ、Ⅳ期胰腺癌采用聚焦三维适形放疗,单次照射剂量3 Gy,5次/周,总剂量51 Gy/17 F,靶区中心BED=72 Gy。同期给予吉西他滨500~750 mg... 目的探讨大分割放疗在局部晚期胰腺癌的可行性,疗效和毒副作用。方法收集我院2010年11月~2015年12月39例Ⅲ、Ⅳ期胰腺癌采用聚焦三维适形放疗,单次照射剂量3 Gy,5次/周,总剂量51 Gy/17 F,靶区中心BED=72 Gy。同期给予吉西他滨500~750 mg/m2,第1天静脉滴注,1次/周,直至放疗结束后继续两周期。结果 39例均全部完成放化疗计划,治疗后缓解腹背部疼痛的总有效率84.26%(27/32),完全缓解率47.15%(15/32),部分缓解率39.31%(12/32),肿瘤退缩率74.72%(29/39)。中位生存期12.8个月,1年生存率62.45%(24/39),2年生存率23.30%(9/39)。主要不良反应为白细胞,血小板减少,恶心、呕吐和肝功能损伤,未发生4级不良反应。结论聚焦大分割适形放疗同步化疗治疗局部晚期胰腺癌有明显的肿瘤杀伤作用,提高患者的生活质量,不良反应可耐受,疗效肯定。 展开更多

In situ protein micro-crystal fabrication by cryo-FIB for electron diffraction

Micro-electron diffraction (MicroED)is an emerging technique to use cryo-electron microscope to study the crystal structures of macromolecule from its micro-/nano-crystals,which are not suitable for conventional X-ray crystallography.However,this technique has been prevented for its wide application by the limited availability of producing good micro-/nano-crystals and the inappropriate transfer of crystals.Here,we developed a complete workflow to prepare suitable crystals efficiently for MicroED experiment.This workflow includes in situ on-grid crystallization,single-side blotting,cryo-focus ion beam (cryo-FIB)fabrication,and cryo-electron diffraction of crystal cryo-lamella.This workflow enables us to apply MicroED to study many small macromolecular crystals with the size of 2-10μm,which is too large for MicroED but quite small for conventional X-ray crystallography.We have applied this method to solve 2.5A crystal structure of lysozyme from its micro-crystal within the size of 10×10×10μm^3.Our work will greatly expand the availability space of crystals suitable for MicroED and fill up the gap between MicroED and X-ray crystallography. 展开更多

Objective:To study the effects of high-intensity focused ultrasound (HIFU) combined with interventional chemoembolization on the advanced cervical cancer lesion growth and cell invasion.Methods:Patients with stage IIB-IVA cervical cancer treated in Suining Hospital of TCM between May 2014 and October 2016 were selected and randomly divided into two groups, HIFU group received HIFU combined with interventional chemoembolization, and the control group accepted interventional chemoembolization. The levels of tumor markers in serum as well as the expression of tumor suppressor genes and invasion genes in tumor lesions were determined before and after treatment.Results: 2 weeks, 3 weeks and 4 weeks after treatment, serum TK-1, SCC and CA125 levels of both groups were lower than those before treatment, serum TK-1, SCC and CA125 levels of HIFU group 2 weeks after treatment were not different from those of control group, and serum TK-1, SCC and CA125 levels of HIFU group 3 weeks and 4 weeks after treatment were significantly lower than those of control group;4 weeks after treatment, AIF, NDRG4, SARI and eIF4E3 mRNA expression in tumor lesions of both groups were higher than those before treatment while FAK, KGFR and MMP9 mRNA expression were lower than those before treatment, and AIF, NDRG4, SARI and eIF4E3 mRNA expression in tumor lesions of HIFU group were higher than those of control group while FAK, KGFR and MMP9 mRNA expression were lower than those of control group.Conclusion: HIFU combined with interventional chemoembolization can be more effective in suppressing the advanced cervical cancer lesion growth and cell invasion than interventional chemoembolization alone. 展开更多

Objective:To study the effect of high-intensity focused ultrasound (HIFU) combined with radiotherapy on tumor malignancy in patients with advanced pancreatic cancer and the corresponding side effects.Methods:A total of 84 patients with advanced pancreatic cancer treated in our hospital between May 2013 and March 2016 were selected and randomly divided into HIFU group and IGRT group, HIFU group accepted high-intensity focused ultrasound combined with radiotherapy and IGRT group received radiotherapy alone. 4 weeks after treatment, the levels of tumor markers, liver and kidney function indexes, perineural invasion-related molecules and cytokines in serum as well as the levels of immune cells in peripheral blood were determined.Results:4 weeks after treatment, serum CA199, CA242, OPN, NGAL, RBP4, NGF, TrkA, p75, BDNF and TrkB levels of HIFU group were significantly lower than those of IGRT group, serum IL-2, TNF-α, IFN-γ and IL-13 levels as well as peripheral blood NKT cell and CD4+T cell levels were significantly higher than those of IGRT group, and serum ALT, AST, Cr and BUN levels were not significantly different from those of IGRT group.Conclusion:HIFU combined with radiotherapy treatment of advanced pancreatic cancer can more effectively kill cancer cells, inhibit pancreatic cancer cell invasion to the peripheral nerve and enhance the antitumor immune response mediated by NKT cells and CD4+T cells. 展开更多

目的 通过检测经聚焦超声(FUS)消融的SD大鼠胰腺组织凝固性坏死区中Tie-2受体mRNA的表达和组织学结构变化,探讨FUS消融对凝固性坏死组织的血管生成的影响。方法 随机将4~6周龄的SPF级SD大鼠60只分为FUS组(50只),对照组(10只)。FU... 目的 通过检测经聚焦超声(FUS)消融的SD大鼠胰腺组织凝固性坏死区中Tie-2受体mRNA的表达和组织学结构变化,探讨FUS消融对凝固性坏死组织的血管生成的影响。方法 随机将4~6周龄的SPF级SD大鼠60只分为FUS组(50只),对照组(10只)。FUS局部消融SD大鼠胰腺组织后,分别取消融后1周,2周,1个月,3个月,6个月和对照组的胰腺组织。用RT-PCR的方法,以GAPDH基因为内参,进行半定量分析检测Tie-2mRNA的表达;同时HE染色、光学显微镜下观察凝固性坏死区组织学的变化;免疫组织化学方法检测胰腺组织中Tie-2的表达。结果 RT-PCR检测到Tie-2mRNA在对照组和FUS消融组织中均有表达。与对照组相比,FUS消融后1、2周Tie-2mRNA的表达量降低(P〈0.01),消融后1个月mRNA表达量增多,于消融后3个月达到最大值(P〈0.01),消融后6个月表达量逐渐降低,但仍高于对照组(P〈0.01)。组织学表明,FUS消融后在凝固性坏死区有新生血管的逐步形成。免疫组织化学表明,在正常的胰腺组织中有Tie-2的表达,但在FUS消融胰腺组织中Tie-2的表达增多。结论 在FUS消融后6个月内,大鼠胰腺凝固性坏死区域内有新生血管的生成,且比正常血管生成速度快。 展开更多

With the development of imaging and localization studies,focused parathyroidectomy with use of intraoperative parathormone monitoring(IPM)is the mainstay of treatment for primary hyperparathyroidism at many health care centers both nationally and internationally.Focused parathyroidectomy guided by IPM allows for surgical excision of the offending parathyroid gland through smaller incisions.The Miami criterion is a protocol that uses a“>50%parathormone(PTH)drop”from either the greatest pre-incision or pre-excision measurement of PTH in a blood sample taken 10 min following resection of hyperfunctioning glands.Following removal of the hyperfunctioning parathyroid gland,a>50%PTH drop at 10 min indicates completion of parathyroidectomy,and predicts operative success at 6 mo.IPM using the Miami criterion has demonstrated equal curative rates of>97%,which is comparable to the traditional bilateral neck exploration.The focused approach,however,is associated with shorter recovery times,improved cosmesis,and lower risk of postoperative hypocalcemia. 展开更多

子宫肌瘤是女性生殖系统最常见的良性病变。文献报道,育龄期妇女中发病率高达70%,其中15%~30%有临床症状,需治疗[1]。传统治疗子宫肌瘤的方法为子宫全切术或肌瘤剔除术,治疗创伤大,术后恢复期长,并发症及不良反应发生率高。近10年来,子... 子宫肌瘤是女性生殖系统最常见的良性病变。文献报道,育龄期妇女中发病率高达70%,其中15%~30%有临床症状,需治疗[1]。传统治疗子宫肌瘤的方法为子宫全切术或肌瘤剔除术,治疗创伤大,术后恢复期长,并发症及不良反应发生率高。近10年来,子宫动脉栓塞术(Uterine Fibroid Embolization, UAE)、高强度聚焦超声(High Intensity Focused Ultrasound, HIFU)、及超声引导经皮微波消融(percutaneous microwave ablation, PMWA)等微创及无创治疗子宫肌瘤的方法在临床上得到了广泛的应用,这些方法不仅避免了手术切除带来的风险,还具有创伤小、可保留患者子宫、治疗后恢复时间短等优点,已成为国内外研究的热点。不少研究表明MRI是用于检测和评价子宫肌瘤最准确的成像技术,磁共振成像(magnetic resonance imaging, MRI)无论是在微创治疗前患者的选择、预测治疗疗效、评估潜在的治疗风险、治疗中手术的操作及管理,还是评估治疗效果及诊断潜在并发症方面都具有很高的应用价值,已成为微创治疗子宫肌瘤前后首选的影像学检查方法[2]。本文将MRI在UAE、HIFU、PMWA等微创及无创治疗中的应用价值综述如下。 展开更多
