Background: Central hepatectomy(CH) is more difficult than extended hepatectomy(EH) and is associated with greater morbidity. In this modern era of liver management with aims to prevent posthepatectomy liver failure(PHLF), there is a need to assess outcomes of CH as a parenchyma-sparing procedure for centrally located liver tumors. Methods: A total of 178 major liver resections performed by specialist surgeons from two Australian tertiary institutions between June 2009 and March 2017 were reviewed. Eleven patients had CH and 24 had EH over this study period. Indications and perioperative outcomes were compared between the groups. Results: The main indication for performing CH was colorectal liver metastases. There was no perioperative mortality in the CH group and four(16.7%) in the EH group( P = 0.285). No group differences were found in median operative time [CH vs. EH: 450 min(290–840) vs. 523 min(310–860), P = 0.328], intraoperative blood loss [850 mL(40 0–150 0) vs. 650 mL(10 0–20 0 0), P = 0.746] or patients requiring intraoperative blood transfusion [1(9.1%) vs. 7(30.4%), P = 0.227]. There was a trend towards fewer hepatectomyspecific complications in the CH group [3(27.3%) vs. 13(54.2%), P = 0.167], including PHLF(CH vs. EH: 0 vs. 29.2%, P = 0.072). Median length of stay was similar between groups [CH vs. EH: 9 days(5–23) vs. 12 days(4–85), P = 0.244]. Conclusions: CH has equivalent postoperative outcomes to EH. There is a trend towards fewer hepatectomy-specific complications, including PHLF. In appropriate patients, CH may be considered as a safe parenchyma-sparing alternative to EH. 展开更多

OBJECTIVE: To calculate and compare the cost of Port and PICC’s application in long-term intravenous administration, and to support the decision making of hospital manager. METHODS: Literature review and patient survey in 2 oncology centers in China were carried out to investigate the cost and impact of Port and PICC for patients. The cost at different time of intravenous administration was calculated and compared. One-way sensitivity analysis was performed and tornado graph was drawn. RESULTS: Direct cost of Port at 0.5, 1, 1.5, and 2 years were7442, 8005, 8553, and 9131 CNY, and 4700, 9399, 14032, 18799 CNY for PICC respectively. Direct & indirect cost at 0.5, 1, 1.5, and 2 years were 9291, 11704, 14101, 16529 CNY for Port and 9697, 19393, 29023, 38787 CNY for PICC. Sensitivity analysis showed that productivity loss and device maintenance cost were the most in?uential factors to the result. CONCLUSION: Port had higher cost in short term and less in long term compared with PICC. Patients expected to get intravenous administration more than 0.5 year should use Port if both direct and indirect costs were included. 展开更多

BACKGROUND Pre-transplant nutrition is a key driver of outcomes following liver transplantation in children. Patients with biliary atresia (BA) may have difficulty achieving satisfactory weight gain with enteral nutrition alone, and parenteral nutrition (PN) may be indicated. While PN has been shown to improve anthropometric parameters of children with BA listed for liver transplantation, less is known about the risks, particularly infectious, associated with this therapy among this specific group of patients. AIM To describe the incidence, microbiology, and risk factors of central line-associated bloodstream infection (CLABSI) among children with BA listed for liver transplantation. METHODS Retrospective review of children aged ≤ 2-years of age with BA who were listed for primary liver transplantation at Texas Children’s Hospital from 2008 through 2015 (n = 96). Patients with a central line for administration of PN (n = 63) were identified and details of each CLABSI event were abstracted. We compared the group of patients who experienced CLABSI to the group who did not, to determine whether demographic, clinical, or laboratory factors correlated with development of CLABSI. RESULTS Nineteen of 63 patients (30%, 95%CI: 19, 43) experienced 29 episodes of CLABSI during 4800 line days (6.04 CLABSI per 1000 line days). CLABSI was predominantly associated with Gram-negative organisms (14/29 episodes, 48%) including Klebsiella spp., Enterobacter spp., and Escherichia coli. The sole polymicrobial infection grew Enterobacter cloacae and Klebsiella pneumoniae. Grampositive organisms (all Staphylococcus spp.) and fungus (all Candida spp.) comprised 9/29 (31%) and 6/29 (21%) episodes, respectively. No demographic, clinical, or laboratory factors were significantly associated with an increased risk for the first CLABSI event in Cox proportional hazards regression analysis CONCLUSION There is substantial risk for CLABSI among children with BA listed for liver transplantation. Geochronology and Petrogenesis of Mafic-Intermediate Intrusions on the Northern Margin of the Central Tianshan(NW China): Implications for Tectonic Evolution

Late Paleozoic mafic-intermediate intrusions are widespread in the central Tianshan belt, but their tectonic settings remain controversial. Petrogenesis of these intrusions can provide insights into the tectonic evolution of the belt and its adjacent areas. This study presents new whole-rock geochemical and zircon U-Pb geochronology and Hf isotopic data for the Yaxi diorite and Qianzishan gabbro on the northern margin of the central Tianshan(NW China). Zircon U-Pb dating yielded the magma crystallization ages of 313±4 and 295±4 Ma for the Yaxi diorite and Qianzishan gabbro, respectively. They have lower Mg# values, Ni and Cr contents than typical mantle-derived primary melt, with negative correlations between MgO, TFeO and SiO2 contents, indicating clinopyroxene and olivine fractionation during magma evolution. They are characterized by enrichment of large ion lithophile elements(e.g., Rb, Ba and Sr) and depletion of high field strength elements(e.g., Nb, Ta and Ti) with high Ba/Th and Rb/Y, suggesting that their mantle sources had been metasomatized by slab-derived fluids. In addition, the Qianzishan gabbro has high Al2 O3 contents(19.54 wt.%–20.88 wt.%) and positive Eu anomalies(Eu/Eu*=1.09–1.42), which can be attributed to accumulation of plagioclase. Geochemical and zircon Hf isotopic compositions reveal that both the Yaxi diorite and Qianzishan gabbro were derived from depleted lithospheric mantle in the spinel stability field with insignificant crustal contamination. In association with previous investigations, we suggest that the Yaxi and Qianzishan intrusions were emplaced in a subduction-related environment, which means that the subduction of the Junggar Ocean lasted at least to the earliest Permian. 展开更多

AIM: To evaluate the diagnostic value of B-scan ultrasound and explore the cytological characteristics of patients with vitreoretinal lymphoma(VRL) and primary central nervous system lymphoma(PCNSL).METHODS: The clinical data and pathologic specimens from patients with VRL diagnosed at the North Huashan Hospital from 2016 to 2017 were retrospectively reviewed. The patients were diagnosed by slit lamp ophthalmoscopy, B-scan ultrasound, cytology of the vitreous, which was obtained by vitrectomy, and cytokine measurements of interleukin(IL)-10 and IL-6.RESULTS: Twenty-six eyes(19.4%) out of 134 eyes of 67 patients(47 men and 20 women) with PCNSL were diagnosed with VRL by B-scan ultrasound, and 14 eyes(10.4%) were diagnosed by slit lamp ophthalmoscopy. Twenty-four eyes(17.9%) of 17 patients were confirmed as having VRL with cytology. No difference in the association between intracranial lesion location and ocular involvement was found. VRL patients had higher levels of vitreous IL-10 and IL-10/IL-6 when compared with macular hole cases, but the difference was not statistically significant.CONCLUSION: A total of 25.4% of the PCNSL patients had VRL, B-scan ultrasound examination had characteristic features and is recommended over slit lamp ophthalmoscopy for the screening diagnosis of PCNSL with intraocular involvement. Moreover, the cytological and immunohistochemical analyses performed after 25-gauge diagnostic vitrectomy were accurate diagnostic techniques. 展开更多

Extracellular vesicles,including exosomes and microvesicles,play a fundamental role in the activity of the nervous system,participating in signal transmission between neurons and providing the interaction of central nervous system with all body systems.In many neurodegenerative diseases,neurons pack toxic substances into vesicles and release them into the extracellular space,which leads to the spread of misfolded neurotoxic proteins.The contents of neuron-derived extracellular vesicles may indicate pathological changes in the central nervous system,and the analysis of extracellular vesicle molecular content contributes to the development of non-invasive methods for the diagnosis of many central nervous system diseases.Extracellular vesicles of neuronal origin can be isolated from various biological fluids due to their ability to cross the blood-brain barrier.Today,the diagnostic potential of almost all toxic proteins involved in nervous system disease pathogenesis,specificallyα-synuclein,tau protein,superoxide dismutase 1,FUS,leucine-rich repeat kinase 2,as well as some synaptic proteins,has been well evidenced.Special attention is paid to extracellular RNAs mostly associated with extracellular vesicles,which are important in the onset and development of many neurodegenerative diseases.Depending on parental cell type,extracellular vesicles may have different therapeutic properties,including neuroprotective,regenerative,and anti-inflammatory.Due to nano size,biosafety,ability to cross the blood-brain barrier,possibility of targeted delivery and the lack of an immune response,extracellular vesicles are a promising vehicle for the delivery of therapeutic substances for the treatment of neurodegenerative diseases and drug delivery to the brain.This review describes modern approaches of diagnosis and treatment of central nervous system diseases using extracellular vesicles. 展开更多

Resistant hypertension (RH) has a prevalence of around 12% and is associated with an increased risk of cardiovascular disease,progression to end-stage renal disease,and even mortality.In 2017,the American College of Cardiology and American Heart Association released updated guidelines that detail steps to ensure proper diagnosis of RH,including the exclusion of pseudoresistance.Lifestyle modifications,such as low salt diet and physical exercise,remain at the forefront of optimizing blood pressure control.Secondary causes of RH also need to be investigated,including screening for obstructive sleep apnea.Notably,the guidelines demonstrate a major change in medication management recommendations to include mineralocorticoid receptor antagonists.In addition to advances in medication optimization,there are several device-based therapies that have been showing efficacy in the treatment of RH.Renal denervation therapy has struggled to show efficacy for blood pressure control,but with a redesigned catheter device,it is once again being tested in clinical trials.Carotid baroreceptor activation therapy (BAT) via an implantable pulse generator has been shown to be effective in lowering blood pressure both acutely and in longterm follow up data,but there is some concern about the safety profile.Both a second-generation pulse generator and an endovascular implant are being tested in new clinical trials with hopes for improved safety profiles while maintaining therapeutic efficacy.Both renal denervation and carotid BAT need continued study before widespread clinical implementation.Central arteriovenous anastomosis has emerged as another possible therapy and is being actively explored.The ongoing pursuit of blood pressure control is a vital part of minimizing adverse patient outcomes.The future landscape appears hopeful for helping patients achieve blood pressure goals not only through the optimization of antihypertensive medications but also through device-based therapies in select individuals. 展开更多

Spinal cord injury is a devastating condition that is followed by long and often unsuccessful recovery after trauma. The state of the art approach to manage paralysis and concomitant impairments is rehabilitation, which is the only strategy that has proven to be effective and beneficial for the patients over the last decades. How rehabilitation influences the remodeling of spinal axonal connections in patients is important to understand, in order to better target these changes and define the optimal timing and onset of training. While clinically the answers to these questions remain difficult to obtain, rodent models of rehabilitation like bicycling, treadmill training, swimming, enriched environments or wheel running that mimic clinical rehabilitation can be helpful to reveal the axonal changes underlying motor recovery. This review will focus on the different animal models of spinal cord injury rehabilitation and the underlying changes in neuronal networks that are improved by exercise and rehabilitation. 展开更多

Objective To investigate the difference in tumor conventional imaging findings and texture features on T2 weighted images between glioblastoma and primary central neural system (CNS) lymphoma. Methods The pre-operative MRI data of 81 patients with glioblastoma and 28 patients with primary CNS lymphoma admitted to the Chinese PLA General Hospital and Hainan Hospital of Chinese PLA General Hospital were retrospectively collected. All patients underwent plain MR imaging and enhanced T1 weighted imaging to visualize imaging features of lesions. Texture analysis of T2 weighted imaging (T2WI) was performed by use of GLCM texture plugin of ImageJ software, and the texture parameters including Angular Second Moment (ASM), Contrast, Correlation, Inverse Difference Moment (IDM), and Entropy were measured. Independent sample t-test and Mann-Whitney U test were performed for the between-group comparisons, regression model was established by Binary Logistic regression analysis, and receiver operating characteristic (ROC) curve was plotted to compare the diagnostic efficacy. Results The conventional imaging features including cystic and necrosis changes (P=0.000),‘Rosette’ changes (P=0.000) and ‘incision sign’(P=0.000), except ‘flame-like edema’(P=0.635), presented significantly statistical difference between glioblastoma and primary CNS lymphoma. The texture features, ASM, Contrast, Correlation, IDM and Entropy, showed significant differences between glioblastoma and primary CNS lympoma (P=0.006,0.000, 0.002, 0.000, and 0.015 respectively). The area under the ROC curve was 0.671, 0.752, 0.695, 0.720 and 0.646 respectively, and the area under the ROC curve was 0.917 for the combined texture variables (Contrast, cystic and necrosis,‘Rosette’ changes, and ‘incision sign’) in the model of Logistic regression. Characteristics of central visual field defect after macular hole surgery

AIM: To investigate the characteristics of postoperative central visual field defect(cVFD) in patients with macular hole(MH).METHODS: Eighteen eyes from 18 MH patients were involved in this retrospective study which reviewed square root of loss variance(sLV) and mean defect(MD) of the visual field test in all subjects. The relationship between cVFD and MH stage, as well as the postoperative ellipsoid zone disruption were evaluated using Spearman’s correlation test. RESULTS: Our analysis determined Spearman coefficient is 0.705 for the correlation between sLV and MH stage(P<0.01), 0.877 for the correlation between sLV and postoperative ellipsoid zone disruption(P<0.01) and 0.721 for the correlation between MD and postoperative ellipsoid zone disruption(P<0.01). A significant relationship was also detected between postoperative ellipsoid zone disruption and MH stage(r=0.470, P<0.05). Univariate regression analysis indicated that sLV and MD were associated with postoperative ellipsoid zone disruption(P<0.01, P<0.01, respectively). CONCLUSION: Postoperative cVFD is highly correlated with MH stage and postoperative ellipsoid zone disruption in patients with MH. 展开更多

BACKGROUND Due to some similarities in the manifestations between central serous chorioretinopathy (CSC) and polypoidal choroidal vasculopathy (PCV),PCV may be misdiagnosed as CSC.More attention should be paid to distinguishing these two disorders.CASE SUMMARY A 52-year-old woman presented to our hospital with blurred vision in her left eye for approximately 1 wk.Anterior segment and intraocular pressure findings were normal in both eyes.Fundus photography of the left eye showed a seemingly normal adult oculus fundus without any obvious hard exudate or hemorrhage.Optical coherence tomography exhibited a hypo-reflective space beneath both the neurosensory retina and the pigment epithelium layer.The late phase of fluorescein angiography revealed increased leakage.The patient was initially diagnosed with CSC.At follow-up,however,the final diagnosis turned out to be PCV.CONCLUSION CSC and PCV are two different retinal entities.Lipid deposition and hemorrhage are the most important elements that lead to confusion between these two entities.Indocyanine green angiography should be performed to make a definitive diagnosis,especially in cases with suspected PCV. 展开更多

In the control scheme, an artificial ... We propose a biomimetic approach for steering motion control of a snake robot. Inspired by a vertebrate biological motor system paradigm, a hierarchical control scheme is adopted. In the control scheme, an artificial central pattern generator(CPG) is employed to generate serpentine locomotion in the robot. This generator outputs the coordinated desired joint angle commands to each lower-level effector controller, while the locomotion can be controlled through CPG modulation by a higher-level motion controller. The motion controller consists of a cerebellar model articulation controller(CMAC) and a proportional-derivative(PD) controller. Because of the fast learning ability of the CMAC, the proposed motion controller can drive the robot to track the desired orientation and adapt to unexpected perturbations. The PD controller is employed to expedite the convergence speed of the motion controller. Finally, both numerical studies and experiments proved that the proposed approach can help the snake robot achieve good tracking performance and adaptability in a varying environment. 展开更多

AIM: To evaluate baseline foveal disorganisation of retinal inner layers(DRIL) as a prognostic factor in eyes with central retinal artery occlusion(CRAO). METHODS: Twenty-eight CRAO patients who were followed-up between 2010 and 2016 were retrospectively investigated. Demographic characteristics and detailed ophthalmological examination findings of all patients were recorded. Macular thicknesses(MTs) from 5 separate spots and DRIL were measured with spectral-domain optic coherence tomography(SD-OCT). Correlations between DRIL score and logMAR converted visual acuity(VA), change in VA, patient reference time(RT), number of hyperbaric oxygen therapy(HBOT) sessions, MT and MT change were investigated.RESULTS: There was a positive correlation between the DRIL score and the final VA(r=0.787) and a negative correlation with the change in VA(r=-0.763). The RT and MT were closely related to the DRIL score. A negative correlation was found between the number of HBOT sessions and the DRIL score(r=-0.341).CONCLUSION: The DRIL score is a parameter assessed by SD-OCT, which can provide us reliable information regarding the prognosis of visual functions and response to the treatment for CRAO patients at acute phase. 展开更多

This study investigates why an extreme hot midsummer occurred in Central and South China(CSC)during 2017.It is shown that the western North Pacific subtropical high(WNPSH)was abnormally intensified and westward-extending,resulting in anomalous high pressure and consequent extreme heat over CSC.The abnormal WNPSH was favored by the warming of the western tropical Pacific(WTP),which was unrelated to ENSO and manifested its own individual e ect.The WTP warming enhanced the convection in-situ and led to anomalous high pressure over CSC via a local meridional circulation.The influence of the WTP was confirmed by CAM4 model experiments.A comparison between the 2017 midsummer and 2010 midsummer(with a stronger WNPSH but weaker extreme heat)indicated that the influence of the WNPSH on extreme heat can be modulated by the associated precipitation in the northwestern flank.The role of the WTP was verified by regression analyses on the interannual variation of the WTP sea surface temperature anomaly(SSTA).On the other hand,the WTP has undergone prominent warming during the past few decades,resulting from decadal to long-term changes and favoring extreme warm conditions.Through a mechanism similar to the interannual variation,the decadal to long-term changes have reinforced the influence of WTP warming on the temperature over CSC,contributing to the more frequent hot midsummers recently.It is estimated that more than 50%of the temperature anomaly over CSC in the 2017 midsummer was due to the WTP warming,and 40%was related to the decadal to long-term changes of the WTP SSTA. 展开更多

We demonstrated a method for measurement of central corneal thickness(CCT) with a sub-micrometer sensitivity using a spectral domain optical coherence tomography system without needing a super broad bandwidth light source. By combining the frequency and phase components of Fourier transform, the method is capable of measurement of a large dynamic range with a high sensitivity. Absolute phases are retrieved by comparing the correlations between the detected and simulated interference fringes. The phase unwrapping ability of the present method was quantitatively tested by measuring the displacement of a piezo linear stage. The human CCTs of six volunteers were measured to verify its clinical application. It provides a potential tool for clinical diagnosis and research applications in ophthalmology. 展开更多
