Development of non-invasive methods to risk-stratify patients and predict clinical endpoints have been identified as one of the key research priorities in primary sclerosing cholangitis(PSC).In addition to serum and histological biomarkers,there has been much recent interest in developing imaging biomarkers that can predict disease course and clinical outcomes in PSC.Magnetic resonance imaging/magnetic resonance cholangiopancreatography(MRI/MRCP)continue to play a central role in the diagnosis and follow-up of PSC patients.Magnetic resonance(MR)techniques have undergone significant advancement over the last three decades both in MR data acquisition and interpretation.The progression from a qualitative to quantitative approach in MR acquisition techniques and data interpretation,offers the opportunity for the development of objective and reproducible imaging biomarkers that can potentially be incorporated as an additional endpoint in clinical trials.This review article will discuss how the role of MR techniques have evolved over the last three decades from emerging as an alternative diagnostic tool to endoscopic retrograde cholangiopancreatography,to being instrumental in the ongoing search for imaging biomarker of disease stage,progression and prognosis in PSC. 展开更多

BACKGROUND It is of vital importance to find radiologic biomarkers that can accurately predict treatment response. Usually, the initiation of antiangiogenic therapy causes a rapid decrease in the contrast enhancing tumor. However, the treatment response is observed only in a fraction of patients due to the partial radiological response secondary to stabilization of abnormal vessels which does not essentially indicate a true antitumor effect. Perfusion-weighted magnetic resonance imaging (PWMRI) techniques have shown implicitness as a strong imaging biomarker for gliomas since they give hemodynamic information of blood vessels. Hence, there is a rapid expansion of PW-MRI related studies and clinical applications. AIM To determine the diagnostic performance of PW-MRI techniques including:(A) dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI);and (B) dynamic susceptibility contrast magnetic resonance imaging (DSC-MRI) for evaluating response to antiangiogenic therapy in patients with recurrent gliomas. METHODS Databases such as PubMed (MEDLINE included), EMBASE, and Google Scholar were searched for relevant original articles. The included studies were assessed for methodological quality with the Quality Assessment of Diagnostic Accuracy Studies 2 tool. Medical imaging follow-up or histopathological analysis was used as the reference standard. The data were extracted by two reviewers independently, and then the sensitivity, specificity, summary receiver operating characteristic curve, area under the curve (AUC), and heterogeneity were calculated using Meta-Disc 1.4 software.RESULTS This study analyzed a total of six articles. The overall sensitivity for DCE-MRI and DSC-MRI was 0.69 [95% confidence interval (CI): 0.53-0.82], and the specificity was 0.99 (95%CI: 0.93-1) by a random effects model (DerSimonianee- Laird model). BACKGROUND Proton magnetic resonance spectroscopy(1H MRS)is a technique widely used for investigating metabolites in humans.Lipids are stored outside the muscle cell are called extramyocellular lipids(EMCL),and lipids stored on the inside of muscle cells are called intramyocellular lipids(IMCL).The relationship between metabolic syndrome and IMCL has been extensively studied.AIM To determine the effects of muscle fiber orientations on muscle metabolites using 1H MRS.METHODS Chicken muscles were used as the subject in this study.MRS spectra were performed on a 1.5T Magnetic resonance imaging machine(1.5 Tesla Philips Achieva).A single voxel(8 mm×8 mm×20 mm)was placed on the chicken extensor iliotibialis lateralis muscle with the muscle fiber oriented at 0°,30°,60°,and 90°to the main magnetic field.1H MRS spectra were acquired using a pointresolved spectroscopy,TR=2000 ms,TE=30 ms,and NSA=256.Metabolites of interest from each orientation to the main magnetic field were compared using Wilcoxon signed-rank test.Differences less than 0.05 were considered to be statistically significant with 95%CI.RESULTS The metabolite profiles were different for each orientation of muscle fibers to the main magnetic field.The orientation at 90°was the most different compared to other orientations.The quantity of IMCL and EMCL exhibited statistically significantly changes with impacts at 30°,60°,and 90°when compared with muscles aligned at 0°to the main magnetic field.Statistical analysis showed statistically significant IMCL(CH3),EMCL(CH3),and IMCL(CH2)at 30°,60°,and 90°(P=0.017,0.018,and 0.018,respectively)and EMCL(CH2)at 30°and 60°(P=0.017 and 0.042,respectively).EMCL(CH2)at 90°was unable to be measured in this study.The muscle lipids quantified at 30°,60°,and 90°tended to be lower when compared to 0°.CONCLUSION Careful positioning is one of the most important factors to consider when studying 1H MRS metabolites in muscles to ensure reproducibility and uniformity of muscle metabolite spectra. 展开更多

目的探讨MRI和CT对胫骨平台隐匿性骨折的诊断效能。方法收集2017年1月至2018年4月本院100例胫骨平台隐匿性骨折疑诊患者为研究对象,所有患者均行CT和MRI检查,对比分析CT和及MRI对胫骨平台隐匿性骨折的诊断效能。结果以临床最终诊断作为... 目的探讨MRI和CT对胫骨平台隐匿性骨折的诊断效能。方法收集2017年1月至2018年4月本院100例胫骨平台隐匿性骨折疑诊患者为研究对象,所有患者均行CT和MRI检查,对比分析CT和及MRI对胫骨平台隐匿性骨折的诊断效能。结果以临床最终诊断作为金标准,MRI诊断胫骨平台隐匿性骨折的灵敏度为97.53%、特异度为100.00%、准确度为98.00%,CT诊断胫骨平台隐匿性骨折的灵敏度为76.54%、特异度为100.00%、准确度为81.00%,MRI诊断的灵敏度和准确度均显著高于CT(P均<0.05)。CT诊断骨皮质骨折的灵敏度、特异度、准确度均为100.00%;MRI诊断骨皮质骨折的灵敏度、特异度、准确度分别为81.82%、70.59%、87.34%;CT诊断骨皮质骨折的准确度显著高于MRI(P<0.05)。结论MRI诊断胫骨平台隐匿性骨折的准确度高于CT,但CT诊断骨皮质骨折更具优势;对于疑诊胫骨平台隐匿性骨折,MRI可作为首选筛查方法,联用CT有助于提高诊断效能。 展开更多

In the field of medical imaging,the traditional local binary pattern (LBP)and its improved algorithms are often sensitive to noise.Traditional LBPs are solely based on the signal information from local differences,and the binary quantization method oversimplifies the local texture features while disregarding the imaging information from the concaveconvex regions between the high-order pixels and the neighboring sampling points.Therefore,we propose an improved Derived Mean Complete Local Binary Pattern (DM_CLBP) algorithm based on high-order derivatives.In the DM_CLBP method,the grey value of a single pixel is replaced by the mean grey value of the rectangular area block,and the difference between pixel values in the area is obtained using the second-order differentiation method.Based on the calculation concept of the complete local binary pattern (CLBP)algorithm,the cascade signs and magnitudes of the two components are encoded and recombined in DM_CLBP using a uniform pattern.The results from the experiments showed that the proposed DM_CLBP descriptors achieved a classification accuracy of 94.4%.Compared with LBP and other improved algorithms,the DM_CLBP algorithm presented in this study can effectively differentiate between lesion areas and normal areas in thyroid MR,(magnetic resonance)images and shows the improved accuracy of area classification. 展开更多

BACKGROUND Gerstmann-Str?ussler-Scheinker(GSS)disease is an inherited prion disease that is clinically characterized by the early onset of progressive cerebellar ataxia.The incidence of GSS is extremely low and it is particularly rare in China.Therefore,clinicians may easily confuse this disease with other diseases that also cause ataxia,resulting in its under-diagnosis or misdiagnosis.CASE SUMMARY Here,we report the first case of genetically diagnosed GSS disease in Northeast China.The patient exhibited typical ataxia and dysarthria 2.5 years after symptom onset.However,magnetic resonance imaging of the brain and spinal cord revealed a normal anatomy.Screening results for the spinocerebellar ataxia gene were also negative.We thus proposed to expand the scope of genetic screening to include over 200 mutations that can cause ataxia.A final diagnosis of GSS was presented and the patient was followed for more than 3.5 years,during which we noted imaging abnormalities.The patient gradually exhibited decorticate posturing and convulsions.We recommended administration of oral sodium valproate,which resolved the convulsions.CONCLUSION Patients with inherited ataxia should be considered for a diagnosis of GSS via genetic testing at an early disease stage. 展开更多

BACKGROUND The Liver Imaging Reporting and Data System(LI-RADS),supported by the American College of Radiology(ACR),has been developed for standardizing the acquisition,interpretation,reporting,and data collection of liver imaging examinations in patients at risk for hepatocellular carcinoma(HCC).Diffusionweighted imaging(DWI),which is described as an ancillary imaging feature of LI-RADS,can improve the diagnostic efficiency of LI-RADS v2017 with gadoxetic acid-enhanced magnetic resonance imaging(MRI)for HCC.AIM To determine whether the use of DWI can improve the diagnostic efficiency of LIRADS v2017 with gadoxetic acid-enhanced magnetic resonance MRI for HCC.METHODS In this institutional review board-approved study,245 observations of high risk of HCC were retrospectively acquired from 203 patients who underwent gadoxetic acid-enhanced MRI from October 2013 to April 2018.Two readers independently measured the maximum diameter and recorded the presence of each lesion and assigned scores according to LI-RADS v2017.The test was used to determine the agreement between the two readers with or without DWI.In addition,the sensitivity(SE),specificity(SP),accuracy(AC),positive predictive value(PPV),and negative predictive value(NPV)of LI-RADS were calculated.Youden index values were used to compare the diagnostic performance of LIRADS with or without DWI.RESULTS Almost perfect interobserver agreement was obtained for the categorization of observations with LI-RADS(kappa value:0.813 without DWI and 0.882 with DWI).For LR-5,the diagnostic SE,SP,and AC values were 61.2%,92.5%,and 71.4%,respectively,with or without DWI;for LR-4/5,they were 73.9%,80%,and 75.9%without DWI and 87.9%,80%,and 85.3%with DWI;for LR-4/5/M,they were 75.8%,58.8%,and 70.2%without DWI and 87.9%,58.8%,and 78.4%with DWI;for LR-4/5/TIV,they were 75.8%,75%,and 75.5%without DWI and 89.7%,75%,and 84.9%with DWI.The Youden index values of the LI-RADS classification without or with DWI were as follows:LR-4/5:0.539 vs 0.679;LR-4/5/M:0.346 vs 0.467;and LR-4/5/TIV: 展开更多

When watching someone performs an action,mirror neurons are activated in a way that is very similar to the activation that occurs when actually performing that action.Previous single-sample case studies indicate that hand-action observation training may lead to activation and remodeling of mirror neuron systems,which include important language centers,and may improve language function in aphasia patients.In this randomized-block-design experiment,we recruited 24 aphasia patients from,Zhongda Hospital,Southeast University,China.The patients were divided into three groups where they underwent hand-action observation and repetition,dynamic-object observation and repetition,or conventional speech therapy.Training took place 5 days per week,35 minutes per day,for 2 weeks.We assessed language function via picture naming tests for objects and actions and the Western Aphasia Battery.Among the participants,one patient,his wife and four healthy student volunteers underwent functional magnetic resonance imaging to analyze changes in brain activation during hand-action observation and dynamic-object observation.Results demonstrated that,compared with dynamic-object observation,hand-action observation led to greater performance with respect to the aphasia quotient and affiliated naming sub-tests and a greater Western Aphasia Battery test score.The overall effect was similar to that of conventional aphasia training,yet hand-action observation had advantages compared with conventional training in terms of vocabulary extraction and spontaneous speech.Thus,hand-action observation appears to more strongly activate the mirror neuron system compared with dynamic-object observation.The activated areas included Broca’s area,Wernicke’s area,and the supramarginal gyrus.These results suggest that hand-action observation combined with repetition might better improve language function in aphasia patients compared with dynamic-object observation combined with repetition.The therapeutic mechanism of this intervention may be associated wi 展开更多

In recent years,it has come a long way in the diagnosis,treatment,and follow-up of prostate cancer.Beside this,it was argued that definitive treatments could cause overtreatment,particularly in the very low,low,and favorable risk group.When alternative treatment and follow-up methods are being considered for this group of patients,active surveillance is seen as a good alternative for patients with very low and low-risk groups in this era.However,it has become necessary to find other alternatives for patients in the favorable risk group or patients who cannot adopt active follow-up.In the light of technological developments,the concept of focal therapy was introduced with the intensification of research to treat only the lesioned area instead of treating the entire organ for prostate lesions though there are not many publications about many of them yet.According to the initial results,it was understood that the results could be good if the appropriate focal therapy technique was applied to the appropriate patient.Thus,focal therapies have begun to find their"middle ground"place between definitive therapies and active follow-up. 展开更多

目的:探讨针刺治疗面神经磁共振检查正常的周围性面瘫时,是否需要联合使用药物。方法:将48例面神经磁共振检查正常的周围性面瘫患者随机分为针药组和针刺组,每组24例。针药组采用针刺联合醋酸泼尼松片治疗,针刺组采用单纯针刺治疗,两组... 目的:探讨针刺治疗面神经磁共振检查正常的周围性面瘫时,是否需要联合使用药物。方法:将48例面神经磁共振检查正常的周围性面瘫患者随机分为针药组和针刺组,每组24例。针药组采用针刺联合醋酸泼尼松片治疗,针刺组采用单纯针刺治疗,两组针刺治疗的方法相同。针刺主要穴取患侧翳风、颧髎、太阳、四白、颊车、地仓、对侧合谷,每次30 min,隔日1次,急性期后患侧翳风与颧髎、颊车与地仓加用2对电针,直至治愈;口服醋酸泼尼松片每天1次,初始剂量30mg,每3天减量10mg,共用9d,治疗3、8周后用Portmann评分观察疗效。结果:针刺组治疗3、8周后治愈率分别为50.0%(12/24)、83.3%(20/24),均略低于针药组的58.3%(14/24)、87.5%(21/24),但两组间差异均无统计学意义(均P>0.05);针刺组治愈患者疗程为(31.2±17.0)d,略长于针药组的(29.5±12.8)d,但两组间差异无统计学意义(P>0.05)。结论:单纯针刺治疗面神经磁共振检查正常的周围性面瘫,能取得针刺联合药物治疗同等的疗效,避免了药物的相关不良反应。 展开更多

The ability to cancel a motor response is critical for optimal functioning in various facets of daily life.Hence,efficient inhibitory motor control is a key function throughout the lifespan.Considering the fact that inhibitory motor function gradually declines with advancing age,it is not surprising that the study of motor inhibition in this age group is gaining considerable interest.In general,we can distinguish between two prominent types of motor inhibition,namely proactive and reactive inhibition.Whereas the anticipation for upcoming stops(proactive inhibition)appears readily preserved at older age,the ability to stop an already planned or initiated action(reactive inhibition)generally declines with advancing age.The differential impact of aging on proactive and reactive inhibition at the behavioral level prompts questions about the neural architecture underlying both types of inhibitory motor control.Here we will not only highlight the underlying structural brain properties of proactive and reactive inhibitory control but we will also discuss recent developments in brain-behavioral approaches,namely the registration of neurochemical compounds using magnetic resonance spectroscopy.This technique allows for the direct detection of the primary inhibitory neurotransmitter in the brain,i.e.,γ-aminobutyric acid,across the broader cortical/subcortical territory,thereby opening new perspectives for better understanding the neural mechanisms mediating efficient inhibitory control in the context of healthy aging.Ultimately,these insights may contribute to the development of interventions specifically designed to counteract age-related declines in motor inhibition. 展开更多

BACKGROUND Mesenchymal stem cells (MSCs) have been widely tested for their therapeutic efficacy in the ischemic brain and have been shown to provide several benefits. A major obstacle to the clinical translation of these therapies has been the inability to noninvasively monitor the best route, cell doses, and collateral effects while ensuring the survival and effective biological functioning of the transplanted stem cells. Technological advances in multimodal imaging have allowed in vivo monitoring of the biodistribution and viability of transplanted stem cells due to a combination of imaging technologies associated with multimodal nanoparticles (MNPs) using new labels and covers to achieve low toxicity and longtime residence in cells. AIM To evaluate the sensitivity of triple-modal imaging of stem cells labeled with MNPs and applied in a stroke model. METHODS After the isolation and immunophenotypic characterization of human bone marrow MSCs (hBM-MSCs), our team carried out lentiviral transduction of these cells for the evaluation of bioluminescent images (BLIs) in vitro and in vivo. In addition, MNPs that were previously characterized (regarding hydrodynamic size, zeta potential, and optical properties), and were used to label these cells, analyze cell viability via the 3-[4,5-dimethylthiazol-2-yl]-2,5 diphenyl tetrazolium bromide assay and BLI analysis, and quantify the internalization process and iron load in different concentrations of MNPs via magnetic resonance imaging (MRI), near-infrared fluorescence (NIRF), and inductively coupled plasma-mass spectrometry (ICP-MS). In in vivo analyses, the same labeled cells were implanted in a sham group and a stroke group at different times and under different MNP concentrations (after 4 h or 6 d of cell implantation) to evaluate the sensitivity of triple-modal images. Wireless power transfer(WPT)to support mobile and portable devices is an emerging wireless technique.Among all kinds of approaches,magnetic resonance coupling(MRC)is an excellent one for mid-range WPT,which provides better mobility,flexibility,and convenience due to its simplicity in hardware implementation and longer transmission distances.In this paper,we consider an MRCWPT system with multiple power transmitters,one intended power receiver and multiple unintended power receivers.We investigate the probabilistic robust beamforming designs and provide efficient algorithms to achieve the local optimums under two different criteria,i.e.,total source power minimization problem and min-max unintended receiving power restriction problem.As the problems are quite typical in robust design situations,our proposed robust beamformers can be conveniently applied to other probabilistic robust design problems,thus reduce the complexity as well as improve the beamforming performance.Numerical results demonstrate that the proposed algorithms can significantly improve the performance as well as the robustness of the WPT system. 展开更多
