Neurodegenerative diseases such as Alzheimer's disease, Parkinson's disease, Huntington's disease, and amyotrophic lateral sclerosis are a heterogeneous group of debilitating disorders with multifactorial etiologies and pathogeneses that manifest distinct molecular mechanisms and clinical manifestations with abnormal protein dynamics and impaired bioenergetics. Mitochondrial dysfunction is emerging as an important feature in the etiopathogenesis of these age-related neurodegenerative diseases. The prevalence and incidence of these diseases is on the rise with the increasing global population and average lifespan. Although many therapeutic approaches have been tested, there are currently no effective treatment routes for the prevention or cure of these diseases. We present the current status of our knowledge and understanding of the involvement of mitochondrial dysfunction in these diseases and highlight recent advances in novel therapeutic strategies targeting neuronal bioenergetics as potential approach for treating these diseases. 展开更多

<strong>Introduction:</strong> The involvement of the endothelial dysfunction (ED) in arterial hypotension (AHPT) post of spinal anesthesia (SA) has not been established. <strong>Objective: </stro... <strong>Introduction:</strong> The involvement of the endothelial dysfunction (ED) in arterial hypotension (AHPT) post of spinal anesthesia (SA) has not been established. <strong>Objective: </strong>To determine if there is a relationship between ED and AHPT observed during the 1st 15 minutes after SA. <strong>Population and Methods:</strong> The study concerned 40 people who were to benefit from a programmed herniorrhaphy under SA and 40 controls (SA<sup>-</sup>). The correlations between mean plasma concentrations of endothelin-1 (CmET-1, pg/ml) and Vascular Cell Adhesion Molecule-1 (CmVCAM-1, pg/ml) taken from SA<sup>+</sup> one hour before (H0), then 15 minutes after the SA (H15) and the mean arterial pression (AMP, mmHg) with heart rate (bpm) were analyzed (p < 5%). <strong>Results:</strong> CmET-1 and CmVCAM-1 of SA<sup>-</sup> were 1.07 and 3.34, respectively, compared to 1.12 (p = 0.735) and 3.57 (p = 0.862) in SA<sup>+</sup> at H0 and H15. In SA+, at H15, CmET-1 was 1.13 (p = 0.823) and CmVCAM-1 was 3.57 (p = 0.257). In SA<sup>-</sup>, a negative correlation existed between CmET-1 and CmVCAM-1 (r = -0.438;p = 0.005). Conversely, in SA<sup>+</sup>, whether at H0 (r = -0.31;p = 0.051) or at H15 (r = 0.024;p = 0.883), no correlation existed between CmET-1 and CmVCAM-1, nor between MAP and ED markers (r = 0.111;p = 0.325). <strong>Conclusion: </strong>These results show that there is no relationship between the AHPT which occurs during the first fifteen minutes post SA, CmVCAM-1 and CmET-1. Intraspinal inflammatory and immune responses are considered to play central roles in the pathological development of spinal cord injury.This study aimed to decipher the dynamics of systemic immune responses,initiated by spinal cord injury.The spinal cord in mice was completely transected at T8.Changes in the in vivo inflammatory response,between the acute and subacute stages,were observed.A rapid decrease in C-reactive protein levels,circulating leukocytes and lymphocytes,spleen-derived CD4~+interferon-γ+T-helper cells,and inflammatory cytokines,and a marked increase in neutrophils,monocytes,and CD4~+CD25~+FOXP3~+regulatory T-cells were observed during the acute phase.These systemic immune alterations were gradually restored to basal levels during the sub-acute phase.During the acute phase of spinal cord injury,systemic immune cells and factors showed significant inhibition;however,this inhibition was transient,and the indicators of these serious disorders gradually returned to baseline levels during the subacute phase.All experiments were performed in accordance with the institutional animal care guidelines,approved by the Institutional Animal Care and Use Committee of Experimental Animal Center of Drum Tower Hospital,China(approval No.2019 AE01040)on June 25,2019. 展开更多

目的观察腰痛宁胶囊联合脊柱微调整脊手法治疗椎间盘源性腰痛的临床疗效。方法60例椎间盘源性腰痛患者随机分为对照组和治疗组各30例。对照组单纯给予脊柱微调整脊手法治疗,每日1次;治疗组给予脊柱微调整脊手法联合腰痛宁胶囊口服,每次1... 目的观察腰痛宁胶囊联合脊柱微调整脊手法治疗椎间盘源性腰痛的临床疗效。方法60例椎间盘源性腰痛患者随机分为对照组和治疗组各30例。对照组单纯给予脊柱微调整脊手法治疗,每日1次;治疗组给予脊柱微调整脊手法联合腰痛宁胶囊口服,每次1.2g,每日1次,睡前30min服。分别于治疗前及治疗3周、6周时采用疼痛视觉模拟评分(VAS)和Oswestry功能障碍指数(ODI)对患者疼痛及功能障碍进行评价。结果两组患者治疗3周、6周时VAS及ODI评分均较本组治疗前明显降低(P<0.05),且治疗6周时均较治疗3周时降低更明显(P<0.05);治疗组治疗3周、6周时VAS及ODI评分明显低于同时间对照组(P<0.05)。结论腰痛宁胶囊联合脊柱微调整脊手法在改善椎间盘源性腰痛患者的疼痛及生活质量方面效果肯定,并且治疗6周效果更好。 展开更多

Astrocytes are integral components of the central nervous system,where they are involved in numerous functions critical for neuronal development and functioning,including maintenance of blood-brain barrier,formation of synapses,supporting neurons with nutrients and trophic factors,and protecting them from injury.These roles are markedly affected in the course of chronic neurodegenerative disorders,often before the onset of the disease.In this review,we summarize the recent findings supporting the hypothesis that astrocytes play a fundamental role in the processes contributing to neurodegeneration.We focus onα-synucleinopathies and tauopathies as the most common neurodegenerative diseases.The mechanisms implicated in the development and progression of these disorders appear not to be exclusively neuronal,but are often related to the astrocytic-neuronal integrity and the response of astrocytes to the altered microglial function.A profound understanding of the multifaceted functions of astrocytes and identification of their communication pathways with neurons and microglia in health and in the disease is of critical significance for the development of novel mechanism-based therapies against neurodegenerative disorders. 展开更多

Objective:To investigate the effects of Borassus flabellifer L.extracts on antioxidant activity,maintenance of cellular redox,and mitochondrial function in cisplatin-induced kidney injury.Methods:The extracts of Borassus flabellifer were obtained from crude male flowers using ethyl acetate and methanol.The antioxidant potential was evaluated by 2,2-azino-bis-(3-ethylbenzothiazoline-6-sulfonic acid),2,2-diphenyl-1-picrylhydrazyl,and ferric reducing antioxidant power,and total phenolic content was also determined.Cytoprotective activity of ethyl acetate and methanolic extracts was assessed after kidney cells were treated with cisplatin.Oxidative stress was determined by glutathione(GSH)assay,and formation of reactive oxygen species(ROS)and changes in mitochondrial transmembrane potential(ΔΨm)using 2’,7’-dichlorofluorescin diacetate and JC-10 assays,respectively.Results:Borassus flabellifer methanolic extract exhibited greater antioxidant activity than the ethyl acetate extract.Cytoprotective effect was demonstrated in both extracts,particularly in the ethyl acetate extract.The extracts showed protection against the cytotoxic effect of cisplatin by prevention of the increased GSSG and declined GSH/GSSG ratio.Both extracts also prevented the increase in ROS formation,and loss ofΔΨm.Conclusions:Both Borassus flabellifer extracts show antioxidant activity and cytoprotective effect against cisplatin-induced cytotoxicity of NRK-52 E cells by preventing oxidative stress and maintenance of GSH redox status.Borassus flabellifer extracts may possess beneficial effects on the prevention of oxidative stressinduced cell injury. 展开更多

BACKGROUND Functional constipation(FC)is a common functional gastrointestinal disease with various clinical manifestations.It is a physical and mental disease,which seriously affects patient physical and mental health and quality of life.Biofeedback therapy is the treatment of choice for FC,especially outlet obstructive constipation caused by pelvic floor dysfunction.High-quality nursing is a new nursing model in modern clinical work and a new concept of modern nursing service.AIM To explore the effect of biofeedback combined with high-quality nursing in the treatment of FC.METHODS A total of 100 patients with FC admitted to our hospital from March 2015 to July 2019 were selected for clinical observation.These patients were randomly divided into two groups of 50:Experimental group(biofeedback combined with highquality nursing treatment group)and control group(biofeedback group).RESULTS The constipation symptom score of the experimental group was significantly lower than that of the control group,and the difference was statistically significant(P<0.05).The anal canal resting pressure and initial defecation threshold of the experimental group were significantly lower than those of the control group,and the maximum squeeze systolic pressure of the anal canal of the experimental group was significantly higher than that of the control group(P<0.05).The Self-Rating Anxiety Scale and Zung’s Self-Rating Depression Scale scores of the two groups were significantly lower than before treatment.The Self-Rating Anxiety Scale and Self-Rating Depression Scale scores of the experimental group were significantly lower than those of the control group(P<0.05).The patient satisfaction score of the experimental group was significantly higher than that of the control group(P<0.05).CONCLUSION The application of biofeedback combined with high-quality nursing in the treatment of FC has significant advantages over pure biofeedback treatment,and it is worthy of promotion in clinical work. 展开更多

Cerebrovascular endothelial dysfunction is involved in the progression of leukoaraiosis. Asymmetric dimethylarginine is a competitive inhibitor of nitric oxide, which is highly expressed in patients with leukoaraiosis. Dimethylarginine dimethylaminohydrolase(DDAH) is a hydrolytic enzyme that is primarily responsible for eliminating asymmetric dimethylarginine, and it plays a role in the pathogenesis of cardiovascular and cerebrovascular diseases. The DDAH2 subtype is expressed in organs rich in induced nitric oxide synthase, including the heart, the placenta, and the cerebral endothelium during cerebral ischemia, in the stress state, or under neurotoxicity. Overexpression of the DDAH2 gene can inhibit asymmetric dimethylarginine-induced peripheral circulating endothelial cell dysfunction. However, it is unknown whether this polymorphism regulates plasma asymmetric dimethylarginine levels in patients with leukoaraiosis. In this doubleblind study, we recruited 46 patients with leukoaraiosis and 46 healthy, matched controls. Plasma asymmetric dimethylarginine levels were determined using enzyme-linked immunoassays. Genomic DNA was isolated from whole blood samples, and polymerase chain reaction, Sma I restriction enzyme digestion, restriction fragment length polymorphisms, and agarose electrophoresis were used to detect DDAH2(-449 G/C) gene polymorphisms. The results revealed that 95.65% of leukoaraiosis patients had recessive genetic models(GG and CG), while 89.13% of healthy control subjects had dominant genetic models(CC and CG). There was a significant difference in the genotype composition ratio between leukoaraiosis patients and healthy controls(P = 0.0002). The frequency of G alleles in the leukoaraiosis patients(71.74%) was significantly higher than in healthy controls, whereas the frequency of C alleles was lower(χ~2 = 13.9580, P = 0.0002). 背景:脊柱畸形矫形手术难点在于如何确定畸形椎体椎弓根位置,应用传统CT、X射线片检查等方法难以实现准确定位,影响手术效果。而近期研究显示,计算机技术、3D打印模板在提高定位及置钉效果方面存在优势。目的:探讨计算机设计联合3D打印... 背景:脊柱畸形矫形手术难点在于如何确定畸形椎体椎弓根位置,应用传统CT、X射线片检查等方法难以实现准确定位,影响手术效果。而近期研究显示,计算机技术、3D打印模板在提高定位及置钉效果方面存在优势。目的:探讨计算机设计联合3D打印模板应用于脊柱矫形对手术精度与矫正效果的影响。方法:选择2016年2月至2019年2月在承德医学院附属医院行矫形手术治疗的脊柱畸形患者73例进行前瞻性单盲随机对照试验,按随机数字表分为2组。试验组36例矫形术中应用计算机设计联合3D打印模板,对照组37例采取常规矫形手术。比较两组手术情况、Cobb角纠正率、置钉成功率、并发症发生率;术后随访12个月,对比两组术前、术后12个月Oswestry功能障碍指数。结果与结论:(1)试验组手术时间较对照组短,术中X射线透视次数、术中出血量较对照组少(P<0.05);(2)试验组Cobb角纠正率89%显著高于对照组70%(P<0.05),试验组置钉成功率100%显著高于对照组81%(P<0.05);(3)两组术后并发症发生率相比,差异无显著性意义(P>0.05);(4)两组术后12个月Oswestry功能障碍指数量表中社交活动、睡眠质量、日常活动、疼痛程度各维度评分较术前降低,且试验组低于对照组(P<0.05);(5)提示计算机设计联合3D打印模板应用于脊柱矫形手术可缩短手术时间,减轻手术创伤,减少术中X射线透视次数,提升手术精度与矫正效果,减轻术后功能障碍程度。 展开更多

BACKGROUND Endothelial dysfunction,a hallmark of diabetes,is a critical and initiating contributor to the pathogenesis of diabetic cardiovascular complications.However,the underlying mechanisms are still not fully understood.Ferroptosis is a newly defined regulated cell death driven by cellular metabolism and irondependent lipid peroxidation.Although the involvement of ferroptosis in disease pathogenesis has been shown in cancers and degenerative diseases,the participation of ferroptosis in the pathogenesis of diabetic endothelial dysfunction remains unclear.AIM To examine the role of ferroptosis in diabetes-induced endothelial dysfunction and the underlying mechanisms.METHODS Human umbilical vein endothelial cells(HUVECs)were treated with high glucose(HG),interleukin-1β(IL-1β),and ferroptosis inhibitor,and then the cell viability,reactive oxygen species(ROS),and ferroptosis-related marker protein were tested.To further determine whether the p53-xCT(the substrate-specific subunit of system Xc-)-glutathione(GSH)axis is involved in HG and IL-1βinduced ferroptosis,HUVECs were transiently transfected with p53 small interfering ribonucleic acid or NC small interfering ribonucleic acid and then treated with HG and IL-1β.Cell viability,ROS,and ferroptosis-related marker protein were then assessed.In addition,we detected the xCT and p53 expression in the aorta of db/db mice.RESULTS It was found that HG and IL-1βinduced ferroptosis in HUVECs,as evidenced by the protective effect of the ferroptosis inhibitors,Deferoxamine and ferrostatin-1,resulting in increased lipid ROS and decreased cell viability.Mechanistically,activation of the p53-xCT-GSH axis induced by HG and IL-1βenhanced ferroptosis in HUVECs.In addition,a decrease in xCT and the presence of deendothelialized areas were observed in the aortic endothelium of db/db mice.CONCLUSION Ferroptosis is involved in endothelial dysfunction and p53-xCT-GSH axis activation plays a crucial role in endothelial cell ferroptosis and endothelial dysfunction. 展开更多

目的应用扩散张量成像技术(diffusion tensor imaging,DTI)研究急性缺血性卒中机体功能障碍与锥体束损伤程度的相关性。材料与方法收集急性缺血性脑卒中患者在发病急性期内的常规MRI平扫、扩散加权成像、DTI检查、工作站自动生成各向异... 目的应用扩散张量成像技术(diffusion tensor imaging,DTI)研究急性缺血性卒中机体功能障碍与锥体束损伤程度的相关性。材料与方法收集急性缺血性脑卒中患者在发病急性期内的常规MRI平扫、扩散加权成像、DTI检查、工作站自动生成各向异性指数(fractional anisotropy,FA)图及ADC图,从中筛选出病变累及单侧基底节区的急性缺血性脑卒中患者16例,分别测量病灶区与大脑对侧相应部位的FA值、ADC值、纤维束数量,并计算出纤维束数量健侧与患侧差值,使用mimics软件对病灶体积进行测量,同时使用美国国立卫生研究院卒中量表(NIH Stroke Scale,NIHSS)、临床医学肌力分级对机体功能障碍程度进行评价。使用统计学方法研究健、患侧纤维束数量、FA值、ADC值的差异;统计出数量差值与病灶体积、NIHSS评分、肌力评分的相关性;统计16例患者出现的所有神经系统阳性表现。通过扩散张量纤维束成像(diffusion tensor tractography,DTT)和3D slicer软件在病变区和健侧对应部位重建,对比观察纤维束的形态变化。结果健、患侧锥体束数量、FA和ADC配对t检验有统计学差异(P<0.05,分别为0.001、0.005、0.001)。纤维束数量差值和病灶体积、肌力分级有显著相关关系(r=0.632,P=0.009;r=-0.554,P=0.026);病灶体积和NIHSS评分有显著正相关关系,差异及相关性有统计学意义(r=0.789,P=0.000)。健、患侧纤维束数量差值和NIHSS评分无相关性(P=0.061),FA值减少程度和ADC减少程度无相关性(P=0.890)。患侧纤维束走形异于健侧,部分患侧纤维束受压、变形、移位、断裂,部分健侧纤维束向患侧延伸。结论急性缺血性脑卒中患者机体功能障碍与锥体束损伤程度有相关性,病灶体积越大,锥体束损伤程度越严重,脑组织损伤程度越重,机体功能障碍程度越明显。 展开更多

Previous studies demonstrate an accumulation of transferrin and transferrin receptor 1(TfR1) in regenerating peripheral nerves.However, the expression and function of transferrin and TfR1 in the denervated skeletal muscle remain poorly understood.In this study, a mouse model of denervation was produced by complete tear of the left brachial plexus nerve.RNA-sequencing revealed that transferrin expression in the denervated skeletal muscle was upregulated, while TfR1 expression was downregulated.We also investigated the function of TfR1 during development and in adult skeletal muscles in mice with inducible deletion or loss of TfR1.The ablation of TfR1 in skeletal muscle in early development caused severe muscular atrophy and early death.In comparison, deletion of TfR1 in adult skeletal muscles did not affect survival or glucose metabolism, but caused skeletal muscle atrophy and motor functional impairment, similar to the muscular atrophy phenotype observed after denervation.These findings suggest that TfR1 plays an important role in muscle development and denervation-induced muscular atrophy.This study was approved by the Institutional Animal Care and Use Committee of Beijing Institute of Basic Medical Sciences, China(approval No.SYXK 2017-C023) on June 1, 2018. 展开更多

Objective:To evaluate the potential therapeutic effect of Sang-Yod rice bran hydrolysates(SRH)and in combination with lisinopril against hypertension,endothelial dysfunction,vascular remodeling,and oxidative stress in rats with nitric oxide deficiency-induced hypertension.Methods:Hypertension was induced in male Sprague-Dawley rats by administration of a nitric oxide synthase inhibitor,Nω-nitro-L-arginine methyl ester(L-NAME)in drinking water for 6 weeks.Hypertensive rats were administered daily with SRH(500 mg/kg/day),lisinopril(1 mg/kg/day),or the combination of SRH and lisinopril by gastric lavage for the last 3 weeks of L-NAME treatment.Hemodynamic status,vascular reactivity to vasoactive agents,and vascular remodeling were assessed.Blood and aortic tissues were collected for measurements of oxidative stress markers,plasma angiotensin-converting enzyme(ACE)activity,plasma angiotensinⅡ,and protein expression.Results:L-NAME induced remarkable hypertension and severe oxidative stress,and altered contents of smooth muscle cells,elastin,and collagen of the aortic wall.SRH or lisinopril alone reduced blood pressure,restored endothelial function,decreased plasma ACEs and angiotensinⅡlevels,alleviated oxidant markers and glutathione redox status,and restored the vascular structure.The effects were associated with increased expression of endothelial nitric oxide synthase and decreased expression of gp91phox and AT1R expression.The combination of SRH and lisinopril was more effective than monotherapy.Conclusions:SRH alone or in combination with lisinopril exert an antihypertensive effect and improve endothelial function and vascular remodeling through reducing oxidative stress and suppressing elevated renin-angiotensin system. 展开更多

OBJECTIVE In patients undergoing cardiac surgery,reduced preoperative ejection fraction(EF)and senior age are associated with a worse outcome.As most outcome data available for these patients are mainly from Western surgical populations involving specific surgery types,our aim is to evaluate the real-world characteristics and perioperative outcomes of surgery in senior-aged heart failure patients with reduced EF across a broad range cardiac surgeries.METHODS Data were obtained from the China Heart Failure Surgery Registry(China-HFSR)database,a nationwide multicenter registry study in mainland China.Multiple variable regression analysis was performed in patients over 75 years old to identify risk factors associated with mortality.RESULTS From 2012 to 2017,578 senior-aged(>75 years)patients were enrolled in China HFSR,21.1%of whom were female.Isolated coronary bypass grafting(CABG)were performed in 71.6%of patients,10.1%of patients underwent isolated valve surgery and 8.7%received CABG combined with valve surgery.In-hospital mortality was 10.6%,and the major complication rate was 17.3%.Multivariate analysis identified diabetes mellitus(odds ratio(OR)=1.985),increased creatinine(OR=1.007),New York Heart Association(NYHA)Class III(OR=1.408),NYHA class IV(OR=1.955),cardiogenic shock(OR,6.271),and preoperative intra-aortic balloon pump insertion(OR=3.426)as independent predictors of in-hospital mortality.CONCLUSIONS In senior-aged patients,preoperative evaluation should be carefully performed,and strict management of reversible factors needs more attention.Senior-aged patients commonly have a more severe disease status combined with more frequent comorbidities,which may lead to a high risk in mortality. 展开更多

OBJECTIVE To assess the role of beta-blockers(BB)in patients with chronic kidney disease(CKD)aged≥75 years.METHODS AND RESULTS From January 2008 to July 2014,we included 390 consecutive patients≥75 years of age with ejection fraction≤35%and glomerular filtration rate(GFR)≤60 m L/min per 1.73 m^2.We analyzed the relationship between treatment with BB and mortality or cardiovascular events.The mean age of our population was 82.6±4.1 years.Mean ejection fraction was 27.9%±6.5%.GFR was 60-45 m L/min per 1.73 m^2 in 50.3%of patients,45-30 m L/min per 1.73 m^2 in 37.4%,and<30 m L/min per 1.73 m^2 in 12.3%.At the conclusion of follow-up,67.4%of patients were receiving BB.The median follow-up was28.04(IR:19.41-36.67)months.During the study period,211 patients(54.1%)died and 257(65.9%)had a major cardiovascular event(death or hospitalization for heart failure).BB use was significantly associated with a reduced risk of death(HR=0.51,95%CI:0.35-0.74;P<0.001).Patients receiving BB consistently showed a reduced risk of death across the different stages of CKD:stage IIIa(GFR=30-45 m L/min per 1.73 m^2;HR=0.47,95%CI:0.26-0.86,P<0.0001),stage IIIb(GFR 30-45 m L/min per 1.73 m^2;HR=0.55,95%CI:0.26-1.06,P=0.007),and stages IV and V(GFR<30 m L/min per 1.73 m~2;HR=0.29,95%CI:0.11-0.76;P=0.047).CONCLUSIONS The use of BB in elderly patients with HFr EF and renal impairment was associated with a better prognosis.Use of BB should be encouraged when possible. 展开更多
