AIM To establish an appropriate N classification system for early gastric cancer(EGC).METHODS Data from 10714 patients who underwent radical gastrectomy between 1988 and 2011 were retrieved from the National Cancer Institute’s Surveillance,Epidemiology,and End Result database.The overall survival(OS)based on the eighth edition and new tumor lymph node metastasis(TNM)staging systems were compared,and the analysis was repeated in an external validation set from the Fujian Medical University Union Hospital database.RESULTS There were no significant differences in OS between N1 and N2 cancers or between N3a and N3b cancers in cases of EGC.The X-tile program identified that the new staging system for EGC consisted of T1N0,T1N1’[1-6 metastatic lymph nodes(LNs)],and T1N2’(≥7 metastatic LNs).Compared with the eighth edition of the TNM staging system,the OS of patients in T1N1’stage was similar to that of patients with stage IIA disease,whereas the OS of patients in T1N2’stage was similar to that of patients with stage IIB disease.The new TNM staging system exhibited a slightly lower Akaike Information Criterion value and higher χ^2 and c-statistic compared with the eighth edition of the TNM classification system.Similar results were found in the external validation dataset from the external validation set.CONCLUSION We have developed an optional new TNM staging system with a better predictive ability that can be used to accurately predict the 5-year OS of patients with EGC. 展开更多

Tumor-induced osteomalacia with IgG4-related lymph node disease

To the Editor:A 60-year-old man presented with progressively worsened limb weakness of 4 years duration,which had rendered him wheelchair-bound since May 2018.The patient also had muscle and joint pain and numbness.He had a history of type 2 diabetes,hypertension,and gout,no family history of bone disease.He took diabetic peripheral neuropathy treatment but the symptoms kept progressing.On admission,the patient was well developed and in normal body shape.Physical examination was unremarkable except for all-sided weakness.Laboratory evaluation showed hypophosphatemia with notable elevated urinary phosphorus loss.Other laboratory abnormalities were elevated alkaline phosphatase levels,mild insufficient vitamin D status,and mild elevated parathyroid hormone.After bone scintigraphy failed to localize an osseous tumor,whole-body positron emission tomography(PET)/computed tomography(CT)using 18F-fluorodeoxyglucose(18F-FDG)and the radiolabeled somatostatin analog 68Ga-DOTATATE were performed to make clear diagnosis of oncogenic hypophosphatemic osteomalacia.Both of the examinations revealed a suspicious mass in subcutaneous tissue around the umbilicus and enlarged right axillary lymph nodes[Figure 1A and 1B]. 展开更多

Social networks are important media for spreading information, ideas, and influence among individuals.Most existing research focuses on understanding the characteristics of social networks, investigating how information is spread through the 'word-of-mouth' effect of social networks, or exploring social influences among individuals and groups. However, most studies ignore negative influences among individuals and groups. Motivated by the goal of alleviating social problems, such as drinking, smoking, and gambling, and influence-spreading problems, such as promoting new products, we consider positive and negative influences, and propose a new optimization problem called the Minimum-sized Positive Influential Node Set(MPINS) selection problem to identify the minimum set of influential nodes such that every node in the network can be positively influenced by these selected nodes with no less than a threshold of ?. Our contributions are threefold. First, we prove that, under the independent cascade model considering positive and negative influences, MPINS is APX-hard. Subsequently, we present a greedy approximation algorithm to address the MPINS selection problem. Finally, to validate the proposed greedy algorithm, we conduct extensive simulations and experiments on random graphs and seven different realworld data sets that represent small-, medium-, and large-scale networks. 展开更多

Biological elements usually exert their functions through interactions with others to form various types of biological networks.The ability of controlling the dynamics of biological networks is of enormous benefits to pharmaceutical and medical industry as well as scientific research.Though there are many mathematical methods for steering dynamic systems towards desired states,the methods are usually not feasible for applying to complex biological networks.The difficulties come from the lack of accurate model that can capture the dynamics of interactions between biological elements and the fact that many mathematical methods are computationally intractable for large-scale networks.Recently,a concept in control theory --controllability,has been applied to investigate the dynamics of complex networks.In this article, recent advances on the controllability of complex networks and applications to biological networks are reviewed.Developing dynamic models is the prior concern for analyzing dynamics of biological networks.First,we introduce a widely used dynamic model for investigating controllability of complex networks.Then recent studies of theorems and algorithms for having complex biological networks controllable in general or specific application scenarios are reviewed.Finally,applications to real biological networks manifest that investigating the controllability of biological networks can shed lights on many critical physiological or medical problems,such as revealing biological mechanisms and identifying drug targets,from a systematic perspective. 展开更多

Nowadays, hybrid satellite-terrestrial cooperative network has emerged as a key technology to provide a great variety of communication services. The deployment of this network will improve coverage and capacity in remote areas. Despite the benefits of this network, by increasing the number of users, communication efficiency based on interference management is a major challenge in satellite-based system. Also, the direct links between satellite system and the terrestrial equipment do not always have desirable channel condition. In order to avoid serious throughput degradation, choosing a cooperative relay node is very important. In this paper, Stackelberg game is exploited for interference management that is raised by satellites in down link over terrestrial equipment. Then, for interference management between ground station and relay node with other mobile users, CVX is used to allocate optimum power. Also, the best relay node in this structure is selected based on the harmonic mean function. Thus, the performance of the heterogeneous satellite-cooperative network is investigated based on three benchmarks, namely, successful transmission, energy consumption and outage probability. Finally, the simulation results showed the effect of proposed system model on the performance of next generation satellite networks. 展开更多

Learning the representations of nodes in a network can benefit various analysis tasks such as node classification, link prediction, clustering, and anomaly detection. Such a representation learning problem is referred to as network embedding, and it has attracted significant attention in recent years. In this article, we briefly review the existing network embedding methods by two taxonomies. The technical taxonomy focuses on the specific techniques used and divides the existing network embedding methods into two stages, i.e., context construction and objective design. The non-technical taxonomy focuses on the problem setting aspect and categorizes existing work based on whether to preserve special network properties, to consider special network types, or to incorporate additional inputs. Finally, we summarize the main findings based on the two taxonomies, analyze their usefulness,and discuss future directions in this area. 展开更多

Objective This study aimed to compare and analyze the clinical efficacy and safety of late-course and simultaneous integrated dose-increasing intensity-modulated radiation therapy(IMRT) for cervical cancer complicated with pelvic lymph node metastasis. Methods Sixty patients with cervical cancer complicated with pelvic lymph node metastasis who were admitted to our hospital from January 2013 to January 2015 were enrolled. The patients were randomly divided into the late-course dose-increasing IMRT group and the simultaneous integrated dose-increasing IMRT group, with 30 cases included in each group, respectively. All patients were concurrently treated with cisplatin. After treatment, the clinical outcomes of the two groups were compared. Results The remission rate of symptoms in the simultaneous integrated dose-increasing IMRT group was significantly higher than that in the late-course dose-increasing IMRT group(P < 0.05). The follow-up results showed that the overall survival time, progression-free survival time, and distant metastasis time of patients in the simultaneous integrated dose-increasing IMRT group were significantly longer than those in the late-course dose-increasing IMRT group(P < 0.05). The recurrent rate of lymph nodes in the radiation field in the simultaneous integrated dose-increasing IMRT group was significantly lower(P < 0.05) than in the late-course dose-increasing IMRT group. There was no significant difference in the incidence of cervical and vaginal recurrence and distant metastasis between the two groups(P > 0.05). The radiation doses of Dmax in the small intestine, D1 cc(the minimum dose to the 1 cc receiving the highest dose) in the bladder, and Dmax in the rectum in the simultaneous integrated dose-increasing IMRT group were significantly lower(P < 0.05) than in the late-course dose-increasing IMRT group.
