Objective response rate assessment in oncology:Current situation and future expectations 认领

The tumor objective response rate(ORR)is an important parameter to demonstrate the efficacy of a treatment in oncology.The ORR is valuable for clinical decision making in routine practice and a significant end-point for reporting the results of clinical trials.World Health Organization and Response Evaluation Criteria in Solid Tumors(RECIST)are anatomic response criteria developed mainly for cytotoxic chemotherapy.These criteria are based on the visual assessment of tumor size in morphological images provided by computed tomography(CT)or magnetic resonance imaging.Anatomic response criteria may not be optimal for biologic agents,some disease sites,and some regional therapies.Consequently,modifications of RECIST,Choi criteria and Morphologic response criteria were developed based on the concept of the evaluation of viable tumors.Despite its limitations,RECIST v1.1 is validated in prospective studies,is widely accepted by regulatory agencies and has recently shown good performance for targeted cancer agents.Finally,some alternatives of RECIST were developed as immune-specific response criteria for checkpoint inhibitors.Immune RECIST criteria are based essentially on defining true progressive disease after a confirmatory imaging.Some graphical methods may be useful to show longitudinal change in the tumor burden over time.Tumor tissue is a tridimensional heterogenous mass,and tumor shrinkage is not always symmetrical;thus,metabolic response assessments using positron emission tomography(PET)or PET/CT may reflect the viability of cancer cells or functional changes evolving after anticancer treatments.The metabolic response can show the benefit of a treatment earlier than anatomic shrinkage,possibly preventing delays in drug approval.Computer-assisted automated volumetric assessments,quantitative multimodality imaging in radiology,new tracers in nuclear medicine and finally artificial intelligence have great potential in future evaluations. 展开更多

BACKGROUND^177Lu peptide receptor radionuclide therapy(PRRT)is a recently approved therapy in Spain that has been demonstrated to be a well-tolerated therapy for positive somatostatin receptor advanced gastroenteropancreatic neuroendocrine tumors.AIM To determine the impact of PRRT on quality of life,radiologic and metabolic response,overall survival,prognostic factors and toxicity.METHODS Thirty-six patients treated with^177Lu-PRRT from 2016 to 2019 were included.The most frequent location of the primary tumor was the gastrointestinal tract(52.8%),pancreas(27.8%),and nongastropancreatic neuroendocrine tumor(11.1%).The liver was the most common site of metastasis(91.7%),followed by distant nodes(50.0%),bone(27.8%),peritoneum(25.0%)and lung(11.1%).Toxicity was evaluated after the administration of each dose.Treatment efficacywas evaluated by two parameters:stable disease and disease progression in response evaluation criteria in solid tumors 1.1 criterion and prognostic factors were tested.RESULTS From 36 patients,55.6%were men,with a median age of 61.1±11.8 years.Regarding previous treatments,55.6%of patients underwent surgery of the primary tumor,100%of patients were treated with long-acting somatostatin analogues,66.7%of patients were treated with everolimus,27.8%of patients were treated with tyrosine kinase inhibitor,and 27.8%of patients were treated with interferon.One patient received radioembolization,three patients received chemoembolization,six patients received chemotherapy.Hematological toxicity was registered in 14 patients(G1-G2:55.5%and G3:3.1%).Other events presented were intestinal suboclusion in 4 cases,cholestasis in 2 cases and carcinoid crisis in 1 case.The median follow-up time was 3 years.Currently,24 patients completed treatment.Nineteen are alive with stable disease,two have disease progression,eight have died,and nine are still receiving treatment.The median overall survival was 12.5 mo(95%confidence interval range:9.8–15.2),being inversely proportional to toxicity in previous treatment 展开更多

Bio-inspired surfaces are usually designed by imitating the surface properties of a particular biological species,or combing with the surface characteristics of multiple biological species to construct a cross-species surface.Herein,inspired by the structure and rhythmic swing of human pulmonary cilia,and the directional migration of pigeons by geomagnetic field,an integrated system of biomimetic hydrophobic magnetic cilia array surface with reversible deformation properties and excellent magnetic response performance for solid microspheres underwater transport is designed and constructed.Driven by an external magnetic field,the magnetic responsive cilia array surface can directionally and continuously transport microspheres underwater in periodic motion.This work will shed new light on the designing of micromanipulation systems for micro-objects transport,and promote the practical application of micro-operating systems in underwater transportation and drug delivery. 展开更多

RtcB, a highly conserved RNA ligase, is found in all three domains of life, and demonstrated to be an essential tRNA splicing component in archaea and metazoans. However, the biological functions of RtcB in bacteria, where there is no splicing, remains to be clarified. We first performed bioinformatics analysis which revealed highly conserved structures and presumably conserved functions of RtcB in bacteria. However, its orthologs only occur in ~0.5% of bacterial species across diverse phyla with significant signals of frequent horizontal transfer, highlighting its non-essential role in bacteria. Next, by constructing an rtcBknockout strain, we find that the removal of antibiotic stress induces a significant impact on rtcB expression in wild-type strain,and furthermore the depletion of RtcB(?RtcB strain) delays the recovery process. Our transcriptomic analysis, comprising the3′-end labeling of RNAs, highlights a significant increase in unmapped reads and cleaved rRNAs in the ?RtcB strain, particularly during recovery. Our observations suggest that the conserved RNA ligase RtcB, repairs damaged r RNAs following stress,which potentially saves energy and accelerates recovery of its host. We propose that acquisition of RtcB by diverse bacterial taxa provides a competitive advantage under stressful conditions. 展开更多

目的分析腾冲市1例基孔肯雅热病例的发现、诊断、个案调查和处置情况,为基孔肯雅病热的防控提供依据。方法对腾冲市人民医院报告的1例疑似基孔肯雅热病例采集患者血液,用Real-time PCR法进行基孔肯雅病毒核酸检测;开展流行病学、临床表... 目的分析腾冲市1例基孔肯雅热病例的发现、诊断、个案调查和处置情况,为基孔肯雅病热的防控提供依据。方法对腾冲市人民医院报告的1例疑似基孔肯雅热病例采集患者血液,用Real-time PCR法进行基孔肯雅病毒核酸检测;开展流行病学、临床表现及诊疗过程、媒介伊蚊调查;采取环境整治、灭蚊、防蚊隔离治疗,追踪同行人员、密切接触者及家庭周围人群控制疫情。结果患者2019年7月31日出现发热、头疼、关节疼痛,8月1日出现皮疹,基孔肯雅热病毒核酸检测阳性,发病前有缅甸探亲史,确认为缅甸输入性病例,经对症支持治疗痊愈;同行人员、密切接触者及家庭周围人群未发现疑似症状者;滇滩岳家寨、猴桥及滇滩口岸均捕获白纹伊蚊,未发现埃及伊蚊,布雷图指数(BI)分别为4.00、9.80、3.85;灭蚊喷洒90余户,面积2.3万m2,处置后,布雷图指数(BI)控制在5以下。结论病例为腾冲市首例输入性基孔肯雅热,调查处置及时有效,未发生继发病例。 展开更多

In this investigation,we propose and analyze a virus dynamics model with multi-stages of infected cells.The model incorporates the effect of both humoral and cell-mediated immune responses.We consider two modes of transmissions,virus-to-cell and cell-to-cell.Multiple intracellular discrete-time delays have been integrated into the model.The incidence rate of infection as well as the generation and removal rates of all compartments are described by general nonlinear functions.Wc derive five threshold parameters which determine the existence of the equilibria of the model under consideration.A set of conditions on the general functions has been established which is sufficient to investigate the global stability of the five equilibria of the model.The global asymptotic stability of all equilibria is proven by utilizing Lyapunov function and LaSalle’s invariance principle.The theoretical results are illustrated by numerical simulations of the model with specific forms of the general functions. 展开更多

喀斯特地区土壤保持功能是衡量石漠化生态治理成效的重要手段之一,基于InVEST模型,利用"3S"技术对2005、2010、2015年的关岭-贞丰花江示范区的石漠化进行解译,研究石漠化与土壤保持量两者的响应机制。结果表明:(1)2005~2015... 喀斯特地区土壤保持功能是衡量石漠化生态治理成效的重要手段之一,基于InVEST模型,利用"3S"技术对2005、2010、2015年的关岭-贞丰花江示范区的石漠化进行解译,研究石漠化与土壤保持量两者的响应机制。结果表明:(1)2005~2015年石漠化程度总体减缓,面积由3 773.56 hm2减少至3 474.23 hm2,土壤保持总量相继上升,从29.29万t提高至37.29万t,2005~2010年单位面积土壤保持增量集中在6~30 t·hm-2·a-1,后五年增量在0~6 t·hm-2·a-1。(2)由于土地利用方式、坡度、土层厚度等差异,土壤保持功能并未呈现简单式下降,而是表现为强度>中度、轻度>潜在、非喀斯特>无石漠化。其中轻度与潜在差值最大,可达5.53 t·hm-2·a-1,说明了潜在石漠化是今后治理的重点。(3)2005~2010年石漠化恢复型土壤保持增量为10.27 t·hm-2·a-1,由于土地利用方式的转变以及治理时效性在2010~2015年降低为3.01 t·hm-2·a-1。稳定型石漠化变化土壤保持变化最为平稳,强度石漠化减缓区土壤保持功能上升快,轻度最为平缓,表明了治理期长且需循序渐进。因而,在治理过程中需考虑不同等级石漠化具有的土壤保持功能特性及其恢复的时效性。 展开更多

This paper describes model tests of single piles subjected to vertical cyclic compressive loading for three kinds of topography: sloping ground, level ground, and inclined bedrock. Comprehensive dynamic responses involving cyclic effects and vibration behaviours are studied under various load combinations of dynamic amplitude, mean load,frequency and number of cycles. Test results show that permanent settlement can generally be predicted with a quadratic function or power function of cycles.Sloping ground topography produces more pronounced settlement than level ground under the same load condition. For vibration behaviour,displacement amplitude is weakly affected by the number of cycles, while load amplitude significantly influences dynamic responses. Test results also reveal that increasing load amplitude intensifies nonlinearity and topography effects. The strain distribution in a pile and soil stress at the pile tip are displayed to investigate the vibration mechanism accounting for sloping ground effects. Furthermore, the dynamic characteristics among three kinds of topography in the elastic stage are studied using a three-dimensional finite method. Numerical results are validated by comparing with experimental results for base inclination topography. An inclined soil profile boundary causes non-axisymmetric resultant deformation, though a small difference in vertical displacement is observed. 展开更多

The liver is considered a vital organ and is the hub for multiple chemical functions,such as intermediary metabolism and the detoxification of ingested toxins,which are essential for the preservation of life,hence,the origin or the word“liver”.The liver has enormous,highly diversified catalytic potential.This enormous catalytic potential generates massive oxidative stress,which is important for the functions of the liver but is detrimental to the viability of the liver.The liver receives approximately 80%of its blood supply from the portal vein,which brings less saturated blood from the gastrointestinal tract.Hepatocytes operate in a relatively hypoxic microenvironment due to this portal inflow.The development of this hypoxic microenvironment of the liver is an important evolutionary adaptation for its detoxification function that is not recognized in the literature as a defence mechanism against the oxidative stress generated during the detoxification process.This review describes liver function in relation to its oxidative catalytic potential and the oxidative stress generated by it as well as the evolutionary defence mechanisms present in the liver against this oxidative stress to provide new insights into liver function. 展开更多

One of the most common cancers worldwide, colorectal cancer (CRC) has been associated with significant morbidity and mortality and therefore represents an enormous burden to the health care system. Recent advances in ... One of the most common cancers worldwide, colorectal cancer (CRC) has been associated with significant morbidity and mortality and therefore represents an enormous burden to the health care system. Recent advances in CRC treatments have provided patients with primary and metastatic CRC a better long-term prognosis. The presence of synchronous or metachronous metastasis has been associated, however, with worse survival. The most common site of metastatic disease is the liver. A variety of treatment modalities aimed at targeting colorectal liver metastases (CRLM) has been demonstrated to improve the prognosis of these patients. Loco-regional approaches such as surgical resection and tumor ablation (operative and percutaneous) can provide patients with a chance at long-term disease control and even cure in select populations. Patient selection is important in defining the most suitable treatment option for CRLM in order to provide the best possible survival benefit while avoiding unnecessary interventions and adverse events. Medical imaging plays a crucial role in evaluating the characteristics of CRLMs and disease resectability. Size of tumors, proximity to adjacent anatomical structures, and volume of the unaffected liver are among the most important imaging parameters to determine the suitability of patients for surgical management or other appropriate treatment approaches. We herein provide a comprehensive overview of current-state-of-the-art imaging in the management of CRLM, including staging, treatment planning, response and survival assessment, and post-treatment surveillance. Computed tomography (CT) scan and magnetic resonance imaging (MRI) are two most commonly used techniques, which can be used solely or in combination with functional imaging modalities such as positron emission tomography (PET) and diffusion weighted imaging (DWI). 目的:探讨代表心室激动时间延迟的类本位曲折间期对心脏再同步化治疗(CRT)反应性的预测价值。方法:连续入选兰州大学第一医院心脏中心2009年12月至2018年2月收治的71例成功植入CRT的心力衰竭患者,收集患者基线资料、体表心电图及超声心... 目的:探讨代表心室激动时间延迟的类本位曲折间期对心脏再同步化治疗(CRT)反应性的预测价值。方法:连续入选兰州大学第一医院心脏中心2009年12月至2018年2月收治的71例成功植入CRT的心力衰竭患者,收集患者基线资料、体表心电图及超声心动图指标。根据患者术后6个月左室收缩末期容积较术前降低≥15%,将患者分为CRT有反应组(n=47)和CRT无反应组(n=24),比较两组患者临床资料,采用logistic回归分析类本位曲折间期对CRT反应性的预测价值,通过受试者工作特征(ROC)曲线判断预测CRT有反应的最佳截断值截。结果:成功植入CRT的慢性心力衰竭患者中男性居多,扩张性心肌病是最常见的病因。CRT有反应组QRS缩短值(ΔQRSd)、术前及术后6个月射血分数的改变值(ΔLVEF)、术前V 5导联类本位曲折间期(ID V5)和V 6导联类本位曲折间期(ID V6)均大于CRT无反应组,差异有统计学意义(P均<0.05)。Logistic回归分析发现ΔLVEF(OR=1.07,95%CI:1.01~1.13,P=0.02)、ΔQRSd(OR=1.13,95%CI:1.07~1.19,P均<0.01)、术前ID V5(OR=1.11,95%CI:1.05~1.18,P均<0.01)和ID V6(OR=1.04,95%CI:1.01~1.07,P均<0.01)与CRT反应有相关性。ROC曲线示当ID V5为71 ms时,约登指数最大为0.390,ROC曲线下面积为0.791(95%CI:0.685~0.900,P均<0.01);当ID V6为79 ms时,约登指数最大为0.459,ROC曲线下面积为0.740(95%CI:0.599~0.880,P均<0.01)。结论:左室侧壁类本位曲折间期对CRT反应性具有预测价值,结合ΔQRSd值可进一步提高对CRT反应性的预测价值。 展开更多

As a non-invasive therapeutic method without penetration-depth limitation,magnetic hyperthermia therapy(MHT)under alternating magnetic field(AMF)is a clinically promising thermal therapy.However,the poor heating conversion efficiency and lack of stimulus–response obstruct the clinical application of magnetofluid-mediated MHT.Here,we develop a ferrimagnetic polyethylene glycol-poly(2-hexoxy-2-oxo-1,3,2-dioxaphospholane)(m PEG-b-PHEP)copolymer micelle loaded with hydrophobic iron oxide nanocubes and emodin(denoted as EMM).Besides an enhanced magnetic resonance(MR)contrast ability(r2=271 m M^-1 s^-1)due to the high magnetization,the specific absorption rate(2518 W/g at 35 k A/m)and intrinsic loss power(6.5 n Hm2/kg)of EMM are dozens of times higher than the clinically available iron oxide nanoagents(Feridex and Resovist),indicating the high heating conversion efficiency.Furthermore,this composite micelle with a flowable core exhibits a rapid response to magnetic hyperthermia,leading to an AMF-activated supersensitive drug release.With the high magnetic response,thermal sensitivity and magnetic targeting,this supersensitive ferrimagnetic nanocomposite realizes an above 70%tumor cell killing effect at an extremely low dosage(10μg Fe/m L),and the tumors on mice are completely eliminated after the combined MHT–chemotherapy. 展开更多

Removal of Mercury(Hg(Ⅱ)) from Seaweed Extracts by Electrodialysis and Process Optimization Using Response Surface Methodology 认领

In this work, response surface methodology(RSM) was employed to model and optimize electrodialysis process for mercury(Hg(Ⅱ)) removal from seaweed extracts. Box-Behnken design(BBD) was utilized to evaluate the effects and the interaction of influential variables such as operating voltage, influent flow rate, initial concentration of Hg(Ⅱ) on the removal rate of Hg(Ⅱ). The developed regression model for removal rate response was validated by analysis of variance, and presented a good agreement of the experimental data with the quadratic equation with high value coefficient of determination value(R2 = 0.9913, RAdj2 = 0.9678). The optimum operating parameters were determined as 7.17 V operating voltage, 72.54 L h-1 influent flow rate and 5.04 mg L-1 initial concentration of mercury. Hg(Ⅱ) removal rate of 76.45% was acquired under the optimum conditions, which showed good agreement with model-predicted(75.81%) result. The results revealed that electrodialysis can be considered as a promising strategy for removal of Hg(Ⅱ) from seaweed extracts. 展开更多

Objective:To investigate Phlebotomus(P.)sergenti Parrot,1917(Diptera:Psychodidae)salivary gland antigens and their immune response in human.Methods:Human volunteers were exposed to sand flies’bites in the laboratory,and following each exposure the size of induration was recorded.The mean protein concentration of salivary gland lysate and specific anti-P.sergenti saliva IgG was measured.Sand fly salivary proteins were separated by SDS-PAGE and their immunoreactivity was examined by Western blotting assays.Results:Individuals exposed to P.sergenti salivary gland lysate for 8 months showed both antibody and delayed type hypersensitivity responses,although exposure for one month did not provoke any immune responses.The trend of antibody fluctuated during the exposure time and dropped by the end of antigen loading.The mean protein content was(0.36±0.08)μg in each pair salivary glands.Salivary gland lysate showed 11 to 12 major protein bands and 3 to 6 of them were immunoreactive.Conclusions:Our study showed that the salivary gland components of P.sergenti provoked both cellular and humoral immune responses in human.Furthermore,there are some immunogenic proteins in P.sergenti saliva which could be subjected for further investigation as vector-based vaccine candidate/s against anthroponotic cutaneous leishmaniasis. 展开更多
