Improved hemostasis with major hepatic resection in the current surgical era 预览

Background: Major hepatic resection, predominantly performed for oncologic intent, is a complex procedure with the potential for severe intraoperative hemorrhage. The current surgical era has the ability to improve hemostasis throughout the performance of major hepatic resections which decreases blood transfusions and the detrimental effects associated with transfusion. We evaluated hemostasis and outcomes in the current surgical era of performing hepatic resections. Methods: Utilizing the American College of Surgeons (ACS) National Surgical Quality Improvement Program (NSQIP) database all major hepatic resections performed between 2012 and 2016 were analyzed in regards to hemostasis. Hemostasis was evaluated by the need for and magnitude of blood transfusions. Additional perioperative variables (including operative time, length of hospital stay, and mortality rates) were analyzed to assess for outcomes with hemostasis. The NSQIP results were compared to previous publications involving major hepatic resections to detect improvement in hemostasis and outcomes in the current surgical era. Results: A total of 22777 major hepatic resections met the inclusion criteria for analysis in the NSQIP database. An additional 21198 cases were compiled within the selected publications for comparative analysis. The transfusion rate in the current surgical era was 13.3% versus 38.7% in the previous era ( P = 0.0001). When a transfusion was required in the current surgical era there was a two-fold reduction in the number of units transfused (1.5 U vs. 3.8 U, P = 0.0001). Statistically significant improvements in operative time and length of hospital stay were presented within the current surgical era ( P = 0.0001). When a transfusion was required there was an increased relative risk score of 7 for mortality (4.9% vs. 0.7%, P = 0.0001), however, improvement in mortality rates did not reach statistical significance across surgical eras (1.3% vs. 4.0%, P = 0.0001). 本研究对Zhou氏土壤DNA抽提方法进行了改良,减少了土壤用量、孵育和裂解时间,增加了土壤淋洗和硫酸铝处理步骤。接着比较了改良法和Zhou氏法抽提四种类型土壤DNA的效果。结果表明,将土壤用量减少至0.4 g、孵育和裂解时间分别降至20 min... 本研究对Zhou氏土壤DNA抽提方法进行了改良,减少了土壤用量、孵育和裂解时间,增加了土壤淋洗和硫酸铝处理步骤。接着比较了改良法和Zhou氏法抽提四种类型土壤DNA的效果。结果表明,将土壤用量减少至0.4 g、孵育和裂解时间分别降至20 min和30 min,极大地简易了操作过程,大幅节省了实验所需时间(约为Zhou氏法所需时间的一半)。裂解前增加淋洗步骤和裂解过程中加入硫酸铝都提高了DNA的纯度。前者同时提高了DNA得率,但后者稍微降低了DNA得率。与Zhou氏法相比较,改良法提取4种类型土壤(黑土,黄土,潮土,红土)的DNA在得率和纯度方面均得到提高,并且可以直接用于16S rRNA基因的PCR扩增等分子操作。总之,本改良法简便了DNA提取过程、大大节省了实验时间,可用于从不同类型土壤中抽提适合于PCR扩增等分子操作的DNA。 展开更多

We design and fabricate a good performance silicon photoconductive terahertz detector on sapphire substrates at room temperature.The best voltage responsivity of the detector is 6679 V/W at frequency 300 GHz as well as low voltage noise of 3.8 nV/Hz1/2 for noise equivalent power 0.57 pW/Hz1/2.The measured response time of the device is about 9 μs,demonstrating that the detector has a speed of>110 kHz.The achieved good performance,together with large detector size(acceptance area is 3μm×160μm),simple structure,easy manufacturing method,compatibility with mature silicon technology,and suitability for large-scale fabrication of imaging arrays provide a promising approach to the development of sensitive terahertz room-temperature detectors. 展开更多

For the target threat evaluation of warships formation air defense, the sample data are frequently insufficient and even incomplete. The existing evaluation methods rely too much on expertise and are difficult to carry out for the dynamic evaluation on time series. In order to solve these problems, a threat evaluation method based on the AR(p)(auto regressive(AR))-dynamic improved technique for order preference by similarity to ideal solution(DITOPSIS) method is proposed. The AR(p) model is adopted to predict the missing data on the time series. Then, the entropy weight method is applied to solve each index weight at the objective point. Kullback-Leibler divergence(KLD) is used to improve the traditional TOPSIS, and to carry out the target threat evaluation. The Poisson distribution is used to assign the weight value.Simulation results show that the improved AR(p)-DITOPSIS threat evaluation method can synthetically take into account the target threat degree in time series and is more suitable for the threat evaluation under the condition of missing the target data than the traditional TOPSIS method. 展开更多

Its economic development, industrial structure and ecological environment have undergone great changes. Research on changes in Zhuhai’s land ecological security is of great significance. Using relevant data from 2007-2012, this study established a land ecological security assessment system based on the PSR conceptual framework model. The system contained 18 indicators from 3 aspects according to the concrete features of Zhuhai. Then we used the matterelement analysis and the improved entropy weight to analyze and evaluate the land ecological security of Zhuhai. The results showed that: from 2007 to 2012, the levels of the land ecological security of Zhuhai were “secure”, and the value increased year by year;as the land ecological security response value increased, Zhuhai was capable of solving land ecosystem problems. However, it should be noted that the structure of land ecosystem in Zhuhai has not formed and that rapid expansion of construction land has caused the shortage of cultivated land and other issues. Measures should be taken to control the construction area, improve land intensive utilization and improve the land ecological security. 展开更多

An efficient solution-processable route employing Pb(Ac)2 as lead source and anti-solvent treatment to achieve fully covered and homogenous perovskite films is reported. The effect of different solution methods and device architectures on the morphologies of perovskite films were systematically investigated. Our results show that the planar perovskite layer fabricated by one-step solution method achieved fully covered and pinhole-free films. Further anti-solvent treatment using chlorobenzene (CB) promoted a perovskite film with highly smooth surfaces and enlarged grain sizes. Device fabricated from CB treated perovskite film achieved a best PCE of 15.80%, in comparison with 14.02%for the untreated device. These results evidently suggest a feasible route towards controlling the crystallization and morphology of planar heterojunction (PHJ) PSCs for improved efficiency. 展开更多

The improved performance of a wavelength-tunable arrayed waveguide grating (AWG) is demonstrated, including the crosstalk, insertion loss and the wavelength tuning efficiency. A reduced impact of the fabrication process on the AWG is achieved by the design of bi-level tapers. The wavelength tuning of the AWG is achieved according to the thermo-optic effect of silicon, and uniform heating of the silicon waveguide layer is achieved by optimizing the heater design. The fabricated AWG shows a minimum crosstalk of 16 dB, a maximum insertion loss of 3.91 dB and a wavelength tuning efficiency of 8.92 nm/W, exhibiting a ~8 dB improvement of crosstalk, ~2.1 dB improvement of insertion loss and ~5 nm/W improvement of wavelength tuning efficiency, compared to our previous reported results. 展开更多

In this paper, the improved version of the meshless singular boundary method (ISBM) is developed for analyzing the hydrodynamic performance of bottom-standing submerged breakwaters in regular normally incident waves. Both the single and dual prismatic breakwaters of rectangular and trapezoidal forms are examined. Only the impermeable breakwaters are considered in this study. The physical problem is cast in terms of the Laplace equation governing an irrotational flow and incompressible fluid motion with the appropriate mixed-type boundary conditions, and it is solved numerically using the ISBM. The numerical results are presented in terms of the hydrodynamic quantities of reflection and transmission coefficients. The values are first validated against the data of previous studies, computed, and discussed for a variety of structural conditions, including the height, width, and spacing of breakwater submergence. An excellent agreement is observed between the ISBM results and those of other methods. The breakwater width is found to feature marginal effects compared with the height. The present method is shown to accurately predict the resonant conditions at which the maximum reflection and transmission occur. The trapezoidal breakwaters are found to generally present a wide spectrum of reflections, suggesting that they would function better than the rectangular breakwaters. The dual breakwater systems are confirmed to perform much better than single structures. 展开更多

The information of expression texture extracted by the completed local ternary patterns(CLTP) method is not accurate enough, which may cause low recognition rate. Therefore, an improved completed local ternary patterns(ICLTP) is proposed here. Firstly, the Scharr operator is used to calculate gradient magnitudes of images to enhance the detail of texture, which is beneficial to obtaining more accurate expression features. Secondly, two different neighborhoods of CLTP features are combined to obtain much information of facial expression. Finally, K nearest neighbor(KNN) and sparse representation classifier(SRC) are combined for classification and a 10-fold cross-validation method is tested in the JAFFE and CK+ databases. The results show that the ICLTP method can improve the recognition rate of facial expression and reduce the confusion between various expressions. Especially, the misrecognition rate of other six expressions recognized as neutral is reduced in the 7-class expression recognition. 展开更多
