As the theoretical source of Traditional Chinese Medicine clinical medicine,Shanghan Lun plays a vital role in guiding the diagnosis and treatment of clinical diseases.Stress is a systemic nonspecific and adaptive response that occurs when the body is stimulated by internal and external environmental factors.This paper discusses the correlation between the three phases of stress containing alarm,resistance,and exhaustion and the three yin and three yang diseases.It was concluded that sanyang diseases were related to the alarm stage and resistance stage,and sanyin diseases were related to the failure stage.The results indicated the essence of diseases of six meridians from the perspective of stress and provided reference for the diagnosis and treatment of six classics diseases. 展开更多

Sarcopenia (“sarx” for muscle,“penia” for loss) is an important problem in the elderly. Although muscle loss is a part of natural aging, excessive loss that limits physical activity is considered pathological. Sarcopenia is associated with age, malnutrition, physical inactivity, inflammatory stress and hormonal changes. Although relationships between sarcopenia and various chronic inflammatory diseases have been shown, the role of rheumatologic disease in sarcopenia development is currently unknown. Our aim in this mini-review was to increase the awareness of clinicians to sarcopenia, and to evaluate studies in which the relationship between sarcopenia and rheumatologic diseases was investigated. We also aimed to determine whether the available literature was sufficient to confirm a strong relationship between these conditions. Although our findings showed that diseases such as rheumatoid arthritis, osteoarthritis and systemic sclerosis may have a role in sarcopenia development and progress, the methodologies and results of the majority of studies were insufficient in determining direct causal relationships. We believe future studies would benefit from focusing on the factors and causes of sarcopenia, with a goal of determining the factors associated with rheumatologic disease that are most effective in sarcopenia development. 展开更多

The incidence of heart valve disease increases significantly with age.Degenerative abnormalities associated with severe aortic stenosis and mitral and tricuspid regurgitation are found in not less than 10%of the population aged≥75 years.Surgical treatment has been considered for years to be the treatment of choice.However,it was not uncommonly associated with high perioperative morbidity and mortality due to frequent comorbidities and overall frailty conditions of these patients.Conventional risk scores such as Society of Thoracic Surgeons and European System for Cardiac Operative Risk Evaluation may underestimate the risk of surgery in elderly patients,leading to inappropriate surgical indication.On the other hand,at least 30% of patients with severe conditions are left untreated due to prohibitive surgical risk.Interventional procedures,which are in continuous development,may be actually considered for high risk patients and,as recent results suggest,also for intermediate risk patients. 展开更多

Following oligodendrocyte injury, newly generated myelinating oligodendrocytes derived from oligodendrocyte progenitors (OPCs) may produce new myelin sheaths around denuded axons (remyelination) restoring neuronal function (Verden and Macklin, 2016). However, impacts of climate change-induced water scarcity on health and well-being are complex.About 80% of illnesses in developing countries are attributed to unsafe drinking water and waterborne diseases. In Southwestern Bangladesh, lack of safe drinking water is a severe crisis due to climate change. The study investigated the impacts of climate change on water resources and human health in a coastal area. A questionnaire survey was carried out in two villages of Shymnagar upazila on the southwestern coast to investigate the present status of safe water sources and health care facilities and their impacts on the local community. The results show that the local community believes that climate change is having substantial impacts on freshwater sources and health. More than 70% of the respondents identified diarrhea, dysentery,and skin diseases as the prime waterborne health risks that occur through climate-related safe water scarcity. By synthesizing the ground data, we suggest pathways to health adaptation to climate change effects and safe water scarcitythrough locally available adaptive practices such as the use of pond sand filters, rainwater harvesting, and importing potable water with the active participation of the government,nongovernmental organizations, and communities. 展开更多

It depletes the level of mature B‐cells by various mechanisms such as mediation of antibody‐ dependent cellular cytotoxicity, complement‐dependent cytotoxicity, and B‐cell apoptosis. Rituximab is a USFDA approved drug for clinical use in non‐Hodgkin's B‐cell lymphoma (NHL), rheumatoid arthritis, chronic lymphocytic leukemia (CLL), granulomatosis with polyangiitis and pemphigus vulgaris. It is also known for its “off label” use in renal disease and renal transplant worldwide. However, the exact mechanisms by which it exerts its effect in the aforementioned condition remain unclear but may be related to its long‐term effects on plasma cell development and the impact on B‐cell modulation of T cell responses. The superfamily of G protein-coupled receptors (GPCRs) contains immense structural and functional diversity and mediates a myriad of biological processes upon activation by various extracellular signals.Critical roles of GPCRs have been established in bone development,remodeling,and disease.Multiple human GPCR mutations impair bone development or metabolism,resulting in osteopathologies.Here we summarize the disease phenotypes and dysfunctions caused by GPCR gene mutations in humans as well as by deletion in animals.To date,92 receptors (5 glutamate family,67 rhodopsin family,5 adhesion,4 frizzled/taste2 family,5 secretin family,and 6 other 7TM receptors) have been associated with bone diseases and dysfunctions (36 in humans and 72 in animals).By analyzing data from these 92 GPCRs,we found that mutation or deletion of different individual GPCRs could induce similar bone diseases or dysfunctions,and the same individual GPCR mutation or deletion could induce different bone diseases or dysfunctions in different populations or animal models.Data from human diseases or dysfunctions identified 19 genes whose mutation was associated with human BMD:9 genes each for human height and osteoporosis;4 genes each for human osteoarthritis (OA) and fracture risk;and 2 genes each for adolescent idiopathic scoliosis (AIS),periodontitis,osteosarcoma growth,and tooth development.Reports from gene knockout animals found 40 GPCRs whose deficiency reduced bone mass,while deficiency of 22 GPCRs increased bone mass and BMD;deficiency of 8 GPCRs reduced body length,while 5 mice had reduced femur size upon GPCR deletion.Furthermore,deficiency in 6 GPCRs induced osteoporosis;4 induced osteoarthritis;3 delayed fracture healing;3 reduced arthritis severity;and reduced bone strength,increased bone strength,and increased cortical thickness were each observed in 2 GPCR-deficiency models.The ever-expanding number of GPCR mutation-associated diseases warrants accelerated molecular analysis,population studies,and investigation of phenotype correlat 展开更多

Methods: A total of 1000 elderly patients with chronic diseases in our health center were randomly selected as the study subject. Questionnaire survey was conducted by self-designed questionnaire. The SF-36 quality of life scale was used to evaluate their quality of life;and linear regression model was used to explore the relevant influencing factors. Result: The SF-36 score showed that there was a correlation between the six dimensions, including overall health, physiological function, somatic pain, vitality, social function and mental health, and the number of chronic diseases in elderly patients. With the increase of chronic diseases, the scores of each dimension showed a downward trend. Linear regression analysis showed that age, anxiety and poor sleep quality were the main factors affecting the quality of life in elderly patients with chronic diseases. Conclusion: The quality of life of elderly patients with chronic diseases in our city is poor. In community work, targeted nursing and intervention should be given according to the types of diseases suffered by the elderly. 展开更多

目的调查某三甲医院抗菌药物临床使用情况。方法利用信息系统数据库提取所有抗菌药物品种、消耗量及消耗金额,各病区住院患者出院人数及平均住院天数,以及各病区收治疾病种类,利用Excel2007软件进行统计分析。结果 2017-2018年使用排名... 目的调查某三甲医院抗菌药物临床使用情况。方法利用信息系统数据库提取所有抗菌药物品种、消耗量及消耗金额,各病区住院患者出院人数及平均住院天数,以及各病区收治疾病种类,利用Excel2007软件进行统计分析。结果 2017-2018年使用排名前三位的抗菌药物种类为青霉素类+β-内酰胺酶抑制剂、一至三代头孢菌素、喹诺酮类;2017-2018年全年抗菌药物使用强度分别为35.36、35.67,2017-2018年全年使用强度最高的科室为:重症医学科、重症烧伤科、呼吸科、普外科和儿科;2017-2018年收治感染性疾病患者最多的科室为均为呼吸科、儿科和普外科。结论某三家医院2018年抗菌药物使用强度增长与该院收治患者病种变化相符,合理用药意识有所提升。 展开更多
