The incidence of gastric cancer ranks 4th in malignant tumors,and the mortality rate ranks 3rd in malignant tumors.Most patients with gastric cancer are already at the advanced stage when diagnosed.Although chemotherapy is the main treatment for patients with advanced gastric cancer,it can extend the overall survival.But adverse reactions are more prominent.With the rise of targeted therapy,the molecular mechanism of gastric cancer occurrence and development continues to develop,and molecular targeted therapy of gastric cancer has gradually emerged.The current targets for gastric cancer mainly include endothelial growth factor receptor,HER2,vascular endothelium growth factor,vascular endothelium growth factor receptor,mammalian rapamycin receptor,hepatocellular growth factor,et al.,but the clinical efficacy is still not satisfactory.At present,most of the studies on molecular targeted drugs for gastric cancer have ended in failure.The heterogeneity between patients and the prognosis of tumors in different parts may be different.It is suggested that screening targeted drugs according to the patient's pathological characteristics and molecular typing,and individualized treatment is the inevitable way of targeted treatment of gastric cancer. 展开更多

In clinical cancer research,it is quite promising to develop multimodal synergistic therapeutic strategies.Photodynamic and photothermal synergistic therapy is a very desirable multimodal therapy strategy.Herein,we report a facile and simple method to construct a nanotherapeutic agent for photodynamic and photothermal therapy.This nanotherapeutic agent(ZnO@Ce6-PDA)is composed of a ZnO nanoparticle core,an interlayer of photosensitizer chlorin e6(Ce6)and an outer layer of polydopamine(PDA).Due to the existence of Ce6,the ZnO@Ce6-PDA can efficiently generate singlet oxygen(1O2)under 660 nm laser irradiation.Moreover,the ZnO@Ce6-PDA can serve as a photothermal agent,because of the excellent photothermal conversion efficiency of the PDA coating layer in the presence of 780 nm laser.Experiment results demonstrated that the designed nanotherapeutic agent had outstanding phototoxicity upon the combination of laser irradiation at 660 and 780 nm.Thus,our work proves that the ZnO@Ce6-PDA is a promising photodynamic/photothermal dual-modal nanotherapeutic agent for enhanced cancer therapy. 展开更多

Diketopyrrolopyrrole(DPP)derivatived photosensitizers(PSs)with near infrared(NIR)absorption and good photophysical properties have drawn tremendous attention in cancer phototherapy.However,current DPP derivatives present unsatisfactory quantum yield of singlet oxygen(1O2)due to the large energy gap between the excited singlet and triplet states.To tackle this challenge,herein the DPP core is functionalized with triphenylphosphine-Au(I)group(Th DPP-Au),leading to a high1O2 quantum yield of 0.65.Theoretical calculation attributes the enhancement to spin-orbit coupling and population of the triplet excition upon photoexcitation.The hydrophilic Th DPP-Au nanoparticals(NPs)are prepared via nano-reprecipitation,which displays homogeneous size and excellent light absorption ability(ε=4.382×104 M-1cm-1).And the Th DPP-Au NPs exhibit low dark toxicity and high phototoxicity,which can effectively kill tumor cells via 1O2 induced mitochondrial apoptotic pathway upon irradiation.Furthermore,in vivo experiments demonstrate that Th DPP-Au NPs can selective accumulation in tumor and present excellent synergistic photodynamic/photothermal therapy guided by fluorescence and photothermal dual imaging. 展开更多

The main challenge in the field of Helicobacter pylori(H.pylori)infection is antibiotic resistance,which influences the efficacy of eradication regimens.Bismuth-containing quadruple therapy has been confirmed as an effective regimen for eradicating H.pylori,especially in strains with antibiotic resistance.High-dose proton-pump inhibitor-amoxicillin dual therapy could decrease the use of unnecessary antibiotics,which is a promising alternative approach.Adjuvant therapy(specific probiotic or vitamin)also showed good results,although more evidence is needed.Novel anti-H.pylori drugs are needed,and the establishment of the H.pylori database is an effective way to acknowledge the real-time information of H.pylori management.This review provides the recent progress of H.pylori treatment,and further studies are needed to address the role of different regimens in improving H.pylori eradication rate,especially in strains with antibiotics resistance. 展开更多

本文结合2001年波兰比亚韦斯托克肿瘤中心(BOC)医用电子加速器辐射事故及该机构的放射治疗设备概况,对波兰电离辐射安全监管体系进行了介绍,并对辐射事故过程、应急响应、IAEA救援、剂量评估、临床过程、结果和经验教训等方面进行了分... 本文结合2001年波兰比亚韦斯托克肿瘤中心(BOC)医用电子加速器辐射事故及该机构的放射治疗设备概况,对波兰电离辐射安全监管体系进行了介绍,并对辐射事故过程、应急响应、IAEA救援、剂量评估、临床过程、结果和经验教训等方面进行了分析和说明。实践表明,导致向患者输出剂量率比预期高许多倍的原因包括:医疗机构的供电不稳定,NEPTUN10P型医用加速器不符合IEC颁布的最新标准,电子枪灯丝电流限制值设置在较高的水平,束流监测系统故障,二极管故障,安全联锁失效,显示屏剂量率低于实际值。IAEA援助小组的建议与援助、剂量评估以及良好的医疗条件为患者提供了医疗保障。本文可作为辐射事故应急的参考。 展开更多

Extracellular vesicles,including exosomes and microvesicles,play a fundamental role in the activity of the nervous system,participating in signal transmission between neurons and providing the interaction of central nervous system with all body systems.In many neurodegenerative diseases,neurons pack toxic substances into vesicles and release them into the extracellular space,which leads to the spread of misfolded neurotoxic proteins.The contents of neuron-derived extracellular vesicles may indicate pathological changes in the central nervous system,and the analysis of extracellular vesicle molecular content contributes to the development of non-invasive methods for the diagnosis of many central nervous system diseases.Extracellular vesicles of neuronal origin can be isolated from various biological fluids due to their ability to cross the blood-brain barrier.Today,the diagnostic potential of almost all toxic proteins involved in nervous system disease pathogenesis,specificallyα-synuclein,tau protein,superoxide dismutase 1,FUS,leucine-rich repeat kinase 2,as well as some synaptic proteins,has been well evidenced.Special attention is paid to extracellular RNAs mostly associated with extracellular vesicles,which are important in the onset and development of many neurodegenerative diseases.Depending on parental cell type,extracellular vesicles may have different therapeutic properties,including neuroprotective,regenerative,and anti-inflammatory.Due to nano size,biosafety,ability to cross the blood-brain barrier,possibility of targeted delivery and the lack of an immune response,extracellular vesicles are a promising vehicle for the delivery of therapeutic substances for the treatment of neurodegenerative diseases and drug delivery to the brain.This review describes modern approaches of diagnosis and treatment of central nervous system diseases using extracellular vesicles. 展开更多

无明确产前诊断的新生儿先天性膈疝的临床诊治体会

目的总结产前未明确诊断的新生儿先天性膈疝(congenital diaphragmatic hernia,CDH)病例的临床特点、治疗方法和预后情况,分析非母胎诊治中心对新生儿CDH的临床诊治经验和体会。方法收集郑州大学附属儿童医院2010年1月至2018年12月急诊... 目的总结产前未明确诊断的新生儿先天性膈疝(congenital diaphragmatic hernia,CDH)病例的临床特点、治疗方法和预后情况,分析非母胎诊治中心对新生儿CDH的临床诊治经验和体会。方法收集郑州大学附属儿童医院2010年1月至2018年12月急诊收治的136例新生儿CDH病例资料,5例术前因家属原因未接受治疗,未纳入研究,余131例患儿纳入研究。131例患儿均无产前诊断,其中男77例,女54例,出生时胎龄(38.3±1.5)周,范围32.0~41.0周,出生体重(3102.8±473.6)g,范围2000.0~4120.0 g。左侧膈疝115例,右侧膈疝16例,合并肺动脉高压21例,伴发畸形81例。92例术前需机械通气,39例术前可维持自主呼吸。依据急诊入院时的日龄分为:≤48 h入院组(90例),>48 h入院组(41例)。结果经胸腔镜手术58例,开腹手术71例。共计死亡14例,其中术前死亡2例,术后死亡12例。术前抢救治疗无效死亡2例,接受手术治疗的患儿总计129例,术后存活117例,12例死亡或放弃治疗,接受治疗病例的总存活率为89.3%(117/131)。≤48 h入院组的病死率为15.6%(14/90),高于>48 h入院组的病死率为0(P<0.05)。早产儿、合并肺动脉高压以及严重心脏畸形病例的病死率相对较高(P<0.05),死亡病例的入院pH值、PaO2、术后平均pH值明显低于存活病例,入院PaCO2明显高于存活病例(P<0.05)。结论无产前诊断的新生儿CDH产后发病越早,病死率越高;合并肺动脉高压、早产和严重心脏畸形的CDH新生儿病死率较高。 展开更多

Alzheimer’s disease is a common progressive neurodegenerative disorder, pathologically characterized by the presence of β-amyloid plaques and neurofibrillary tangles. Current treatment approaches using drugs only alleviate the symptoms without curing the disease, which is a serious issue and influences the quality of life of the patients and their caregivers. In recent years, stem cell technology has provided new insights into the treatment of neurodegenerative diseases, including Alzheimer’s disease, Parkinson’s disease, and amyotrophic lateral sclerosis. Currently, the main sources of stem cells include neural stem cells, embryonic stem cells, mesenchymal stem cells, and induced pluripotent stem cells. In this review, we discuss the pathophysiology and general treatment of Alzheimer’s disease, and the current state of stem cell transplantation in the treatment of Alzheimer’s disease. 在缺血性卒中的病理过程中,免疫系统通过固有免疫反应和适应性免疫反应发挥着重要的作用。缺血性卒中发生后,一方面通过激活的免疫细胞释放破坏性细胞因子,损伤血管内皮,破坏血脑屏障和神经元,并在淋巴细胞与特异性抗原结合后诱发适应... 在缺血性卒中的病理过程中,免疫系统通过固有免疫反应和适应性免疫反应发挥着重要的作用。缺血性卒中发生后,一方面通过激活的免疫细胞释放破坏性细胞因子,损伤血管内皮,破坏血脑屏障和神经元,并在淋巴细胞与特异性抗原结合后诱发适应性免疫反应,加重神经元损伤;另一方面通过清除坏死组织,释放保护性细胞因子,调节免疫抑制等途径减轻神经元损伤。免疫调节治疗为卒中的治疗提供了一个新的方向,以芬戈莫德、米诺环素、那他珠单抗等药物为代表的免疫调节药物在缺血性卒中动物模型实验和临床研究中都呈现出较好的疗效。本文就缺血性卒中免疫调节治疗的研究进展做一综述。 展开更多

BACKGROUND Preoperative chemoradiotherapy regimens using a second drug for locally advanced rectal cancer are still under clinical investigation.AIM To investigate the clinical outcomes of patients with locally advanced rectal cancer treated with preoperative chemoradiotherapy using tegafur/gimeracil/oteracil(S-1)plus irinotecan(CPT-11).METHODS This was a single-center retrospective study of 82 patients who underwent radical surgery for rectal cancer after chemoradiotherapy with S-1(80 mg/m2/d),CPT-11(60 mg/m2/d),and radiation(total 45 Gy)between 2009 and 2016.The median follow-up was 51 mo(range:17–116 mo).RESULTS Twenty-nine patients(35.4%)had T3 or T4 rectal cancer with mesorectal fascia invasion,36(43.9%)had extramural vascular invasion,24(29.8%)had N2 rectal cancer and eight(9.8%)had lateral lymph node swelling.The relative dose intensity was 90.1%for S-1 and 92.9%for CPT-11.Seventy-nine patients(96.3%)underwent R0 resection.With regard to pathological response,13 patients(15.9%)had a pathological complete response and 52(63.4%)a good response(tumor regression grade 2/3).The 5-year local recurrence-free survival,relapsefree survival and overall survival rates were 90.1%,72.5%and 91.3%,respectively.We analyzed the risk factors for local recurrence-free survival by Cox regression analysis and none were detected.Previously described risk factors such as T4 stage,mesorectal fascia invasion or lateral lymph node swelling were not detected as negative factors for local recurrence-free survival.CONCLUSION We demonstrated good compliance and favorable tumor regression in patients with locally advanced rectal cancer treated with preoperative S-1 and CPT-11. 展开更多
