As coal-fired power industry consumes around a half of China’s coal production, it is always thought to be a big air pollutant emission source. As more and more strict legislations in coal-fired power industry have been issued by the government, the emission performance in coal-fired power industry has been drastically reduced recently.Based on a brief review of the development of emission control in China’s coal-fired power industry, the affecting mechanism among the development of installed capacities of emission control device, pollutant emission, and emission performances in coal-fired power industry is studied. According to a systematic study on the development of emissions of classified categories, the role of coal-fired power industry as a pollutant source is reevaluated.It is found that, coal- fired power industry has contributed the most to China5S emission reduction, and the barycenter of air pollutant emission has been transformed to other high energy consumption industries,like heat, iron/steel, and cement.Then some development strategies are suggested, such as maintaining the current emission standard in coal-fired power industry;expending the coal-fired power emission standards to categories of heat generation and supply, nonmetallic mineral production and ferrous metals smelt-ing and processing;and controlling other heavy metal by consulting the method of Hg control. 展开更多

Purpose: After damage control surgery, trauma patients are transferred to intensive care units to restore the physiology. During this period, massive transfusion might be required for ongoing bleeding and coagulopathy. This research aimed to identify predictors of massive blood transfusion in the surgical intensive care units (SICUs). Methods: This is an analysis of the THAI-SICU study which was a prospective cohort that was done in the 9-university-based SICUs in Thailand. The study included only patients admitted due to trauma mechanisms. Massive transfusion was defined as received >10 units of packed red blood cells on the first day of admission. Patient characteristics and physiologic data were analyzed to identify the potential factors. A multivariable regression was then performed to identify the significant model. Results: Three hundred and seventy patients were enrolled. Sixteen patients (5%) received massive transfusion in the SICUs. The factors that significantly predicted massive transfusion were an initial sequential organ failure assessment (SOFA)>9 (risk difference (RD) 0.13, 95% confidence interval (CI): 0.03—0.22, p = 0.01);intra-operative blood loss > 4900 mL (RD 0.33, 95% CI: 0.04—0.62, p = 0.02) and intra-operative blood transfusion > 10 units (RD 0.45, 95% Cl: 0.06 to 0.84, p = 0.02). The probability to have massive transfusion was 0.976 in patients who had these 3 factors. Conclusion: Massive blood transfusion in the SICUs occurred in 5%. An initial SOFA >9, intra-operative blood loss >4900 mL, and intra-operative blood transfusion >10 units were the significant factors to predict massive transfusion in the SICUs. 展开更多

Delirium is the most frequent manifestation of acute brain dysfunction in intensive care unit(ICU).Although antipsychotics are widely used to treat this serious complication,recent evidence has emphasized that these agents did not reduce ICU delirium(ICU-D)prevalence and did not improve survival,length of ICU or hospital stay after its occurrence.Of note,no pharmacological strategy to prevent or treat delirium has been identified,so far.In this scenario,new scientific evidences are urgently needed.Investigations on specific ICU-D subgroups,or focused on different clinical settings,and studies on medications other than antipsychotics,such as dexmedetomidine or melatonin,may represent interesting fields of research.In the meantime,because there is some evidence that ICU-D can be effectively prevented,the literature suggests strengthening all the strategies aimed at prevention through no-pharmacological approaches mostly focused on the correction of risk factors.The more appropriate strategy useful to treat established delirium remains the use of antipsychotics managed by choosing the right doses after a careful case-by-case analysis.While the evidence regarding the use of dexmedetomidine is still conflicting and sparse,this drug offers interesting perspectives for both ICU-D prevention and treatment.This paper aims to provide an overview of current pharmacological approaches of evidence-based medicine practice.The state of the art of the on-going clinical research on the topic and perspectives for future research are also addressed. 展开更多

Implementation of a nurse-led protocol for early extubation after cardiac surgery:A pilot study 预览

BACKGROUND Protocols for nurse-led extubation are as safe as a physician-guided weaning in general intensive care unit(ICU).Early extubation is a cornerstone of fast-track cardiac surgery,and it has been mainly implemented in post-anaesthesia care units.Introducing a nurse-led extubation protocol may lead to reduced extubation time.AIM To investigate results of the implementation of a nurse-led protocol for early extubation after elective cardiac surgery,aiming at higher extubation rates by the third postoperative hour.METHODS A single centre prospective study in an 18-bed,consultant-led Cardiothoracic ICU,with a 1:1 nurse-to-patient ratio.During a 3-wk period,the protocol was implemented with:(1)Structured teaching sessions at nurse handover and at bed-space(all staff received teaching,over 90%were exposed at least twice;(2)Email;and(3)Laminated sheets at bed-space.We compared“standard practice”and“intervention”periods before and after the protocol implementation,measuring extubation rates at several time-points from the third until the 24th postoperative hour.RESULTS Of 122 cardiac surgery patients admitted to ICU,13 were excluded as early weaning was considered unsafe.Therefore,109 patients were included,54 in the standard and 55 in the intervention period.Types of surgical interventions and baseline left ventricular function were similar between groups.From the third to the 12th post-operative hour,the intervention group displayed a higher proportion of patients extubated compared to the standard group.However,results were significant only at the sixth hour(58%vs 37%,P=0.04),and not different at the third hour(13%vs 6%,P=0.33).From the 12th post-operative hour time-point onward,extubation rates became almost identical between groups(83%in standard vs 83%in intervention period).CONCLUSION The implementation of a nurse-led protocol for early extubation after cardiac surgery in ICU may gradually lead to higher rates of early extubation. 展开更多

There are various case reports ... Purpose: Fat embolism syndrome (FES) is systemic manifestation of fat emboli in the circulation seen mostly after long bone fractures. FES is considered a lethal complication of trauma. There are various case reports and series describing FES. Here we describe the clinical characteristics, management in ICU and outcome of these patients in level I trauma center in a span of 6 months. Methods: In this prospective study, analysis of all the patients with FES admitted in our polytrauma intensive care unit (ICU) of level I trauma center over a period of 6 months (from August 2017 to January 2018) was done. Demographic data, clinical features, management in ICU and outcome were analyzed. Results: We admitted 10 cases of FES. The mean age of patients was 31.2 years. The mean duration from time of injury to onset of symptoms was 56 h. All patients presented with hypoxemia and petechiae but central nervous system symptoms were present in 70% of patients. The mean duration of mechanical ventilation was 11.7 days and the mean length of ICU stay was 14.7 days. There was excellent recovery among patients with no neurological deficit. Conclusion: FES is considered a lethal complication of trauma but timely management can result in favorable outcome. FES can occur even after fixation of the fracture. Hypoxia is the most common and earliest feature of FES followed by CNS manifestations. Any patient presenting with such symptoms should raise the suspicion of FES and mandate early ICU referral. 展开更多

A transition from a large open pit into a novel“macroblock variant”block caving geometry at Chuquicamata mine,Codelco Chile 预览

With a history of more than 100 years, Chuquicamata has become one of the largest open pits in the world. Its current depth of 1100 m, length of 4500 m and width of 3000 m, resulting in haulage distances of over 11,000 m from pit bottom to the processing plant or waste dumps, with waste/ore ratio greater than 4 and much lower grades, are effectively determining its profitable limit by the end of this decade. Currently, there are over 4300 Mt of geological resources containing 0.7% of copper and 340 ppm (1 ppm = 1 g/t) of molybdenum below the final pit shell. The corresponding orebody geometry is 3000 m northsouth, 300-800 m eastwest and the drilling campaign suggested the existence of more than 900 m of mineable ore from the final pit bottom. This is in spite of the fact that the Chuquicamata orebody is still open at depth. The business case to continue with the mining of the Chuquicamata orebody is to change from the current large open pit to a novel macroblock variant block caving geometry. This requires a production rate of 140,000 t/d (50 Mt per annum), with a continuous material handling system to achieve the 7-year ramp-up to full production. The resolution of a number of known technical, operational and logistical challenges outside current industry practises which Chuquicamata will face over its projected 40-year mine life is therefore essential. These challenges include simultaneous operations of open pit and underground macroblocks and from the initial lift to the second lift, rapid ramp-up and optimum sequencing of the planned macroblocks, early dilution from the west fault and late dilution from potential slope failures, and the potential for major operational hazards. An equally important issue to be addressed is cultural given that the mining history in the northern region of Chile, which includes Chuquicamata, is predominantly open pit. Urban land intensive use is an important indicator in harmonizing the relationship between land supply and demand. The system dynamics(SD) can be used to construct the feedback loop between urban construction land supply and demand and index variable function. Based on this, this study built a supply and demand system dynamic model of urban construction land for Chang-Zhu-Tan urban agglomeration. This model can simulate the change trends of supply and demand of construction land, industrial land, and residential land in 2016–2030 by three scenarios of low, medium, and high intensity modes. The results showed that the scale of construction land of urban agglomeration is expanding, with a rapid increase rate for the urban construction land. The scale and speed of land use based on the three intensity modes existed differences. The large scale and supply of construction land in the low intensity mode caused easily the waste of land resources. In high intensity mode, the scale and supply of construction land were reduced against the healthy development of new-type urbanization. In the medium intensity mode, the scale and supply of land use adapted to the socio-economic development and at the same time reflected the concept of modern urban development. In addition, the results of this study found that the proportion of industrial land in construction land ranged from 15% to 21%, which increased year by year in the low intensity mode, and decreased slowly and stabilized in medium and high intensity modes. The proportion of residential land in construction land ranged from 27% to 35%, which decreased in the low and the medium intensity modes, and maintained a high level in the higher intensity mode. This study contributes to provide scientific reference for decision-making optimization of land supply and demand, urban planning, and land supply-side reform. 展开更多

While in recovery unit child acutely deteriorated with tachycardia, oxygen desaturation, and hypotension. Background: Blood purification (BP) is one of the most important rescue measures for patients with critical illness in the intensive care unit (ICU), espedally for those with acute kidney injury. The purpose of this nationwide survey was to reveal the real world of current BP practice in different ICUs all over China. This study was designed to be a multi-center cross-sectional study. Methods: All adult patients (over 18 years of age), who were admitted to ICU and required BP in 35 sub-centers across China were included during 30-day survey period in 2018. Demographic characteristics and clinical data were recorded including the timing of treatment initiation, indications, modality, relative contraindication, establishment of vascular access, selection of filter/membrane, settings, anti-coagulation, executive department, complication, intake, and output. Discussion: This nationwide survey may contribute to reveal the real world of current BP practice in different ICUs all over China. 展开更多
